On August 23rd at 9:45 the conditions are good, sunny outside around 92 degrees. Work area was well lit with little debris on the ground. EE was working on the hammer mill crusher trying to mock up the grates for repairs. When the soft sling EE was using wasn't choked all the way slid off center smashing EE's hand between the hammer mill crusher frame and the grate.

Showing all 17Individual was attempting to clear what was thought or appeared to be an obstruction on a conveyor belt roller. Individual used the handle of a hammer to knock away what appeared to be mud but was a hole in the roller. The handle was caught in the hole and slammed EE's hand into the conveyor frame.
While using sledge hammer to close crusher a slice of metal flew off of steel and struck employee in chest.
Strained back working with sledge hammer on plant. Employee did not want to seek medical care until 27July11.

EMPLOYEE WAS REACHING FOR A 3/4" RATCHETT. IT FELL HITTING HIM ABOVE HIS RIGHT EYE.
EMPLOYEE WAS HELPING REPLACE A WEAR PLATE ON A LOADER BUCKET. WHEN HE HIT THE PLATE WITH A SLEDGE HAMMER, A PIECE CHIPPED OFF THE PLATE AND HIT HIM IN THE CHEST.
EE WAS HITTING BUCKET TOOTH ON A FRONT END LOADER WITH A SLEDGE HAMMER AND A PIECE CHIPPED OFF THE TOOTH HITTING HIS LEG.
EE WAS HOLDING A WEDGE WHILE ANOTHER EE HIT IT WITH A SLEDGEHAMMER. HE GOT HIS FINGER BETWEEN THE WEDGE AND THE SLEDGEHAMMER AND SMASHED THE TIP OF HIS RING FINGER.
EE WAS CUTTING A SECTION OF CONVEYOR BELT WITH A BOX KNIFE. THE KNIFE CAME APART AND THE BLADE CUT HIS FINGER.
EE WAS REPLACING A WHEEL ON A TRUCK AND HURT HIS BACK PULLING ON A WRENCH. HE CONTINUED TO WORK UNTIL11-07-01 WHEN HE BEGAN LOSTING TIME FROM WORK.
EE CUT A COTTER PIN WITH A PAIR OF PLIERS, THE CUT END OF THE PIN FLEW UP AND EMBEDDED IN HIS EYE.
WRENCH SLIPPED-EE HIT TOP OF HAND ON METAL BRACKET. CUT TOP OF HAND.
