10/10/25 around 9:50am nice day sunny temp was 70dgrees. blasting crew was setting off a shot when fly rock struck the blast shelter and scared EE into a panic attack.
EE was filling hydraulic oil in 651 scraper when hand got smashed by the 55 gal. drum. weather 79 degrees wind was blowing 3mph.
Overcast skies, temperature is 70 degrees, little to no wind, work area clean of debris with a little buildup in the chute. Technician was welding in the chute, lost balance and fell to the screen below.

On August 23rd at 9:45 the conditions are good, sunny outside around 92 degrees. Work area was well lit with little debris on the ground. EE was working on the hammer mill crusher trying to mock up the grates for repairs. When the soft sling EE was using wasn't choked all the way slid off center smashing EE's hand between the hammer mill crusher frame and the grate.
cloudy day EE was in the 170 box car working on a gearbox, the are was clean off debris and was well lite. EE turned to get/look at another component when the gearbox fell striking right above EE's steal toe boot on EE's left foot.
it was sunny day. they where working to replace a hammer on the crusher. the alignment rod came out and struck the right hand, index finger of the worker. worker was wearing gloves.

Fueling haul truck tripped over fuel hose causing deep bone bruise on right forearm.
Individual was attempting to clear what was thought or appeared to be an obstruction on a conveyor belt roller. Individual used the handle of a hammer to knock away what appeared to be mud but was a hole in the roller. The handle was caught in the hole and slammed EE's hand into the conveyor frame.
While operating Terex 100 ton haul truck seat bottomed out and a bump and jammed EE's back

Employee was attempting to open rail-hopper car gate. The tool he was attemting to use slipped causing him to fall hitting his head. He was taken to ER - given a CAT scan and released to work the following day.

Operator had drilled approx. 16 feet of a 45 foot face when he noticed that part of the face had caved off to his right. He raised the drill stem and began to raise the outriggers when the drill slowly slid off the face and turned on its side. Since the boom was fully extended, it stopped the drill on the caved off material 20 feet from the floor.
EE slipped on a rock and sprained his ankle.
EE was standing on the ladder of the maintainer when his knee popped and began to hurt.

Employee was rewiring a backup alarm on haul truck #132. He started to step down from the frame of the truck and his right foot folded under on a lump of dirt and sprained his ankle.
An employee was loaded and headed east going to the scalehouse when he saw a train on the south end of Old Hole, and said he looked to his left and R50 Euclid was too close to the crossing and he did not have time to stop.
Welding lifting eye on railcar and eye broke loose and hit employee on cheek.
Was welding on a chute at the secondary plant when he slipped and landed on his left foot and twisted his ankle.
Employee was checking fluid levels on truck #773 (100 ton truck). When he finished he started down the ladder on the truck and pulled a muscle in his back.
Screening plant was down because of screen over flowing and all truck drivers, scraper operators and plant operators were trying to shovel off excess, when a rock fell from the top of the screen to the bottom, where EE was.
EE was welding in the same area as two other welders and flash burned his eyes.

EE bent over to pick up an object on the ground and the door to a tool box mounted on a truck fell open as he stood up, hitting him on top of the head.

EE WAS OPERATING A SCRAPER AND STATED HIS BACK BEGAN HURTING EARLY IN THE WEEK AND CONTINUED TO GET WORSE AND THAT THE CAUSE WAS ROUGH ROADS, JERKY SHIFTING AND A BROKEN SEAT.
EE WAS CUTTING A SECTION OF CONVEYOR BELT WITH A BOX KNIFE. THE KNIFE CAME APART AND THE BLADE CUT HIS FINGER.
EE WAS REPLACING A WHEEL ON A TRUCK AND HURT HIS BACK PULLING ON A WRENCH. HE CONTINUED TO WORK UNTIL11-07-01 WHEN HE BEGAN LOSTING TIME FROM WORK.
A CUSTOMER DRIVER REACHED INTO THE TICKET CHUTE AT THE TRUCK SCALE AND AT THE SAME TIME TOOK HISFOOT OFF THE BRAKE PEDAL. THE TRUCK ROLLED FORWARD AND HIS RING ON HIS LEFT HAND GOT CAUGHT ON A FLAP AT THE END OF THE CHUTE. THE RING WAS FLATTENED DOWN AND CUT HIS FINGER.
THE EMPLOYEE WAS INSTALLING A PIN CUP ON A FRONTEND LOADER BUCKET. THE CUP FELL AND PINCHED HIS FINGERS BETWEEN THE CUP AND THE SHOP FLOOR.
SHORTLY AFTER ARRIVING AT WORK AND BEFORE THE SHIFT STARTED, THE EE BEGAN HAVING CHEST PAINS. HE WAS TAKEN TO THE HOSPITAL BY AMBULANCE AND DIAGNOSED WITH AN ANXIETLY ATTACK. HE RETURNED LATER AND RESIGNED.
EE SLIPPED AND FELL ON STEPS AND LANDED ON HIS LEFT SIDE.
EMPLOYEE WAS WELDING WITH AN ARC WELDER, LIGHT FROM THE ARC CAUSED SKIN BURNS WHICH BECAME INFECTED.
