EE was working in the welding stall clean of debris around 48 degrees. EE was working on hydraulic when EE smashed EE's right hand.

EE was walking to the water pump when EE felt a pain in EE's right leg area was clean of debris and it was just getting dark out

Employee was working on 154 plant weather was sunny temp was 82 degrees. the plant was clean of debris. Employee was bent over went to stand struck head on I-beam

10/10/25 around 9:50am nice day sunny temp was 70dgrees. blasting crew was setting off a shot when fly rock struck the blast shelter and scared EE into a panic attack.

EE was filling hydraulic oil in 651 scraper when hand got smashed by the 55 gal. drum. weather 79 degrees wind was blowing 3mph.

Overcast skies, temperature is 70 degrees, little to no wind, work area clean of debris with a little buildup in the chute. Technician was welding in the chute, lost balance and fell to the screen below.

Temperature 76 d sunny wind16 mph employee was working in the shop with the door open little to no debris on the ground and a slight dust covering the ground. Employee was tightening bolts on the crusher. The air hose slipped off the fitting

EE was working removing grates from the crusher, the area was clean of debris and was dry, sun was shining temp around 68 degrees good conditions

Little to no debris on or around accident

On August 23rd at 9:45 the conditions are good, sunny outside around 92 degrees. Work area was well lit with little debris on the ground. EE was working on the hammer mill crusher trying to mock up the grates for repairs. When the soft sling EE was using wasn't choked all the way slid off center smashing EE's hand between the hammer mill crusher frame and the grate.

cloudy day EE was in the 170 box car working on a gearbox, the are was clean off debris and was well lite. EE turned to get/look at another component when the gearbox fell striking right above EE's steal toe boot on EE's left foot.

it was sunny day. they where working to replace a hammer on the crusher. the alignment rod came out and struck the right hand, index finger of the worker. worker was wearing gloves.

Clear, dry, open ground. Operator of haul truck failed to maintain proper control of EE's equipment.

Fueling haul truck tripped over fuel hose causing deep bone bruise on right forearm.

Individual was attempting to clear what was thought or appeared to be an obstruction on a conveyor belt roller. Individual used the handle of a hammer to knock away what appeared to be mud but was a hole in the roller. The handle was caught in the hole and slammed EE's hand into the conveyor frame.

While operating Terex 100 ton haul truck seat bottomed out and a bump and jammed EE's back

Employee was changing a bearing on a haul truck and bearing fell on right hand.

Employee was climbing onto water truck and the swing step swung out on them causing the employee to strain their knee

Walked under scraper while it was lowering and hood struck shoulder.

While stepping off ladder of water truck twisted knee and tore MCL. Note: employee failed to notify company until 23 Aug 2018

While operating a Volvo scraper coming off pile, turned left didn't see another Volvo scraper and hit the other scraper. As a result left window shattered causing cuts to EE's left elbow requiring stitches. Also bruised left hip.

Strained left shoulder while lifting bucket. Note: Complications arose several days later. The company at first didn't know they were related.

EE was climbing down water truck. Someone tossed gloves to EE. EE reached out to catch them, lost balance and fell.

Raining - employee climbed on bumper of pickup, slipped off bumper and fell off, landing on back.

Cleaning a shoot and dirt flew into face, particles scratched left eye. Individual was on top of screen when air blew particles into his face. Individual was not wearing safety glasses.

Employee was attempting to remove belt from pulley while pulley still in motion. Index finger on left hand was fractured.

Working on bus strained L knee climbing into drivers seat

While using sledge hammer to close crusher a slice of metal flew off of steel and struck employee in chest.

Hydraulic cylinder fell on left thumb.

Tripped carrying water cooler strained Left shoulder.

Slip and fall. Broken ribs.

Corneal foreign body

Strained right knee climbing on mobile equipment.

Strained back working with sledge hammer on plant. Employee did not want to seek medical care until 27July11.

Working in shop stabbed himself with screw driver.

Welder box lid fell on EE's right arm.

Step on hammer broke while employee was climbing on machine tore ligaments in his right ankle. Not reported until 27 of April 2011.

Was reaching up to retrieve parts from parts bend. Pulled groin. Double hernia. Happened on 16 March 2011, reported 24 March 2011.

Twisted knee climbing off equipment.

Lifting ladder strained his back.

Foreign body flew behind safety glasses.

Using shovel to clean crusher pulled back.

EE was cleaning superfine screen. EE cut his wrist on angle iron under frame.

Was helping to replace a head pulley guard, pulled groin, possible hernia. Accident happend on 6/25/09, reported on 7/2/09

Employee was attempting to open rail-hopper car gate. The tool he was attemting to use slipped causing him to fall hitting his head. He was taken to ER - given a CAT scan and released to work the following day.

Operator had drilled approx. 16 feet of a 45 foot face when he noticed that part of the face had caved off to his right. He raised the drill stem and began to raise the outriggers when the drill slowly slid off the face and turned on its side. Since the boom was fully extended, it stopped the drill on the caved off material 20 feet from the floor.

EE slipped on a rock and sprained his ankle.

EE was standing on the ladder of the maintainer when his knee popped and began to hurt.

Employee was rewiring a backup alarm on haul truck #132. He started to step down from the frame of the truck and his right foot folded under on a lump of dirt and sprained his ankle.

An employee was loaded and headed east going to the scalehouse when he saw a train on the south end of Old Hole, and said he looked to his left and R50 Euclid was too close to the crossing and he did not have time to stop.

Welding lifting eye on railcar and eye broke loose and hit employee on cheek.

Was welding on a chute at the secondary plant when he slipped and landed on his left foot and twisted his ankle.

Employee was checking fluid levels on truck #773 (100 ton truck). When he finished he started down the ladder on the truck and pulled a muscle in his back.

Screening plant was down because of screen over flowing and all truck drivers, scraper operators and plant operators were trying to shovel off excess, when a rock fell from the top of the screen to the bottom, where EE was.

EE was welding in the same area as two other welders and flash burned his eyes.

Employee was pulling on a wrench & hurt his back.

EE bent over to pick up an object on the ground and the door to a tool box mounted on a truck fell open as he stood up, hitting him on top of the head.

Employee was working inside the crusher on his hands and knees on a steel surface and bruised his knee.

Employee was holding a steel plate in place while another employee cut it with an acetylene torch. The torch slipped and burned his arm.

EE was climbing down a ladder of a front end loader. As he stepped to the ground, he slipped and felt a pain in his lower back that continued to get worse. He went to the doctor one week later.

EE was sitting in his truck, a front end loader picked up thr truck tailgate and dropped it.

EE was pulling a tub of oil from under a loader, the row rod pulled loose and he fell backward landing on his wrist.

EE WAS PRYING WITH A BAR, THE BAR SLIPPED AND HIT HIM ON THE LEFT TEMPLE.

Trying to turn on light in the shop and tripped over a platform scale in the dark and fell.

Employee was picking up a M length of pipe and felt a sharp pain in his back and was unable to stand up.

EMPLOYEE WAS CARRYING A FIVE GALLON BUCKET OF GREASE & FELT A PAIN IN HIS LOWER BACK WHICH GOT WORSE OVER THE WEEKEND.

EMPLOYEE WAS SHOVELING ROCK & THROWING IT OVER HIS LEFT SHOULDER. HE STRAINED A MUSCLE IN HIS UPPER BACK.

Employee was lifting a welding machine out of the crusher and felt a pain in his lower back.

EMPLOYEE WAS REPLACING AN OXYGEN CYLINDER ON A DOLLY AND PINCHED A FINGER BETWEEN THE BOTTLE AND THE DOLLY.

EMPLOYEE WAS REPLACING A GRILL ON A TRUCK. IT SLIPPED AND FELL CUTTING HIS FINGERS.

EE BACKED A DOZER OVER A 24 INCH LEDGE AND JARRED HIS NECK. HE FELT VERITGO THE NEXT DAY (FRIDAY) AND HAD A STROKE SATURDAY. CLAIMED INJURY ON 5-11-04. 6/28/04: BASED ON MEDICAL INFORMATION THIS WAS NOT AN INJURY CAUSED BY EMPLOYMENT, BUT A NATURAL PROGRESSION OF PRE-EXISTING CONDITION.

EE WAS FUELING UP A LOADER & FELT SOMETHING BITE HIS LEG. BITE BECAME SWOLLEN & PAINFULL THE NEXT DAY.

EE WAS REMOVING A TEMPERATURE GUAGE SENDING UNIT ON AN ENGINE AND HOT COOLANT SPRAYED OUT.

EE WAS TURNING THE CRUSHER IMPELLER WITH HIS FEET AND STRAINED HIS BACK.

EE WAS THROWING A LARGE ROCK OUT OF THE CRUSHER IT BOUNCED BACK AND MASHED HIS FINGER.

EMPLOYEE WAS CUTTING A CYLINDER WITH A TORCH, HYDRAULIC OIL SPRAYED AND IGNITED CAUSING A FLASH FIRE.

EE WAS WORKING IN THE PLANT AREA AND FELT SOMETHING BLOW INTO HIS EYE.

EE WAS HITCHING A LIGHT PLANT TO A PICKUP AND SPRAINED HIS BACK LIFTING ON THE HITCH.

EE WAS REPAIRING A CHUTE IN THE PLANT AND STRAINED A MUSCLE IN HIS LEG.

EMPLOYEE WAS CUTTING A PIECE OF CONVEYOR BELT WITH A BOX KNIFE AND CUT HIS HAND.

EMPLOYEE WAS REACHING FOR A 3/4"" RATCHETT. IT FELL HITTING HIM ABOVE HIS RIGHT EYE.

EE MISSED A STEP ON THE TRUCK LADDER AND FELL TO THE GROUND.

EMPLOYEE WAS PULLING ON A LOCOMOTIVE BRAKE HANDLE AND FELT PAIN IN HIS LOWER BACK THAT CONTINUED TO WORSEN.

EMPLOYEE WAS HELPING REPLACE A WEAR PLATE ON A LOADER BUCKET. WHEN HE HIT THE PLATE WITH A SLEDGE HAMMER, A PIECE CHIPPED OFF THE PLATE AND HIT HIM IN THE CHEST.

EMPLOYEE WAS WELDING IN THE CRUSHER. HE SLIPPED & FELL ONTO HIS KNEE. SEPTIC BURSITIS DEVELOPED SHORLY THERAFTER.

EE WAS GETTING UNDER A FRONT END LOADER TO DO MAINTENANCE WORK AND HURT HIS BACK.

EE WAS HITTING BUCKET TOOTH ON A FRONT END LOADER WITH A SLEDGE HAMMER AND A PIECE CHIPPED OFF THE TOOTH HITTING HIS LEG.

EE WAS HOLDING A WEDGE WHILE ANOTHER EE HIT IT WITH A SLEDGEHAMMER. HE GOT HIS FINGER BETWEEN THE WEDGE AND THE SLEDGEHAMMER AND SMASHED THE TIP OF HIS RING FINGER.

EE HAD JUST FINISHED FUELING A TRUCK-AS HE STARTED TO DRIVE OFF, HE REALIZED THE FOLD DOWN PLATFORM AT THE FUEL TANK WAS STILL DOWN-HE PUT THE TRUCK IN NEUTRAL AND SET THE PARKING BRAKE-AS HE STEPPED OUT ON THE TRUCK LADDER HE HIT THE GEAR SHIFT WITH HIS ELBOW AND THE TRUCK MOVE BACKWARD ABOUT ONE FOOT AND PINNED HIS LEFT FOOT BETWEEN THE LADDER AND THE PLATFORM.

EE WAS OPERATING A SCRAPER AND STATED HIS BACK BEGAN HURTING EARLY IN THE WEEK AND CONTINUED TO GET WORSE AND THAT THE CAUSE WAS ROUGH ROADS, JERKY SHIFTING AND A BROKEN SEAT.

EE WAS CUTTING A SECTION OF CONVEYOR BELT WITH A BOX KNIFE. THE KNIFE CAME APART AND THE BLADE CUT HIS FINGER.

EE WAS REPLACING A WHEEL ON A TRUCK AND HURT HIS BACK PULLING ON A WRENCH. HE CONTINUED TO WORK UNTIL11-07-01 WHEN HE BEGAN LOSTING TIME FROM WORK.

A CUSTOMER DRIVER REACHED INTO THE TICKET CHUTE AT THE TRUCK SCALE AND AT THE SAME TIME TOOK HISFOOT OFF THE BRAKE PEDAL. THE TRUCK ROLLED FORWARD AND HIS RING ON HIS LEFT HAND GOT CAUGHT ON A FLAP AT THE END OF THE CHUTE. THE RING WAS FLATTENED DOWN AND CUT HIS FINGER.

THE EMPLOYEE WAS INSTALLING A PIN CUP ON A FRONTEND LOADER BUCKET. THE CUP FELL AND PINCHED HIS FINGERS BETWEEN THE CUP AND THE SHOP FLOOR.

SHORTLY AFTER ARRIVING AT WORK AND BEFORE THE SHIFT STARTED, THE EE BEGAN HAVING CHEST PAINS. HE WAS TAKEN TO THE HOSPITAL BY AMBULANCE AND DIAGNOSED WITH AN ANXIETLY ATTACK. HE RETURNED LATER AND RESIGNED.

EE SLIPPED AND FELL ON STEPS AND LANDED ON HIS LEFT SIDE.

EMPLOYEE WAS WELDING WITH AN ARC WELDER, LIGHT FROM THE ARC CAUSED SKIN BURNS WHICH BECAME INFECTED.

EE WAS CLIMBING DOWN A TRUCK LADDER WHEN A RING HE WAS WEARING CAUGHT ON A STEP AS HE STEPPED OFTHE LADDER.

PARTICLE OF STEEL BLEW INTO EYE WHILE EE WS IN A BACKHOE.

THE EMPLOYEE WAS DUMPING A TRUCK. THE TRUCK SLIPPED OVER THE EDGE OF THE BANK AND TILTED. HE HITHIS KNEE ON THE GEAR SHIFT AND HIS SHOULDER ON THE SIDE OF THE CAB.

A CUSTOMER STOPPED HIS TRUCK BEHIND A FRONT END LOADER THAT WAS LOADING TRUCKS FROM A STOCKPILE.THE LOADER BACKED UP APPROX THE LENGTH OF THE LOADER AND HIT THE LEFT SIDE OF THE TRUCK CAB, MOV ING THE TRUCK APPROX 1 FOOT. THE TRUCK DRIVER DID NOT APPEAR TO BE INJURED BUT LATER REQUESTED AN AMBULANCE.

EMPLOYEE WAS LIFTING A SCREEN AND FELT A PAIN INHIS GROIN.

A CUSTOMER WAS ON TOP OF HIS LOADED TRUCK AT THE SCALE AREA AND FELL OFF.

EE CUT A COTTER PIN WITH A PAIR OF PLIERS, THE CUT END OF THE PIN FLEW UP AND EMBEDDED IN HIS EYE.

WRENCH SLIPPED-EE HIT TOP OF HAND ON METAL BRACKET. CUT TOP OF HAND.

EMPLOYEE WAS SHOVELING MATERIAL INTO A DRILL HOLE AND FELT A PAIN. REPORTED INJURY 6/9/00.

A CUSTOMER'S HIGHWAY TRUCK STOPPED BEHIND A FRONT END LOADER AS IT WAS BACKING UP. THE LOADER BACKED INTO THE TRUCK TRAILER CAUSING DAMAGE TO THE TRUCK CYLINDER HOIST FRAME. THE DRIVER DID NOT APPEAR TO BE INJURED AFTER THE ACCIDENT BUT NOW IS CLAIMING AN INJURY.
