Showing all 16Individual was attempting to loosen a screw conveyor, while going through equipment checkout. procedure was to pull towards them with the pipe wrench. The screw would not loosen, subject attempted to move it in the opposite direction. The pipe wrench slipped off and individual fell forward injuring his arm with a minor laceration.
