EE thinks lower back pain is due to EE swinging a 20# sledgehammer driving a pin out of an outrigger on a forklift. EE thinks the torque of the swings resulted in a tightness in lower back that became painful a few days later.

EE was cutting a hole in belting when the knife went through the belting while being drawn over the belting and cut the EE's right thigh.

Employee was cutting an old splice from a belt and when the knife went through the end of the belt the blade turned (due to a sag in the belt) cutting the employee's thumb.

EE was placing a roll of belting (to be used on a conveyor) onto the fork lift and when he removed his pry bar it pinched his finger. (Pry bar did the pinching). The fork lift was not in motion, cut caused by the pry bar.

A U-belt had jumped a pulley and EE was pulling on the belt while another employee was prying the belt over to the next pulley when the shaft went over center catching EE's hand between the belt and the pulley.

EE WAS LOOSENING A PIPE WITH A PIPE WRENCH WHEN HE FELT A POP IN HIS RIGHT SHOULDER. HE DIDN'T THINK HE WAS REALLY HURT SO DID NOT REPORT THE INCIDENT TO THE OFFICE. HE SAID IT WORSENED TIL HE WENT TO THE DR. WHO TOLD HIM HE HAD DAMAGED HIS ROTATOR CUFF. HE REPORTED THE ACCIDENT ON 12-12-01.
