Struck by flying object

Employee was removing a belt wipe for replacement and was holding an end wrench on a stuck nut while another employee used an electric impactor to the bolt end for easy removal. The injured employee was holding the wrench loosely and the wrench spun with the nut striking the end of right thumb, fracturing themost distal phalange.

Fall onto or against objects

Employee was cleaning the pads around the jaw crusher when they stepped into some mud and slipped causing them to fall into a steel supporting structure causing a small cut on their scalp. Employee stated their hard hat came off as their head went forward.

Struck by falling object

Lying on back grinding metal from a screen deck and debris entered EE's left eye. Employee was wearing a shield and safety glasses. The condition did not bother EE until a day and a half later. EE never missed work but was restricted for one day. (No operating equipment and out of any dust)

Employee was positioning a shim beneath a toggle plate on a feeder and pulled a muscle while positioning it. Employee's initial diagnosis was a pulled muscle and on employee's return visit to the clinic the doctor gave employee a steroid injection. The employee has not missed any work and is still performing regular job.

Employee was shoveling spilled crushed white rock from beneath a belt at the main plant (build up on concrete surface) and when they forcefully pushed the shovel into the rock they ""tweaked"" the left shoulder.

Operator was climbing up the stairs of the loader to check out some recent damage to the machine and when the employee reached up to feel the metal on the hood, there was a sharp piece that had resulted from a recent fender bender that cut employee's right hand on the dorsal surface. (The machine was a Komatsu a WD-500.

EE was assisting another ee in standing up a 85 gallon drum of transmission fluid (drum was on its side) when ee felt a pain in lower back. EE had left hand on the top rim of the drum and right hand on the bottom lip. The other ee was reversed.

Employee was welding and when replacing the argon tank, it tipped and began to fall. The employee reached for the tank at arms length and strained back muscles.

Employee was shoveling loose rock from beneath belt when he felt his shoulder pop. Was in the act of forcing the shovel into the loose rock. (Shift end maintenance).

EE was doing a pre-op check and tripped over a wheel chock. When he fell he caught himself with his right hand causing it to twist. He reported that it was nothing and refused to go to medical facility. 3 days later the wrist began to bother him so he went to dr. He was allowed to RTW with no restrictions but will have to have surgery. Will become reportable on 10/22/13, surgery.

Employee was holding a 25# steel wear plate while another employee was lining up the bolt holes. The other employee miscommunication with the injured employee and pulled the trigger on the pneumatic air wrench pinching injury's finger.

Flash burns (electric)

Employee was replacing a fuse in the jaw disconnect box. After throwing the knife in the box, the employee used a fuse puller to remove the bad fuse. When replacing the fuse, the employee got the fuse holder into live electricity and got flashed.

EE was replacing a u-belt onto a pulley and when he was trying to slip the belt over the pulley his fingers (index & middle) got caught between the pulley and the v-belt pinching ee's fingers.

EE was grinding the head off a bolt in a belt splice when the finger dug into the belt and grabbed his right hand making a 1/2"" cut into his hand.

Employee was changing a rip-rap screen and when finished he wiped his face with his glove and a foreign object entered his right eye. The eye became irritated over the weekend. Diagnosis was a bruised eye but the Dr. put antibiotic drops in the eye making injury recordable.

EE was returning from dinner when he stepped on a rock and turned his ankle. The x-ray showed he had a hairline fracture in his lower left leg.

Employee was lifting a fork to place in the mast of CAT TH63 forklift when his helper dropped his end. This caused the employee's right thumb to get pinched between the fork and the mast.

EMPLOYEE WAS PUSHING BUTTONS IN THE CONTROL BOOTH AND HIS BACK BEGAN TO ACHE. THE PAIN GOT WORSE DURING THE DAY AND HE HAD TO LEAVE WORK THE NEXT DAY. EMPLOYEE SAYS HE CAN THINK OF NOTHING THA T HE DID TO CAUSE THE PAIN.

WHILE CLIMBING UP THE LADDER ON HIS ENDLOADER EMPLOYEE HAD MUD ON BOOTS. WHEN HE GOT TO THE 2ND RUNG HIS RIGHT FOOT SLIPPED CAUSING THE EMPLOYEETO STRIKE HIS NOSE AND FOREHEAD ON THE PORCH OF THE MACHINE.
