An employee was assisting the operators mechanic with a routine inspection on a piece of plant equipment. When prying the piece of equipment with a pry bar, the pry bar slipped and struck the employee in the lower lip. The employee went to seek medical treatment and was given 2 stitches to seal the sustained injury.

Showing all 18Employee was attempting to line up 2 pipe flanges so they could be bolted together. As he pulled up on pry bar to align bolt holes, he felt something pull in right shoulder blade area. He did not report this but @ 1530 hours his back began cramping & he notified the Foreman. He was seen in local ER and diagnosed with a muscle strain. Pain med prescribed.
Employee was tightening a flanged suction pipe connection to a diesel pump when the open ended wrench slipped and he fell backwards landing on the frozen ground. Employee suffered a contusion to his buttocks. Seen by Dr who provided prescription and 2 shots as well as 2 weeks "restricted duty."
Electrician & mechanic were changing junction box on motor. Electrician threaded 2 of 4 mounting bolts & was having difficulty starting threads on 3rd bolt. As he started threads, mechanic tapped top left corner of box with hammer. This was at the same time the electrician placed his gloved hand on top left corner of box and was struck by hammer.
EE was clearing blockage on distributor pot of coronastat. Rubber and metal end cap came off pipe and landed on hand, causing laceration to right middle finger. Seen in ER and given 4 stitches to close wound. Returned to work, no restrictions.
Employee was using maul and brass punch to drive out stuffing box on 2" X 1.5" pump. Missed punch with maul and struck top of left hand causing large hematoma.
EE was stripping electrical cable jacket to facilitate termination in junction box. Knife blade slipped out of rubber jacket, stabbing EE in the left pointer finger. EE was cutting towards himself with knife. Required 4 stitches to close laceration.
EMPLOYEE IS HIRED THROUGH A TEMPORARY STAFFING AGENCY. ON 8/13 THIS LABORATORY TECH COMPLAINED OF RIGHT THUMB PAIN FROM USING INDUSTRIAL SHEARS ON A REPETATIVE BASIS FOR SEVERAL WEEKS. SEEN BY TEMP AGENCY PHYSICIAN & DIAGNOSED WITH RIGHT THUMB TENDONITIS/STRAIN. RIGHT HAND SPLINTED WITH LIMITED & RESTRICTED DUTY FOR 2 WEEKS.
Employee was using a wrench to loosen interior fittings on an air compressor. While exerting pressure on the wrench, the fitting released causing him to strike the middle knuckle on his left middle finger against a sharp edge. This resulted in a jagged laceration requiring 2 stitches to close the wound.
EE WAS CUTTING DEBRIS OFF HDPE PIPE END WITH A POCKET KNIFE. THE KNIFE SLIPPED OFF THE MATERIAL AND STRUCK HIS R THUMB. HE RECEIVED APPROX. 1" U SHAPED LACERATION ON PAD OF R THUMB. SEEN AND D R'S OFFICE AND GIVEN 5 STITCHES AND TETNUS SHOOT RETURN TO WORK BUT KEEP WOUND DRY.
EE WAS USING PORTAPOWER TO MOVE A DRIVE PLATE ONDRYER. PORTAPOWER SLIPPED & HIT HIS PALM, STRAINING HIS LEFT SHOULDER
EE EXPERIENCED NUMBNESS TO LITTLE FINGER AND RING FINGER ON RIGHT HAND AFTER STRIKING A TRUCK U BOLT WITH A HAMMER IN AN UPWARD MOTION. EE WAS SENT TO DR ON 10-30-01 AFTER REPORTING CONTINUED NUMBNESS.
AN EE WAS REMOVING A VEHICLE U JOINT FROM A DRIVER SHAFT BY STRIKING IT WITH A HAMMER. A SMALL FRAGMENT BROKE FREE FROMT HE U JOINT AND STRUCK AN EE STANDING APPROX. FIVE FEET AWAY INT HE LEFT FOREARM. THE EMBEDDED SLIVER WAS REMOVED BY THECO DR. EE RETURNED TO WORK AND REGULAR STATIS.
EMPLOYEE WAS TRANSFERRING STEEL CHANNEL BARS FROM THE FORKS OF S FORKLIFT TO A STORAGE RACK. EMPLOYEE'S THUMB WAS PINCHED BETWEEN PIECE OF CHANNEL BAR AND HIGHER RACK LEVEL WHEN UNEVEN WEIGHT DISTRIBUTION CAUSED METAL BAR TO RISE UP UNEXPECTEDLY. VICTIM PULLED BAR DOWN AND REMOVED THUMB.HE HAD NO BROKEN BONES. RETURNED TO WORK LIGHT DUTY.
EMPLOYEE WAS SHOVELING ZIRCON PRODUCT OUT FROM AROUND PRODUCT ELEVATOR. EMPLOYEE CAME INTO CONTACT WITH HOT ZIRCON PRODUCT SCREEN; BURNED RIGHT ELBOW.
ATTEMPTING TO REMOVE A PULLY FROM ALTERNATOR USING HAMMER WHEN A PIECE OF METAL FROM THE HAMMER BROKE OFF AND LODGED IN HIS STOMACH.
EE WAS REMOVING PIN FROM BUCKET CYL.ON BACKHOE. IN HIS EFFORT TO DO THIS, HE WAS USING ABAR TO BEAT PIN THRU EYE OF CYL. WHEN HIS RIGHT HAND WAS SMASHED BETWEEN BAR AND VISE LOCATED ON BACK OF SERVICE TRUCK THUS CUTTING HAND AND FINGRS.
EE WAS BENDING OVER PULLING ON A WRENCH ON 4/19/00 WHILE WORKING ON MINING UNIT, AND HAD PAIN IN LOWER PART OF BACK MOSTLY ON LEFT SIDE ACCORDING TO MEDICAL EXAMINERS REPORT. EE WAS PUT ON LIM ITED DUTY FOR 5 DAYS AND RECEIVED PRESCRIPTION MEDICATION.
