Struck against a moving object

Lull overturned in stockpile. Operator egressed through rear window. Operator was diagnosed with contusion in shoulder. Restricted to no heavy lifting.

Struck by falling object

Employee was directing adjustment placing of pipe on a pipe welding machine. Pipe made a sudden movement thinking it was going to fall the employee made an effort to move. He placed his hand on a part of the machine which was not in use. This was on the lower half of a clamp. The upper half dropped hitting the middle finger on the left hand. Finger was fractured

Fall onto or against objects

Employee was bringing tools to another Employee and slipped on concrete floor which had ""flock"" on it. Employee's elbow hit an anchor bolt.

Struck against stationary object

Employee was observing the discharge of a sanded line. When the slug broke loose, the line pulled free and sprayed material. The employee was turning to get out of the line of fire and hit his head on a piece of fixed equipment.

An employee was assisting the operators mechanic with a routine inspection on a piece of plant equipment. When prying the piece of equipment with a pry bar, the pry bar slipped and struck the employee in the lower lip. The employee went to seek medical treatment and was given 2 stitches to seal the sustained injury.

Fall to the walkway or working surface

Employee was walking in mining field flagging power cable when his left heel stepped on something (unknown) and employee fell with left leg underneath him. Employee said it burned and he reported incident to his supervisor. On 8/2 employee asked to see Dr. and was diagnosed with strained calf and given 2 weeks light duty.

Contact with hot objects or substances

Employee stepped inside CAT generator trailer with heel of boot landing on lip of catch pan causing employee to fall forward and burn arm on diesel engine turbo. Employee received a ""dollar bill"" size second degree burn to left forearm.

Fall onto or against objects

Employee was walking toward pipe valves, slipped and fell in mud, striking right shin just below the knee on bolted metal pipe flange. Employee was seen in ER, diagnosed with large hematoma below knee, given 2 days of restricted duty, pain medicine and anti-imflammatories.

Over-exertion in pulling or pushing objects

Plant operator was pulling a rolling ladder across the floor when one of the legs caught the toe plate causing the ladder to come to an abrupt stop. This caused a sudden twisting to his lower back resulting in a muscle strain.

Fall to the walkway or working surface

Supertindent was walking to parking lot attempted to step over silt fence and got r foot caught in fence. Fell to ground breaking both lower leg bones, Dislocating ankle and tearing ligament. Required surgery (6 pins/plat) to repair fracture.
