Showing all 9Employee was removing a conveyor splice joiner pin with vice grips and a hammer. The hammer glanced off the vice grips and struck the top half of his left index finger causing a contusion of the left hand.
Cleaning tracks on D-10 dozer with a bar. The bar slipped and hit the dozer frame and then hit the mechanics right hand resulting in a closed fracture of the navicular bone of wrist.

THE EE WAS HITTING A HOPPER IN ORDER TO EMPTY IT OUT. WHEN HE HIT THE HOPPER THE LAST TIME THE HAMMER GOT CAUGHT BETWEEN THE PACKER HOPPER AND A LIMIT SWITCH SUPPORT ROD. AS A RESULT HE TWISTE D HIS ARM.
EMPLOYEE WAS HOLDING A HAMMER WRENCH BEING STRUCK BY ANOTHER EMLOYEE. WHEN THE OTHER EMPLOYEE WAS SWINGING THE HAMMER HE HIT A BOLT, WHICH THE HAMMER BOUNCED OFF OF AND INTO THE HAND OF THE EMPLOYEE.
