EMPLOYEE WAS ASSISTING IN TIRE REPAIR TO HIS TRUCK. LOST BALANCE WHEN TIRE TOOL SLIPPED FROM RIM AND TIRE BEAD. IN A ATTEMPT TO BREAK HIS FALL, SPRAINED WRIST WHEN HE FELL TO THE FLOOR. TIRE W AS LYING FLAT ON SMOOTH CONCRETE FLOOR WITH NO OBSTRUCTIONS.

THE INJURED PER8ON AND THE DRAGLINE OPERATOR WERE DRIVING A NEW TRACK PIN INTO THE TRACK OF THE DRAGLINE. WHEN THE NEW PIN WAS TOO FAR INTO THE TRACK TO BE STRUCK BY SLEDGE HAMMER, THE OLD PI N WAS BATTED UP3AGAINST THE NEW PIN SO THEY COULD FINISH DRIVING IT IN. WHEN THE OLD PIN WAS STRUCK SEVERAL TIMES BY THE DRAGLINE OPERATOR, A FRAGMENT BROKE OFF AND ENTERED INJUREDS LEFT ARM.
