Employee was lifting a gas cylinder bottle on the welding trailer. EE lost grip due to a light frost; the cylinder fell back into the mounting slot with EE's right hand positioned on the bottom end of the bottle to allow EE to guide it down. EE's finger was in-between the bottle and metal rack resulting in injury to the tip of EE's right Pinky Finger, needing 4 stitches.

on 04/06/2017, the EE in question reported that they had hyperextended their right knee while relocating a pump but continued working unaffected by the incident. It was later determined that the EE had torn the ACL and Meniscus of the right knee. EE reported to work on restricted duty on 06/08/2017 following corrective procedure on the knee.

He was removing a cutting edge on FEL and cutting edge fell on his right foot. Unit wheels chocked and bucket lowered onto jack stand 24 inches from floor. Bolts securing edge were cut and he used a hammer to remove edge. The edge dropped striking his safety toe shoe, which caused a laceration of his foot. Examined and bandaged, then transported to hospital. Treated, released.

OPERATOR WAS CHANGING IMPLEMENTS ON SKID LOADER FROM PALLET FORKS TO A UTILITY BUCKET FOR CLEANING PLANT. IT HAS 2 LEVERS THAT CAN LOCK THE BUCKET INTO PLACE. HE WAS SITTING IN OPERATOR'S SEAT & USED HIS RT FOOT TO PUSH LEVER TO LOCK BUCKET. HIS LT FOOT WAS IN SUPPORT ON PART OF THE FRAME, IT SLIPPED & LANDED ON PEDAL THAT LOWERED BOOM ONTO RT FOOT. CONTUSION OF RT FOOT.

EE WAS REMOVING A PLASTIC TRASH CONTAINER FROM SHOP AREA. A RADIATOR COWLING FROM A PIECE OF EQUIPMENT THAT WAS IN THE SHOP FOR REPAIR, WAS LAYING FLAT ON THE SHOP FLOOR. A GUARD FOR THE COWLI NG WAS LEANING ON THE COWLING. EE MOVED THE COWLING TO HAVE MORE ROOM TO MANEUVER THE TRASH CONTAINER. THE GUARD THEN FELL OVER ONTO THE EE'S RT. FOOT, BREAKING THE BIG TOE OF HIS RIGHT FOOT.
WAS ASSISTING IN LOADING A 15 GALLON DRUM OF OILINTO SERVICE TRUCK. AS HE AND ANOTHER WORKER LIFTED THE DRUM ONTO THE TRUCK TAILGATE, IT FELL CAUSING THE DRUM TO CUT HIS FINGER. THE TAILGATE LEGS WERE IN CONTACT WITH THE TRUCK BUMBER, BUT SLIPPED AND DRUM STARTED TO FALL OFF. HE STILL HAD HIS HAND UNDER THE DRUM WHICH CUT HIS FINGERAS IT STARTED TO FALL. 3 STICHES TO CLOSE WOUND.
