EMPLOYEE WAS HELPING TO INSTALL A CRUSHER CHUTE AND WAS USING A CHAIN FALLS TO HOIST ONE ENDOF CHUTE WITH JUST A CHAIN WRAPPED AROUND THE END, AND THE CHUTE SLIPPED FROM THE CHAIN BRUISING HIS SIDE.
