Drill operator was using sledge hammer to break down chuck on the bit. While striking the chuck a piece of metal broke off and struck operator in the leg resulting in a laceration.

The employee was installing a satellite dish on a pole and while tightening a mounting bolt, the employees' hand slipped and the right ring finger knuckle caught the bolt which resulted in a laceration to the right ring finger knuckle.

An iron worker foreman, struck his left index finger with a four pound sledge, from a glancing blow to an alignment pin. This caused a crush injury with laceration that required sutures to close the wound.

Lube mechanic was striking shovel bucket dig tooth with steel hammer to drive the tooth off. A small shard of steel from tooth was sheared off & imbedded into the EE's leg. After the injury, only 1st aid & medical exam was provided. Multiple Drs advised EE against removal of the small steel splinter. At a much later date (5-3-11) EE had it removed as elective procedure.
