Driller assistant stepped off of day bed of pipe truck. EE's left foot contacted a wooden block that EE did not see causing EE to roll their ankle.

While cutting 1 1/4"" pipe using a wheel grinder, the grinder slipped, striking the employee above the knee resulting in a laceration.
Water truck was traveling down a grade fully loaded when the operator noticed flames coming from the right rear wheels. Operator safely stopped the truck and exited the cab after initiating a Mayday call.
3/21/21 Operator called their supervisor to call off work saying they slept wrong causing their neck to be sore. 3/29/21 Operator was seen by site nurse for neck pain and sent home.
Employee sustained a sprain to the lower back when shovel operator lowered bucket into the back of the haul truck bed.

Employee sustained an injury to the right knee after hopping off equipment at end of shift.
An employee received a left ankle sprain when the employee's foot slipped out from under the employee when stepping down onto a slippery surface at the bottom of the stairway at the Primary Crusher.
EE was cutting laminated glass using denatured alcohol from a 1 gal can. Somehow the alcohol inside the can was ignited, and blew the bottom of the can off. Burning alcohol sprayed onto the EE, causing extensive 2nd degree burns to the front of EE's body.
Employee wash pushing on the post of a tire machine so cleaning could be performed underneath the machine. When the tire machine was lowered to the ground, the upper arm swung and pinched the employees left middle finger.
Miner was accessing leach pad cut in a 631 G Scraper from the Haul Road. When the miner made the transition into the leach pad they struck a rough patch which caused the scraper cab to move up and down in a rapid motion. This caused the hairline fracture to thorax vertebrae T7 & T8. Seat Belts were worn, PPE was used.
Employee was climbing down from a D11 dozer. There was a rock stuck in the bottom step on the push arm. Employee's foot slipped on the rock. As left foot hit the ground, it landed unevenly, and ankle rolled inwards.
Employee was sitting in a chair in a conference room during a leadership meeting. Employee fainted and fell to the floor, striking forehead and left hand causing bruising to hand and head as well as minor abrasions to forehead. Employee regained consciousness within 10 seconds. No cause could be identified for fainting.
Employee was working to remove press plates from a filter press when their left ring finger became pinched between the plate and the frame of the press. This resulted in an open fracture of the left ring finger tip.

Miners were removing a frozen water hose from the catwalk beside a conveyor. One end of the hose was hung over the railing of the catwalk and was lowered to the ground. Gravitational energy started the hose falling freely, running along the rail, and the hose struck a miner in the face, breaking nose and knocking them to the catwalk floor.
A small sulfur fire ignited spontaneously in an inactive pit. No persons or equipment had been working in the pit for some months. It was determined to restrict access to the pit, provide gas detection monitors to personnel in the vicinity, and to allow the fire to burn itself out.
Employee was climbing small ladder on the side of the lube truck. Employee carried a hose and as such did not maintain 3 points of contact. Instead of grabbing the handrail, employee grabbed a loose ladder stored on the truck and fell off the ladder landing on feet and fracturing an ankle.
Haul truck operator was about to dump load at the dump point. EE reached down for the dump lever and felt a pop in the shoulder and pain thereafter.
Laboratory employee felt pain in hand as employee picked up a sample bucket. Reported the injury to supervisor that day. Did not seek medical attention until 8/5/2019.
Temporary employee was climbing into a pickup truck. Employee hit hard hat on the door frame, spraining neck.
Employee initially reported pain upon exiting a light vehicle away from the mine site on 5/15/19. Later the employee reported that the injury must have occurred on 5/14/19 while EE was working on a tire, but cannot point to any specific time, place, or event where injury took place.

A process maintenance mechanic was bending over in the bed of a pick up truck to retrieve a sling to secure some oxygen/acetylene bottles. As they bent forward they felt a pop and sharp pain in the lower back. Employee was taken to the local emergency room and diagnosed with muscle strain and spasm.
Assay technician was stepping down from the rear bumper of a pick-up truck. When EE placed EE's left foot on the ground, EE lost EE's footing and fell to the ground, twisting EE's left knee. EE was seen in the local emergency room and placed on light duties.
Employee was sitting in a chair in the AA room of the assay lab, when employee pivoted in chair and struck knee cap against the corner of a filing cabinet. Knee was contused.

As the employee was walking between the mud mixing tank and the drill helper working area, tripped and sprained left thumb when employee came into contact with the ground.
Employee was standing on a ladder removing a deck from a haul truck when the exhaust pipe shifted and the employee jumped approximately 5' to the ground striking right foot.
Employee suffered a non-work related injury when the batteries for an e-cigarette that employee had placed in the front left pants pocket ignited, causing 1st, 2nd and 3rd degree burns to left leg and ankle.

As a pump was being lifted, employee placed EE's right hand on the sling, the load shifted/slid and caught employee's right index finger between the sling and metal forks.
Employee provided lab sample from private MD/lab showing high Thallium, Results were from 1-29-16. Co. lab results from 1-19-2016 showed 0.2mcg/l3. Notified by employee on 2-11-16. Results pending on 3rd independent test.

EE tried to pull a long length of 1"" rubber hose, stained his back and then asked for help moving the hose, moments later 2 more EE's arrived to help move hose, after hose was moved the EE felt pain in his back and reported the injury to his supervisor they filled out a C-1 report. EE did not want medical help at this time, worker had pain next morning, taken to hospital for exam.
While cleaning, the worker lifted a bag of household type garbage, while simultaneously twisting in order to carry it out of the office resulting in a lower lumbar strain.
Worker completed pre-shift equipment inspection on Unit #408 (CAT D11R Dozer). The worker began the process of ""climbing onto the machine"" in order to access the cab by reaching for the grab handle with his right arm. The workers foot shifted, causing tension to his extended right arm resulting in a dislocated, right shoulder.
The worker removed an 18""x18"" grate near the sump in order to help facilitate cleaning of the process room. He stepped over the opening in order to help another worker with a hose. After helping, he stepped back with his right leg, which went into the opening. He slipped onto his right side, resulting in a bruise/strain to his right rib cage area.
While loading drip lines onto a fork lift the employee states wind blew dust under his glasses and into his eye.
While closing clarifier door, employee reached up move lever into lock position, his finger became pinched between locking arm and clarifier.
SV805 Lube Truck was traveling towards the Porter Pit to service SH004 when unit lost power in cab, operator safely brought the truck to a complete stop and set park brake. Operator then looked out the window to the right seen haul truck passing then looked left and saw a red glow coming out of the articulation.

There was an existing sprinkler system in the area cooling hot spots. MSHA issued a 103(j) order at 7:40 am to prevent destruction of evidence. Sprinkler system extinguished fire at 8:25 am.

On 1-27-15 Mine Operator reported a back injury occurred at mine while being loaded by the electric shovel. Under investigation, pending results.

A fire was reported to MSHA 01-05-15 at approximately 10:40 pm in the pit. MSHA issued a 103(j)order to prevent the destruction of evidence. Upon MSHA inspection at approximately 1:00 am there was a small hotspot on the south highwall.

Leach pad operator was changing a flat tire on a pick up, and pinched hand between fender and tire causing contusion on hand.
Mine operator was spotting chain for high wall clean up when chain struck the left lower leg below knee resulting in compound open fracture.
Employee was moving a pin across a dozer track to give to a second employee, second employee lost grip and dropped pin causing first employee to injure finger when caught between pin and track.
Upon entering lube truck, maintenance mechanic's right knee came in contact with the seat adjuster resulting in right knee pain.
While cutting metal inadequate control was taken when metal was fully cut off, resulting in an uncontrolled descent of wear plate. The plate struck EE left shin, resulting in a laceration that required 8 stitches.
Brimstone refiner was dropping mold to dislodge dore' bar. While using right hand to hold the mold handle, the refiner smashed the tip of the right hand finger between the mold and the floor.
Refiners were cleaning the mercury scrubber after it had been flooded. Two refiners opened the access door on the side of the scrubber to clean/remove residual water. Upon opening access plate, both refiners were exposed to SO2. After the initial exposure, one of the refiners continued to feel ill while on his days off and went to the hospital to be examined.
Drill operator was using sledge hammer to break down chuck on the bit. While striking the chuck a piece of metal broke off and struck operator in the leg resulting in a laceration.
While preparing to leave the Mine Operations Building, a Coach USA driver was walking out of the building and tripped on the entrance rug. The driver then fell through the doorway, making contact with the metal landing on the stairwell which resulted in a laceration and five stitches to the forehead.
The employee was removing a section of 18 inch pipe when the pipe slipped. The employee's left hand was between the pipe and an electrical cable tray. The employee's left index finger was pinched between the two, causing a laceration to the left index finger.
While loading old drill steel with a forklift at the Vortex ready line a driller was on the ground while the forklift was picking up the steel to be placed on the trailer. The drill steel started rolling off forks and onto left foot.

The cable crew team was reeling up excessive shovel cable alongside the haul road in the C-4 Pit with the cable reel. While pulling cable from behind the windrow and across the road the shovel cable struck a crew member in the feet, knocking them out from underneath, causing a same level fall onto the left elbow and shoulder.

Employee was working on some haul trucks in cold temperatures which resulted in hypothermia.
Mechanic was placing wheel chocks down on fork lift, while walking away, tripped over rope that connected chocks causing mechanic to fall striking right knee on the ground.

The employee was operating a Forklift and ran over a pothole. The force of running over the pothole resulted in a lumbar strain.

As the employee was moving concrete forms, a form slipped and landed on his left foot. The foot did not bother him until 06/21/2013. The employee wanted to seek medical attention and a small fracture was detected.
Mill operator was grabbing samples from CIC#2 when he was coming down the stairs and slipped. He then grabbed the hand rail and twisted his right wrist. The operator was parked and being loaded by a loader. The operator claims a large rock hit the side of his bed causing a jarring effect and straining his back.

The employee bent over to pick up a large battery that is very heavy. The employee used improper lifting techniques resulting in a lower back strain.
EE was cutting bubble wrap with a razor knife, cutting towards him the razor contacted his left pinky resulting in a minor laceration.

Employee called and reported off to the lead man on 8/16/2012 for his scheduled shift on 8/17/2012 claiming breathing difficulty. Employees physician placed him on no duty status effective 8/17/2012 during visit pending further evaluation.
Injured employee strained back while lifting pipe on the leach pad and reported incident to supervisor at end of shift. Injured employee didn't seek medical attention until 8/14/2012 and was placed on modified work. Employee released to full duty on 8/20/2012. Employee worked three scheduled shifts on modified work.

Employee was walking out of the admin. building and tripped on a rock causing her to fall onto her lt. knee causing injury.
Employee was carrying a block of wood and slipped while walking on leach pad, he attempted to catch himself resulting in an awkward fall injuring his Rt. Ankle.
While assisting with cleaning of filter presses, the EE's arm was contacted by the discharge of a steam cleaner wand. Another EE was changing position and allowed the discharge end of the high pressure wand to pass in close proximity of the second EE's arm causing a pressure contact wound.
Employee was assisting in re-alignment of drill mast components. While pushing on component, the metal block slipped into place resulting in a drill section swinging and pinching the employee's finger against the metal frame.
Employee was descending the rear mounting ladder on the lube truck. As the employee reached the bottom step and transferred to the ground the employee has holding onto the ladder step. The ring on his left ring finger caught on the anti-slip metal protrusion of the ladder handle resulting in a laceration to the ring finger

In response to Part 50 Audit 2-5-14. Mechanic was traveling through man door at warehouse, when door slammed left elbow.
Employee was moving a 12""x12"" cribbing block when he twisted his left knee. The employee initially stated he desired no further medical care, however approximately two weeks post incident, the employee stated he need additional medical care for the injured knee. This report was filed due to the employee being prescribed a temporary knee brace.
Employee was cutting rubber flashing with a knife. He stabbed himself in the rt. hand with the knife.
Employees working in a drainage ditch while wearing rubber boots - resulting in wet feet. Reserve for insurance is set at $200.
Employee was attempting to free the powerstep from the locked position with a pry bar, when it released it threw him out of basket, landing on his head and knees.
While loading a large steel pipe flange into the bed of a pickup truck, the EE's left middle finger was pinched between the flange and the bed of the truck. The EE sustained a crush of the distal end of the finger and a 2 inch laceration.
Drill helper was sliding a PQ inner Tube in to the shack. The core lifter case had a bur on it that caught the helpers glove and pulled it in to the stairs smashing his middle finger between the stairs and the shack.
While in the process of changing a drill bit on a blast drill, the employee sustained a 2 cm laceration to his left ring finger. A small burr of steel on the bit sliced through the employee's glove and caused the laceration.
While doing general cleanup around a lime silo, the EE had removed his protective eye ware in prep. of leaving area. A haul truck accessed the discharge chute of the silo and activated the discharge. The prevailing wind carried some lime dust in the direction of the EE and the EE received lime dust in his right eye.
EE was tripping out drill rods, on one pull he failed to properly tension the rods with the mainline. This caused the weight of the rods to pull the slack out of the mainline causing the line to break and the line weight ball to fall striking the driller in the right thumb.
Lube mechanic was striking shovel bucket dig tooth with steel hammer to drive the tooth off. A small shard of steel from tooth was sheared off & imbedded into the EE's leg. After the injury, only 1st aid & medical exam was provided. Multiple Drs advised EE against removal of the small steel splinter. At a much later date (5-3-11) EE had it removed as elective procedure.

Employee was carrying a box of core samples and stepped off of a sidewalk. His ankle rolled causing a fall and injury.
While loading drums containing soil, EE and a Hycroft EE were attempting to place a drum between another drum and the side of the skid. The drum became hung-up on the section of E Rail track and caused the drum to fall foward and out of the drum picker. The drum ring landed on the EE's foot above his steel toes. This caused a fracture in the bones of his left foot.
In the process of changing the drill bit on a mobile bast drill, the drill steel kicked off to one side while the EE was holding the bit. The right pinkey finger of the EE was mashed between the bit and the steel frame work.
Employee states that while pumping mud down the center of the pipe using diaphram pump and 2"" he was struck in chin by hose.
Trying to get heater working employee tried to remove a fuel hose to see if it was clear. Hose came free and the employees thumb went into fan blades on the heater cutting the top of the thumb which required stictches to fix.
Employee was using a pipe hook to adjust some pipe on the pipe truck. When he pulled on the pipe his right shoulder popped.

Employee states that he was looking up holding carbon with rope and stepped into a muddy hole causing injury on 12/31/2009, due to time of injury and holiday I was not informed of injury until 01/04/2010. Employee was restricted from work 12/31/2009-01/04/2010.
Employee cutting dripline with box cutter slipped and cut calf
EE was performing electrical connections at generator set when tightning the fitting with wrench the wrench slipped. EE injured left index finger as it was pinched between wrench and steel frame.
EE was on top of mill filter press when he lost his valance and fell backwards, falling to floor of pressroom. EE said he was fine, but was taken to doctor for evaluation.
Lifting sample bags in desert bit by unknown insect.
Employee was standing between large tire truck and metal dumpster, as tire truck backed up he was pinched between truck fender and dumpster, sustaining a compression injury to shoulder.
Employee was handling a glass beaker. When moving beaker employee felt a sharp pain in wrist. Employee recently had surgery procedure on this wrist (not work related). It is assumed that the extension and rotation motion of the hand may have caused some injury to the previous surgergical site.
While walking toward his equipment, employee stepped into chock ditch, twisting left knee. ***Employee did not notify employer of medical care until 6/2/09*** This report was filed the day after the employeer was notified that the employee had received medical care(06-02-09)
Smashed left pinky finger between two rings inside bowl
EE and witness report a very low speed impact between the two haul trucks. At time of incident both involved operators denied any injury. The EE was in a fully padded suspension seat at the time of the impact. There was minimal damage to the haul trucks. At a much later date, the EE claimed a back injury. The EE was denied worker compensation on two occasions for this claim.

While walking on an ice covered road, employee slipped on ice, fell, twisted his right ankle, hearing a popping sound immediately followed by intense pain.
EE was operating an All Terrain Vehicle in an unauthorized & unsafe manner(jumping a berm). The ATV flipped over forward, landing on the EE who sustained a broken leg & dislocated hip as a result. The EE had been instructed not to operate the ATV. The ATV was inoperative until the injured EE & another EE made the ATV operative against the supervisor's instructions.
Employee was adding drill rod, he put his hand under rod as it was lowered onto table, pinching finger between rod and table. Laceration requiring stitches.
Was loading and unloading core from under core table and twisted lower back.

WHILE USING A HAUL GRINDER, A SMALL SPECK OF MATERIAL GOT INTO HIS RIGHT EYE. HE SAID HE DID NOT NOTICE IT AT THE TIME. HE RETURNED TO WORK THE NEXT DAY SAYING THAT THE SPECK WAS IRRITATING HI S EYE. HE WAS SENT TO THE EYE DOCTOR. HE WAS PREVIOUSLY SCHEDULED FOR VACATION & WILL NOT RETURN TO WORK UNTIL MAY 6, 2002.
