Employee was replacing O-ring of steering cylinder with 14mm wrench and it slipped off bolt, right hand jerked back hitting against the corner edge of metal frame on the loader. Employee could not open his hand afterward and was told to go to the doctor.

Employee was shoveling around a crusher feed belt, which was feed by another belt. He noticed some material had built up in the rock box and was spilling over onto the ground. As he attempted to clear the material with a shovel, he placed his hand on the rock box and was struck on the left hand index finger tip by a rock.
