Employee was tightening a bolt on excavator bucket with 3/4"" impact wrench. His finger got caught between bolt and h-link causing a gash on left middle finger. Employee was taken to the nearest hospital where his finger was stitched.

Employee was putting the guardrail back on a loader. The rail slipped and cut the employee's left index finger.
Cutting on El-Jay screener - a piece of slag fell down into left top of boot causing a burn to the ankle area. Removed boot, administered first aid and employee was taken to the doctor.
Employee was cutting a screen with a grinder. The grinder slipped and cut his finger.
Employee was cleaning up floor pit with a loader. He hit a rock which stopped the loader - seat belt came lose and the employee hit his head on the windshield and his stomach hit the steering wheel causing some bruising.
Employee was replacing O-ring of steering cylinder with 14mm wrench and it slipped off bolt, right hand jerked back hitting against the corner edge of metal frame on the loader. Employee could not open his hand afterward and was told to go to the doctor.

EE WAS ATTEMPTING TO CLEAN UP A PISTON ON THE WIRE WHEEL SIDE OF THE SHOP BENCH GRINDER. THE TOOL REST WAS BROKEN OFF & ONE WAS ON ORDER. THE GRINDER SHOULD NOT HAVE BEEN USED. EE WAS HOLDIN G PISTON BETWEEN HIS 2 HANDS & PISTON GOT CAUGHTIN THE WIRE BRUSH & CAUGHT THE TIPS OF HIS FINGERS
TURNING A LOADER WHEEL OVER- FLANGE OF WHEEL SLIPPED - FINGER GOT CAUGHT BETWEEN FLANGE AND WHEEL AND RIGHT FINGER WAS LACERATED.
OPERATOR USED HIS FOOT TO TRY TO MOVE A STUCK PIECE OF ASPHALT AND IT KICKED BACK AT HIM PINCHING HIS ANKLE EITHER BETWEEN TWO PIECES OF ASPHALT OR BETWEEN THE SIDE OF THE FEEDER AND THE ASPHA LT.
THE WELDER WAS USING ELECTRIC GRINDER AND A PIECE OF FOREIGN MATERIAL GOT INTO HIS LEFT EYE.
