Employee was cleaning a screw as part of janitor cleaning task they tried to clean it thoroughly and used their hand to clean further and they cut the tip of their middle finger, left hand they then went to foreman and they provided first aid and as bleeding continued, employee was then taken to emergency room.

Showing all 34Employee was using a hammer and chisel to drive a counter weight off a motor shaft. When striking the chisel with the hammer, the hammer slipped off the chisel and struck employee's left thumb.
Employee was removing a flexible conduit connection to make a repair. While using a pipe wrench to turn a fitting, the wrench slipped and his right little finger struck the conveyor frame next to the electrical fitting. The tip end of the right little finger was fractured.
Employee was cutting straps off of a roll of plastic wrap. EE bent over and cut the strap pulling the box cutter toward his leg. When the strap separated, the knife blade was pulled into his leg.
EE was removing a piece of metal caught between a conveyor belt and a bin chute. He was using a pry bar to pull the piece out when the bar slipped and his left hand came into contact with the edge of the metal. His left little finger was fractured and both little finger and ring finger were cut. Little finger required 5 stitches, ring finger required 4 stitches.
Employee was cutting a hose using a box cutter. The blade was not long enough to go all the way through the hose so the EE flipped the hose over to cut on the other side. When the box cutter cut through the hose, the blade slipped free, contacting his left second finger and cutting it
Employee was assisting with the closing of a kiln hood, using a come-along and a wire rope choker. While working the come-along, the wire rope choker snapped under tension. The choker flew back and struck the employee in the right knee.
Worker was breaking loose a bolt, the bolt came loose and he smashed his knuckles and hand.
Stripping insulation from electrical wire, slipped cutting hand. Reuqired seven stitches.
Repeated hammering of drift at 678 blower pipe.
Employee was using a wrench to loosen bolts on a blower when he felt something pull in his right upper arm.
Employee was cleaning a piece of flint from the spike roller with a pry bar. While prying, he struck his left ring finger against a spike roller tooth resulting in a fractured finger.
Employee was replacing seals-bearings on Equalizer bar of Cat Dozer D9R. Space is limited; he crawled underneath the dozer using a hammer to beat out the seals. While trying to remove seals he hit his left hand three times with the hammer causing bruising - swelling of his left hand. *Note: employee has vacation Sept. 29-Oct. 5, 2005. Will return full capacity Oct. 6, 2005.
EE was exerting force on a jack in an effort to reposition a rail car. While operating the jack, He felt a pain in his groin area.
EE was using a hammer to align a pipe flange. He struck the tip of his right thumb with a hammer blow that inadvertently missed the pipe flange. This case became reportable when Max was seen by a physician on 4/19/05 and an x-ray revealed a fracture.
Employee was cutting shrink film off a pallet of bagged product. EE was cutting in a downward motion with his left (dominant) hand while holding the film with his right hand. The knife blade came in contact with his right index finger at the middle knuckle, causing a laceration.
Ee was hammering on a plugged vacuum pipe when he felt pain and soreness in his right elbow. This event occurred on 8/12/04 and became reportable when ee was treated by a physician and placed on modified work activities on 9/17/04.
EE was working on #601 Cyclone Top Hat. EE struck a drift with a hammer. The hammer glanced off the drift, causing his right index finger to be caught between the hammer handle and the floor.
EE was unplugging the discharge spout of a packaging machine. EE was struck in the right palm by a rod he was using to clean the a plugged packer spout.
EE complained of discomfort in his right shoulder after repeated use of a chain block to install the back head of 625A blower. This event occurred on 6/21/04 at 3:00 pm but become reportable when the treating physician placed EE on work limitations beginning 7/7/04.
Employee was using a shoe knife to cut some duct tape. The knife slipped, resulting in a laceration to Gilbert's left index finger.
EE was rebuilding screw for fines system. Pulling on a 1 1/4" wrench felt pain in right arm & shoulder.*Note:EE was on vacation starting 7/19, we didn't know he went to DR (7-19-04) until he returned to his regular scheduled shift July 29th bringing paperwork from dr for light duty work. Tendinitis right shoulder-possible rotator cuff.
MOVING RADIAL STACKER TO DIFFERENT LOCATION REQUIRES WHEELS MUST BE ROTATED. A CHAIN IS ATTACHED TO LIFT STACKER ON ONE SIDE SO WHEEL CAN BE ROTATED. AS THE STACKER WAS BEING LOWERED, EMPLOYEE HAD HOLD OF CHAIN TO DISENGAGE. THE STACKER STARTED TO ROLL SO CO-WORKER LIFTED STACKER CAUSING THE CHAIN TO TIGHTEN AND CATCH EMPLOYEE'S HAND BETWEEN CHAIN AND STACKER.
EE WAS WORKING ON PRESSURE DIFFERENTIAL RAIL CAR PADS ON FEBRUARY 22, 2003 WHEN HE NOTICED SOME SHOULDER PAIN. EE REPORTED THE INJURY AND OBTAINED MEDICAL ATTENTION ON 3/10/03. A PHYSICIAN PRE SCRIBED MODIFIED WORK.
EMPLOYEE WAS USING A PIECE OF PIPE TO BANG OR HIT A PLUGGED DOWNCOMER. HE HIT HIS RIGHT THIRD FINGER WITH THE PIPE, SMASHING IT BETWEEN PIPE AND THE DOWNCOMER.
EE WAS STANDING UP USING HANDLE ON CABLE RATCHET TO LIFT GEARBOX & 3HP MOTOR TO ITS STAND. PULLED TOO HARD ON HANDLE STRAINING LOWER BACK. EE SHOULD HAVE USED CABLE RATCHET W/GREATER LIFTING C APACITY THAN RATCHET USED.
EE WAS SHOVELING MUD FROM UNDER ATTRITION SCRUBBER. WHILE TAKING A SHOVEL FULL OF MUD OUT OF THESCRUBBER EE'S RIGHT ELBOW STRUCK THE PAD EYE ATTACHED TO THE SCRUBBER MOTOR. CO. RECEIVED NOTICE 4-8-02. TREATMENT DATE 2-4-02. 2-1-02 HAD REQUESTED MEDICAL TREATMENT & SENT TO E.R. AFTER WAIT,DECLINED TREATMENT. WENT TO OWN DR, EVALUATION ONLY. RELEASED TO FULL WORK.
EE WAS CENTER PUNCHING HOLES. HE MISSED THE CENTER PUNCH AND HIT HIS LEFT INDEX FINGER WITH A HAMMER BLOW.

EE WAS LIFTING A CHAIN FALL WEIGHING APPROX 50 LBS. TO CONNECT TO AN OVERHEAD PAD-EYE. EE FELT SOME PAIN IN HIS RIGHT SHOULDER. THE EVENT OCCURRED ON 10-23-00 BUT BECAME REPORTABLE ON 11-6-00 DUE TO WORK LIMITATIONS.
EE WAS SUING A TORQUE MULTIPLIER TO TIGHTEN A BOLT TO THE BLADE FOR THE METAL SHEAR. THE MULTIPLIER SLIPPED CAUSING EE'S TWO MIDDLE FINGERS TO BE CAUGHT BETWEEN THE TOOL HANDLE AND WORK DECK.
EE WAS UNPLUGGING A #18 GLORY HOLE WITH A DIGGING POL & A SLEDGE HAMMER. DURING A STRIKE WITH THE HAMMER EE'S RIGHT THUMB WAS CAUGHT BY THE POLE& CRUSHED
