Employee was attempting to unroll a tape wrap on an extension cord bundle. The employee removed gloves to unroll the tape. After failing to unroll the tape the employee pulled out personal pocket knife in an attempt to cut the tape. During cutting a cut occured to EE's left hand between the thumb and index finger.

The injured person was using pliers to pull zip ties to secure electrical wiring. The pliers slipped off the zip tie and hit the employee in the mouth breaking a tooth and chipping another tooth. The injured employee return to work the next scheduled shift and has not lost any time for the incident.

A mine production ee was changing miner bits. During the removal process a bit became stuck and as the ee pulled addition force to free the bit, the bit wrench slipped causing the bit wrench handle to strike another bit, pinching the ee finger between the bit wrench handle and a miner bit, resulting in a fracture to the ee right index finger.

Worker was repairing a conveyor belt. The worker was cutting the belt close to an area where there was a mechanical splice in the belt. The knife blade caught on the mechanical splice, the worker lost control of the knife blade and sustained a laceration to EE's right knee. Medical treatment in the form of 4 Stitches was required. The worker was released to return to work.

A Mine Production crew was repositioning trim chain onto a sprocket, trying to rotate it by striking it with a hammer, so employee was kneeling in front. A fragment of metal separated from the sprocket and struck the employee in the upper leg. An attempt to retrieve it was done on 2/21;ee received two stitches and was placed on restriction to allow the injury to heal.

An employee using a metal hammer, connecting a pin into the links on a conveyor chain sustained a puncture wound that was caused by a piece of metal that had come from the pin as it was struck by the hammer, in EE's thigh. After, EE became aware of this, a few days later, EE was taken for medical evaluation where it was determined that the metal fragment should be removed.

A Mine employee was positioning a relay bar being installed on the longwall set-up face and supported by a come along (chain style). While positioning the relay bar, the come along chain broke and struck the employee in the face. The employee sustained a laceration wound on the chin that required stitches and a tooth was also damaged. The employee has been released to work.

A hoist mechanic removing a check valve nut using a 4lb hammer and a pin hammer, when ee felt a pull in arm. The employee was treated on site, before being taken to the local hospital, and was released back to work pending further evaluation. Upon medical recommendation on October 26, 2017, employee elected surgery, scheduled for October 30, 2017.

An employee received a laceration while tightening a bolt with a hammer wrench that slipped, pinching employee's finger between the wrench and breather pipe cap. The employee was treated on site before being taken to the local hospital where the wound was closed with seven stitches. The employee was released back to work without restriction.

An operator was locking out liquor in a valve that had no handle, the operator used a pipe wrench to open the valve. When the operator pulled hard on the pipe wrench, he felt pain in his right shoulder. After further evaluation and treatment on 1/30/15 the employee was placed on restricted duty.

Using a shovel to remove rocks the employee felt a sharp pain in his abdominal area. His supervisor was notified. July 3rd, he stated that a hernia had developed and was diagnosed with having sustained an abdominal hernia. July 14, the surgical report was reviewed and confirmed that the injury sustained was linked to the employees work activities on June 26th.

The EE had just finished changing the blade on his utility knife. As he attempted to close the knife it became stuck due to the build-up of soda ash, and the EE's thumb slipped from its hold on the handle and slid across the blade. The EE was seen by on-site first aid, and then taken to the clinic where he received 4 stitches, no prescriptions, and no lost time or restrictions.

The employee was utilizing a 5lb pin to cut a 3/8 inch wire cable over a 90 degree metal edge and proceeded to strike the cable with the blunt end of the pin. In this process, the employee struck his little finger on his right hand causing a Tuft fracture to the end of the finger.

The employee was attempting to free up an adjustment piece on a 48" pipe wrench. A piece of metal broke off when the pipe wrench was hit with a hammer causing the piece of metal to penetrate the skin of the employee's arm.

A summer student was using a utility knife to cut vinyl siding from a wall. He was standing with the utility knife in his right hand. and his left hand and forearm resting against the side of the wall. While applying downward pressure on the knife, it came out of the cut and the employee then jabbed himself in the left forearm, sustaining a laceration which required 3 sutures.

The employee was utilizing a rock hammer to tap on a slip-fit packing gland located on a shuttle car tram motor, when a piece of metal broke off the hammer and embedded in the employees chin. The employee sought medical attention and the piece of metal was removed and the wound closed with sutures. The employee has been placed on restricted duty until the sutures are removed.

While rotating the dryer to remove a spring, the employee stopped the pipe threader but the dryer kept spinning. The employee went to check to see why it was spinning on its own and the handle of the pipe threader struck him on the right side of his hard hat causing a laceration to his brow. The employee received sutures to close the wound and was given a full work release.

EE was in the process of stripping the outer jacket from a 3 conductor 250 MCM motor feeder cable. He was using a utility knife to cut the outer jacket of the cable to strip it back to perform an electrical test before terminating it to the motor. During this process his utility knife slipped out of the outer jacket and cut the middle and index finger of employee.

An employee was repairing brakes calipers on a jeep. He was attempting to remove old allen head bolts with an allen socket and wrench. The wrench slipped out of position and the employee's elbow struck the top of the jeep's fender where there was a sharp edge of metal. The employee sustained a laceration above the elbow which required 3 stitches to close the wound.

The employee was trying to replace a shear. In order to replace the pin, the broken pin needed to be removed from the clevis with a relay bar. The clevis pushed back, pinching the employee's right thumb between the relay bar and the clevis. He sustained an avulsion to his right thumb and is on restricted duty until this heals to avoid infection.

While the employee was changing a trapping shoe on the shear with a hammer, he felt pain in his left shoulder. He has been seeking medical attention off and on since the injury. It was determined to be a rotator cuff tear on October 9th and he received surgery on October 29th.

Employees were working on a miner using a puller when the race popped, which released the tension. This caused the employee to slip, losing his balance on uneven ground and roll his left ankle. The employee was taken to the emergency room where x-rays indicated that the employee sustained an Avulsion Fracture.

The employee was using his measuring tape while in a jeep waiting for the driver to return. When the driver returned and started to leave, the end of the tape measure was caught on a roof bolt and the employee tried to free the tape measure. In doing so, the tape measure cut the employee's right finger and hand. The employee received ten stitches.

The employee was working on a light on the miner and had finished stripping the conduit on the cable. He placed the utility knife down and was pulling on the cable when the knife fell and the blade struck him in the finger, causing a laceration. He received 3 stitches to close the wound and was given medical clearance to return to work without any restrictions.

On 1/16/07 while in longwall warehouse the employee was trying to retrieve a snatch hook when he felt sharp pain in shoulder. He was taken to seek medical attention and was put on RD until his MRI on 1/25/07.

Employee was using a sledge hammer to unplug BF-56. A 4 inch piece of steel was protruding from the bag house. His finger hit that steel before the hammer hit the side of the bag house. On 3/1/05 he had surgery to his fractured finger.

Employee was changing head bits on 6 BMR. He was using the wrench turned around backwards. When he pushed on the wrench it slipped off the bit causing him to hit the head with his finger between the wrench and the head. It split his right ring finger at the first knuckle requiring three stitches.

Employee was making a splice on a belt. They were having trouble with the crimper, it was not working properly. The employee pointed inside the crimper with his right index finger just as another employee worked the handles to crimp. His finger was smashed inside the crimper tool. He was taken to the emergency room and received 10 stitches and a full work release.

THE RIGGING BECAME TANGLED ON THE CROSS BRACES; THE EE MOVED THE RIGGING AND THE TUBE SHIFTED SMASHING HIS LEFT HAND AGAINST THE SIDE OF THE SCREEN BODY. HE WAS TRANSPORTED TO THE E.R. HE SHATTERED THE BONE IN THE TIP OF HIS INDEX FINGER AND RECEIVED SEVERAL STITCHES. HE DID RECEIVE A FULL WORK RELEASE.
