WAS ASSIGNED SHUTDOWN WORK OF CUTTING SKIRTING FOR A HOPPER. HE WAS GIVEN INSTRUCTION PRIOR TO STARTING WORK. HE WAS GIVEN A UTILIY KNIFE. CARPENTERS SQUARE AND ANGLE TO BE USED AS A CUTTING E DGE. INDIVIDUAL WAS SCORING THE BELTING BY USING THE CARPENTERS SQ. THE KNIFE SLIPPED AND RAN INTO THE LEFT THUMB.

EMPLOYEE WAS TIGHTENING A BOLT WITH A WRENCH WHILE STANDING ON A FLAT CONCRETE FLOOR. THE BOLT BROKE AND HE WAS OFF BALANCE AND FELL. I CONSIDER THIS A FREAK ACCIDENT WHICH WAS UNAVOIDABLE.
