Showing all 21Employee was working to remove the well at the re-crush plant. The well got hung up on a collar. Employee used a hammer and wedge to remove collar. Once collar got knocked free the well casing slid down and pinched employee's left hand ring finger tip.
Employee was using a hammer to attempt to unclog a chute from outside the chute. After doing so, EE felt pain in EE's right shoulder.
North Railroad switch in North Branford Quarry was frozen. Heat was used to melt the ice on the switch. It freed up but not all the way. Employee used pry bar to see where switch was hanging up. Pry bar slipped and employee fell backward onto right wrist.

Employee was cleaning the feeder chute and smashed right thumb between the tool and the chute. Employee was taken to urgent care and received 2 stitches.
While swinging a sledge hammer to clean process chute, employee's finger got caught between the sledgehammer handle and the angle-iron of the chute. Employee reported the incident to his supervisor at lunchtime. Employee went to the hospital on 6/5/15, where it was determined that the employee has dislocated and fractured his finger.
Employee was using a bar with a hook on the end to move a steel plate on the screen deck in an area with very low clearance. While lifting the plate with the tool, it slipped off and struck the employee in the mouth chipping his front tooth.

A CO-WORKER WAS STRIKING THE WRENCH THAT THE EMPLOYEE WAS HOLDING (THEY WERE TIGHTENING BOLTS). THE WRENCH STRUCK THE EMPLOYEE'S THUMB, FRACTURING IT.
EE CLAIMS HE FELT A "RIPPING" SENSATION IN HIS BACK WHILE PULLING RAILROAD SPIKES. HE IS CURRENTLY LOSING TIME FROM WORK. QUESTION IF WORK RELSATED DUE TO OTHER CIRCUMSTANCES.
