Struck against stationary object

Employee cut a box open with a utility knife and placed the knife on top of the box. When the employee pulled the box open with EE's left hand EE's left index finger make contact with the blade of the utility knife resulting in a cut to EE's left index finger.

North Railroad switch in North Branford Quarry was frozen. Heat was used to melt the ice on the switch. It freed up but not all the way. Employee used pry bar to see where switch was hanging up. Pry bar slipped and employee fell backward onto right wrist.

WHEN THE PUSHER BOLT WAS BACKED OFF PARTIALLY THE LINE UP PIN WAS REMOVED BECAUSE IT WAS LOOSE IN THE BORE. WHEN HE TOUCHED THE END CASTING IT RELEASED THE SPRING TENSION PROPELLING THE HOUSING STRIKING HIS HAND AGAINST A WORKBENCH VISE.

EE WAS GETTING READY TO DISCONNECT A STRING OF AMTRAK CARS REQUIRING HIM TO SHUT OFF THE AIR TOTHE 2 AIR BRAKE LINES. HE THOUGHT HE FORGOT TO SHUT OFF THE AIR IN THE LINE WHEN IN REALITY THE Y WERE ALREADY IN THE OFF POSITION. INSTEAD HE TURNED THE VALVES TO THE ON POSITION SENDING 150PRI OF COMPRESSED AIR THROUGH THE HOSES STRIKINGHIM IN THE FACE.
