Swinging small sledge hammer to replace ripper shank on a dozer and the head flow off in the middle of a down swing causing weight change in the handle and causing the miner to strike his middle finger on a piece of the shank. Cutting right middle finger.

The injured employee was working near and in the area below the sag mill. The employee above knocked an open end box wrench off of the nut he was tightening with his elbow. The wrench fell 3 feet and hit the employee below in the lip, causing a laceration.

AT APPROX. 1445 ON 7-15-02 A TEMPOARY EE FROM A STAFFING AGENCY RECIVED A CUT TO HIS LEFT HAND WITH A PERSONAL KNIFE. THE TEMP WAS CUTTING A ONE HOSE WHEN HIS KNIFE SLIPPED FROM THE CUTTING AR EA AND CUT THE OUTSIDE OF HIS LEFT HAND.

EE WAS USING A SOFT FACE HAMMER TO REMOVE CATHODE SHEET. DURING MID SWING OF HAMMER THE HEAD LIP LACERATION AND LOOSENING OF SEVERAL FRONT TEETH REQUIRING STITCHES TO LIP AND GUM. EE HAS BEEN RETURNED TO WORK BY PHYSICIAN. NO TOOTH LOSS INCURRED.
