Showing all 7Ee was removing a frozen valve 3" to thaw it out once all bolts were removed the valve was stuck in place. He pulled on valve when it broke loose it. He did not have a good grip on it and it fell on his wrist the valve weighed approx. 40 lb.
EE WAS USING A PRYBAR TO MOVE A PIECE OF HEAVY STEEL WHEN THE PRYBAR SLIPPED PINCHING HIS FINGER BETWEEN THE PRYBAR AND A NEARBY I BEAM. ADEQUATE INFORMATION WAS NOT PROVIDED TO DETERMINE REPORTABLE TO THE MAIN OFFICE UNTIL 1-12-06.
EE WAS TRYING TO STRAIGHTEN A BENT SPUD BAR BY PULLING IT AGAINST THE BEND HE PULLED TOO HARD & HURT HIS BACK.
REMOVING WEDGED ROCK FROM JAW CRUSHER USING SPUD BAR. COWORKER LOST GRIP ON SPUD BAR, STRIKING EMPLOYEE IN HAND. THE SHAFT OF THE SPUD BAR FIRST STRUCK HEAD CAUSING HARD HAT TO FALL OFF EXPOSI NG HEAD WHEN BUTT THEN STRUCK HEAD.
PULLING ON WRENCH TO GET TIGHT BOLT LOOSE.
EE WAS LOSENING BOLTS ON THE LID OF PRIMARY CRUSHER TO DO MAINTANCE INSIDE WHEN THE WRENCH HE WAS USING SLIPED OFF CAUSING EE TO LOSE HIS BALANCE AND FELL BACKWARDS APPROX. 3 FT LANDING ON A S UPPORT TOWER FOR THE CRUSHER LID WHICH INJURED HIS LEG HE CAUGHT HIS FINGER TRYING TO STOP HIS FALL.
