Employee was hammering on a piece of metal clamped in a vice when his hammer came in contact with the plant structure on the down swing causing the hammer to deflect into his thumb. Laceration to left thumb requiring six sutures. Vice was relocated to an area without obstructions.

Fall down stairs

Injured employee and another worker just finished repairing a damaged belt. Injured employee was picking up extension cords to take back to the tool trailer. When he started down the stairs he slipped and fell resulting in a ligament sprain to the right foot. The steps were not wet or slippery and there was nothing on the steps creating a tripping hazard.

Struck against stationary object

The employee had just finished filling the water truck with water and was in the process of disconnecting the water hose from the truck. He took a step back and stepped on a #4 size rock. This caused him to lose his footing, resulting in a sprained Right ankle and knee.

Employee was walking by a wood work bench in the Lab when his left hand brushed the bench. From this contact the employee received a splinter in his knuckle of his Left Index finger. Employee was taken to the ER where he received 4 stitches.

Operator was driving rock truck on 11-8-07 for about 2 hours. On 11-9-07 at noon he reported to his supervisor that he hurt his neck on 11-8-07 while driving rock truck.

EE WAS GETTING A SAMPLE OF #8 GRAVEL FROM THE STOCK PILE AT PLANT IN A THREE GAL BUCKET AFTER PUTTING APPROX 15 OB OF GRAVEL IN THE BUCKET HE LEANED OVER TOOK A COUPLE OF STEPS WHEN HE FELT A SHARP PAIN IN HIS BACK.

EE WAS TRYING TO STRAIGHTEN A BENT SPUD BAR BY PULLING IT AGAINST THE BEND HE PULLED TOO HARD & HURT HIS BACK.

EE WAS ENTERING HIS TRUCK TO START SHIFT WHEN HIS FOOT SLIPPED OFF THE STEP INTO HIS TRUCK. HE LOST HOLD WITH HIS RIGHT HAND & TWISTED HIS BACK. STEPS WHERE WET FROM HEAVY RAIN.

GOT OUT OF MY CAR AFTER CHECKING THE FUEL LEVEL IN THE FUEL TANK. MY FOOT WENT OUT FROM UNDER MEON THE SNOW AND I LANDED WITH MY LEFT LEG UNDERNEATH ME.

CUSTOMER ON MINE SITE TO GET A LOAD OF GRAVEL BUT BEFORE DOING SO HE RAISED THE BED OF HIS TRUCK TO CLEAR IT OF DEBRIS. THE BED WAS UP APPROX 18"" TO 24"" WHEN A PROBLEM WITH THE HOISTING MECHAN ISM CAUSED THE INJURED TO REACH IN BETWEEN THE RAISED TRUCK BODY AND THE TRUCK FRAM (THE TRUCK BODY WAS NOT BLOCKED AGAINST MOVEMENT) WHEN THE TRUCK BED FELL ON HIS ARM.

EE WAS LOSENING BOLTS ON THE LID OF PRIMARY CRUSHER TO DO MAINTANCE INSIDE WHEN THE WRENCH HE WAS USING SLIPED OFF CAUSING EE TO LOSE HIS BALANCE AND FELL BACKWARDS APPROX. 3 FT LANDING ON A S UPPORT TOWER FOR THE CRUSHER LID WHICH INJURED HIS LEG HE CAUGHT HIS FINGER TRYING TO STOP HIS FALL.
