Employee was assisting with the replacement of a head-drum shaft bearing on the #1 screen conveyor on the Portable Plant. The chain that was used to support the head-drum and shaft assembly snapped, striking Tyler Whipple middle finger on left hand, fracturing the 3rd digit / tip of finger.

Showing all 12Employee was attempting to remove the swivel adapter from the auger head. A 36-inch pipe wrench was attached to the adapter and while attempting to rotate the auger shaft, the auger shaft unexpectedly rotated with the pipe wrench attached. The pipe wrench struck EE in the right chest and pushed EE against the back of the drill truck.
EE was removing a worn bucket tooth on the 990H Loader. When the tooth failed to separate from the bucket adapter, EE used a sledgehammer to strike the tooth resulting a piece of shrapnel from the tooth striking EE in the chest and becoming imbedded.
Employee cutting belting for back flaps for feed ends of screen. Employee had the belting on the ground cutting, while on knees the utility knife slipped from the rubber, causing small laceration to inner left side of right knee.
EE using sledgehammer to dislodge frozen material from feeder, sledgehammer ricocheted back, hitting EE right foot, slamming the foot into the side of the feeder.
Unwiring an electric motor and utility knife slipped when cutting tape and sliced through coveralls, jeans, and long johns to make lacerate inside of left thigh. A few stitches and no lost time.
Employee was cutting tape on wires to a motor and it was snow covered, wet, and slippery and the utility knife slipped causing it to cut through his gloves and lacerate the back side of his left thumb.
EE was using a vise grip pliers to close the end of an s-hook from a hold down strap. The vise grip pliers slipped off of the s-hook and pinched his left middle finger.
IE was changing screens in the middle deck of #4 screen with co-worker. Co-worker was swinging a 4 lb hammer to set the screen in the screen deck. IE inadvertently put his left hand next to the area that co-worker was swinging and IE's left middle finger was struck by the hammer.
EE was removing a rock from the feeder grizzly. The sledge hammer head came off the the handle causing the handle to strick the rock which over extended ee left elbow. (acute strain).
EE was lifting a ratchet chain hoist in place to raise the gravity take-up weight for the Pit Long Belt Unit # 34-135-S. While lifting the ratchet chain hoist in place he strained a muscle in his right shoulder.
EE WAS HAMMERING WEDGES IN TO TIGHTEN THE SCREENS ON PLANT. THE ONE BEHIND THE MOTOR, WHEN STRUCK, CAME BACK OUT HITTING EE BY THE LEFT EYE.
