Struck by flying object

Employee was assisting with the replacement of a head-drum shaft bearing on the #1 screen conveyor on the Portable Plant. The chain that was used to support the head-drum and shaft assembly snapped, striking Tyler Whipple middle finger on left hand, fracturing the 3rd digit / tip of finger.

Team members replacing return roller, shaft began to slip, team member grabbed the shaft, causing pain to the right wrist.

Employee was attempting to remove the swivel adapter from the auger head. A 36-inch pipe wrench was attached to the adapter and while attempting to rotate the auger shaft, the auger shaft unexpectedly rotated with the pipe wrench attached. The pipe wrench struck EE in the right chest and pushed EE against the back of the drill truck.

EE was washing Tower #2 back pad, EE shut the fire hose off went to change locations & started to slip, falling backwards, but caught self with left hand on a pillar causing a strain to right neck & shoulder. Injury Type: Neck Strain. EE didn't request med. treatment at the time. Was seen by a medical provider on 12/26/23 when injury became reportable due to restrictions.

Over-exertion in lifting objects

EE was lifting a return roller and handing it to a co-worker when EE twisted at the waist and strained back. (Lifting a Return Roller). Update: EE resigned, was not terminated by Knife River South Dakota.

EE was removing a worn cutting-edge bolt from the loader bucket. When EE went to remove the nut, the bolt dropped and continued to spin when EE attempted to secure it by grabbing it with EE's left hand, cutting EE's left index finger.

Struck by flying object

EE was removing a worn bucket tooth on the 990H Loader. When the tooth failed to separate from the bucket adapter, EE used a sledgehammer to strike the tooth resulting a piece of shrapnel from the tooth striking EE in the chest and becoming imbedded.

EE was pulling a 1 1/2 inch fire hose to clean off piece of equipment, as EE was walking backwards, tripped in rut and fell to ground.

Employee cutting belting for back flaps for feed ends of screen. Employee had the belting on the ground cutting, while on knees the utility knife slipped from the rubber, causing small laceration to inner left side of right knee.

EE dismounting loader ladder, boot tread got caught in the bottom rung of step, fell to the ground, striking left knee on concrete floor. EE fell on 3/4/20, reported to clinic on 3/5/20. On 3/16/20 EE given physical therapy.

Employee was exiting loader with ladder, right foot already on group, when left foot became caught on bottom step, causing employee to lose balance and fall to ground. Employee tried to catch self and fracturing left wrist.

Window in loader was being closed, 5th fingertip slipped and was smashed between window and trim.

Employee pulling down on cable jack while in a man lift and felt a pull in right shoulder.

Employee was washing material on the back pad under Tower 2 with a fire hose. Employee lost control of the fire hose and it hit them in the upper chest, below the collarbone. (Chest wall contusion) Employee did not advise the supervisor until 4/23/2018 that they requested medical treatment.

EE was walking to haul truck when EE slipped and fell on packed snow straining Rt. knee.

Employee had completed cutting an H Beam on the band saw in the welding shop. EE picked up the cut off piece off the band saw that weighed 30 pounds and felt a pain in back.

EE was carrying a screen in Tower 2 to position it so that it could be installed in Screen 3. When EE picked up the screen EE felt a pain in right shoulder and bicep.(This incident was discussed with EE from the Denver Co. MSHA office on June 22, 2016 and directed to file an injury report due to the PT ordered by the Physician on 6/21/2016).

Personnel was loading scrape metal onto a flatbed trailer his glove caught a piece of slag and caused the metal to pinch his finger between the metal and the wooden side boards of the trailer.

Unwiring an electric motor and utility knife slipped when cutting tape and sliced through coveralls, jeans, and long johns to make lacerate inside of left thigh. A few stitches and no lost time.

Employee was cutting tape on wires to a motor and it was snow covered, wet, and slippery and the utility knife slipped causing it to cut through his gloves and lacerate the back side of his left thumb.

Employee was doing a pre-shift inspection on Loader unit #15-415. He was inspecting the z bar links, pins and bushings on the left front of the machine. He leaned over to inspect the machine, slipped on snow that had fallen the night before. He fell and his left side (upper chest) fracturing three ribs when he contacted the lift arms on the loader.

Employee was washing the HP 400 crusher decks off with a fire hose before scheduled crusher maintenance. He tripped on a small rock and fell to the crusher deck dislocating his left shoulder.

IE was changing screens in the middle deck of #4 screen with co-worker. Co-worker was swinging a 4 lb hammer to set the screen in the screen deck. IE inadvertently put his left hand next to the area that co-worker was swinging and IE's left middle finger was struck by the hammer.

Employee fell and strained his right knee while running to the break room to avoid getting wet when it began to rain.

While hoisting a 3'x20.5' sheet of corrugated metal siding, the sheet was caught by a gust of wind causing the clamps to fail. The 20#sheet fell to the ground from a height of approx. 10' striking injury. in his attempt to get out of the way he fell to the ground. He suffered a bruise to his abdomen. Slight damage to the sheeting/siding.

The service vehicle was used for transportation to and from the field. The service truck was unoccupied and parked before the fire incident. At this time the cause of the vehicle fire is still under investigation but is believed to be electrical in nature.

Fire in an unoccupied structure: The fire is still under investigation, but it is believed to have been started by repairs associated with welding activities.

EE had finished washing out material from under the conveyor and was in the process of stowing the fire hose when he slipped backwards extended his L hand to caught himself and dislocating his left pinkie finger or pip joint. Note:EE was wearing his hard hat,safety glasses,high visibility garment,gloves & steel toed boots.

Wind was blowing approximately 30 mph - he was changing out the south HP crusher. Dust blew in face and right eye.

Employee checks pumps on weekends. Cart tipped down into water while greasing pump. Tried to keep cart from falling into water while at the same time adding rock for weight to tongue to prevent it from falling into water.

WHILE ON THE INCLINED CATWALK BETWEEN #2 & #3 SCREENS, OPENING THE SPRAY BAR ON #3 SCREEN. AFTER OPENING 2 VALVES, STOOD UP TO MOVE TO ANOTHER SET OF VALVES. A SHARP PULL WAS FELT IN LOWER BACK.

EE HAD STOPPED HIS TRUCK TO REMOVE A ROCK FROM THE HAUL ROAD. WHILE LIFTING THE ROCK FROM THE ROAD, HE STRAINED HIS BACK. HE WENT HOME AND THE NEXT MORNING HE COULD NOT GET OUT OF BED, AN AMBULANCE WAS CALLED TO BRING HIM TO HOSPITAL.
