EMPLOYEE STATES HE WAS STANDING ON THE THIRD STEP OF A 6 FT. STEP LADDER. HE WAS PULLING ON A 15" WRENCH WHILE USING A CONDUIT KNOCK OUT PUNCH ON TOP OF AN ELECTRICAL PANEL. THE WRENCH SLIPPED OFF CAUSING HIM TO LOSE HIS BALANCE AND FALL OFF THE LADDER. HE LANDED ON THE HEEL OF HIS RIGHT FOOT.
