Showing all 19EE was attempting to enlarge a hole on a rubber skirt. The knife EE was using slipped and cut the web of EE's other hand. The EE received one (1) suture in their hand.
Employee's right finger was pinched between a come along and a metal plate causing a break to occur requiring a splint and restrictions.

IE was assisting a fellow EE in the removal of a water pump. It was being lifted with the aid of a come-a-long. It appears there was a mechanical failure in the lifting device that caused the pump to drop suddenly. When the pump dropped it jerked his arm as he was holding onto the chain guiding the pump. This caused a strain to his right arm. He returned to modified duty.
Employee was doing maintenance on a crusher and caught his finger between bar and side of crusher
EE was trying to unplug the chute on the jaw crusher when a plate fell down in the chute and he was using a prybar to try to pry the plate loose when his finger got caught between the bar and the chute. This pinched his middle finger on his left hand. His finger is fractured.
Pocket knife in hand, cutting tie strap, knife slipped cutting left hand between thumb and index finger.
Conveyor belt needed a small piece cut off of it. Employee had a utility knife, pulling it towards himself slipping off of belt & cutting himself.
EMPLOYEE WAS SPLITTING STONE WITH A HAMMER & CHISEL. HE ACCIDENTALLY HIT HIS HAND WITH THE HAMMER.
EMPLOYEE WAS TAKING GREASE FITTING OUT OF ADJUSTMENT CYLINDER FOR TAILPULLEY & PRESSURE IN CYLINDER SHOT GREASE FITTING INTO HIS LEFT HAND BETWEEN INDEX & MIDDLE FINGER CAUSING PUNCTURE WOUND.
INJURED WAS HOLDING A DRIFT PIN AND CO-WORKER WAS SWINGING A SLEDGE HAMMER AT IT, WHEN HE STRUCK HIS THUMB WITH THE HAMMER.
EE WAS TRIMMING BELTING TO FIT BACK OF SCREEN WITH A UTILITY KNIFE. EE WAS KNEELING AND KNIFE WENT THRU BELTING AND CUT HIS INNER THIGH ON HIS LEFT LEG.
EE WAS TORQUEING BOLTS TO 475 FT. LBS. ON WAS SCREEN WITH A TORQUE WRENCH WHEN IT SLIPPED A GEAR CAUSING EE TO INJURE HIS SHOULDER.
EE SAW BIG HUNK OF MUD ALONG SIDE OF BELT. HE WANTED TO PUSH IT OUT. BELT WAS STOPPED. HE PICKED UP MUD FORK AND INVERTED IT TO POKE IT THROUGH. PUSHED FORWARD AND SPIKE OF FORK HIT OUTER EAR RIGHT.
EE WAS REMOVING ANVILS FROM CRUSHER ANVIL RING WITH SLEDGE HAMMER CAUSING A PIECE OF ANVIL TO BREAK OFF AND HIT VICTIM IN LEFT KNEE GIVING HIM A LACERATION THAT TOOK 3 STITCHES ON INSIDE AND 3 STITCHES ON OUTSIDE.
