Struck by falling object

The EE was loading lifters when a piece of rock (approx. 4'x 5' x 2') fell from the ceiling and struck the EE. The EE was not pinned under the rock. This resulted in a fatal accident.

Fall to the walkway or working surface

EE was on the ground helping load a face for a shot. The EE stepped on a rock and twisted EE's left knee and fell to the ground. Work Care was called and recommended a clinic visit. The clinic did an x-ray and said nothing was broken and referred EE to an orthopedic doctor. EE visited the orthopedic doctor on 7/29 who placed the worker on restricted duty and physical therapy.

Struck by falling object

EE was spraying a hose when it slipped, hitting the right forearm. The hose clamp caught their arm causing a laceration.

EE was attempting to enlarge a hole on a rubber skirt. The knife EE was using slipped and cut the web of EE's other hand. The EE received one (1) suture in their hand.

Struck against stationary object

While opening the access door on the ANFO truck, the EE's right index finger struck the tanks frame. A jagged/rough area cut through the EE's glove causing a tear drop shaped laceration across EE's right index finger. The EE needed ten (10) sutures to close the wound.

EE was descending down the ladder on the scaler. As they did, a foot slipped off of the second (2nd) step. The EE did not fall. They held on to the railing with their left hand and momentum spun their body. This caused their left shoulder to dislocate.

While closing the gate to the mine, the wind caught the door and slammed into the EE's foot

Accident type, without injuries

No injuries to persons/no miners exposed. Incident occurred overnight, no one was underground. Accident investigation is ongoing to determine cause, including if there was a pillar failure. Area affected by the fall included NW corner of M29&S37 to the NE corner of M29&S49 to the SE corner of S49&M47 to the SW corner of M47&S37 and back N to M29&S37. No equipment involved.

The tunnel belt head box became jammed with rock which was impeding operation. Trying to clear the jam with a pry bar, the workers hand was impinged between the bar and I beam.

Worker fractured and lacerated right index finger while tightening a nut and bolt on a crusher. The wrench sprung off the nut from the stored tension and struck the finger, smashing it between the wrench and the base of crusher.

Miner was walking from a face drill to a man-lift, distance of about 150ft, when EE lost footing causing discomfort and some pain in the left knee. The miner reported the walkway/work area was illuminated (cap lamp, drill lights & man-lift lights) and was free of slip & trip hazardous.

Truck operator injured shoulder as EE was climbing up access ladder of CAT 773.

Struck by falling object

Worker fractured right long-finger (metacarpal). EE was cleaning travel way at the scalping screen when a rock fell from the tunnel belt striking employee's right hand. EE's hand was rested on steel railing at moment of impact.

On 3/15/17 the employee slipped and fell. EE was treated and released to full duty at that time. As days/weeks/months went by EE began having pain in right shoulder. A medical exam in June 2017 revealed a shoulder injury likely caused by the fall and requiring surgery. Surgery occurred July 5th.

Haul truck driver sprained their right ankle when EE slipped from the last step of the truck and fell to the ground as EE was dismounting the truck.

Worker received a contusion and swelling to his right foot. He another miner were attempting to reattach a 2A chute door when the door fell from a 4inch crib block and landed on injured worker's foot.

Worker received contusions to his lower leg when struck by the cover from a 300Hp motor's junction box. Cover was blown from box just as the slow-start motor was initially beginning to be energized. No signs of an arc-flash or overloaded motor. Fuse was intact following the incident.

Worker received a shoulder strain while pushing down on a pry-bar during repairs on a front end loader. He requested medical care the morning following the incident.

Fall onto or against objects

Haul truck driver was beginning to climb up truck's access ladder when the rubber hanger for the bottom ladder rung broke. This caused worker to strike right side of his torso against the truck bumper. Worker refused medical treatment but requested then requested treatment 3 days later.

Fall from machine

Worker fell apx 3 to 4 ft from catwalk of mechanical scaler while performing pre-shift exam on the mobile equipment. He stretched up to release a door to engine compartment when he lost balance and fell. He landed back-side first on mine floor against battery pack of his cap lamp.

Employee was working to unplug drill stem that was being used to drill blast holes in working face. He realized the stem was plugged once he noticed a stoppage in the water flow from the drill holes.

Employee was walking around the ANFO loader when he tripped over a rock and landed on his left elbow. EE had a metal plate surgically implanted in his left elbow a few years prior in an injury unrelated to the mine or current case. The 9/9/08 incident aggravated the ee's left elbow & surgery to remove the plate implant was performed on 11/7/08.

Employee was walking down the steps at the stone processing plant by the 1560 Crusher and overstepped a step causing his foot to go beyond the stair tread, pulling his knee. He did not request medical attention at the time of the incident. Subsequently, he was examined at his request by a medical doctor on 8/25/2008 and was placed on restricted duty.

THE INJURED EE MOVED A COME-ALONG WHICH HAD A FRAYED CABLE. A WIRE BROKE OFF THE CABLE AND WAS PROPELLED UPWARDS TOWARDS THE EE'S FACE. IT ENTERED FROM THE SIDE AND PASSED BETWEEN HIS SAFETY G LASSES AND FACE, CAUSING A CORNEAL ABRASION. HE WAS GIVEN A PRESCRIPTION TO PREVENT INFECTION.

EMPLOYEE CLIMBING INTO POSITION TO ADD HYDRAULIC FLUID TO SCALER STEPPED DOWN & SLIPPED IN SOME OIL & FELL. HE SAID HE TWISTED HIS LEFT FOOT, STRUCK LEFT ELBOW & CUT HIS LEFT THUMB. AT TIME OF ACCIDENT HE DID NOT INDICATE THAT ANY MEDICAL ATTENTION WAS NECESSARY.

EE WAS OPERATING A ROCK TRK THAT WAS LOADED WITHSILT. TRUCK WAS TRAVELING UP A STEEP GRADE WHEN THE LOAD SHIFTED AGAINST THE TAILGATE CAUSING THE TRUCK TO TIP STRAIGHT ONTO THE TAILGATE. AFT ER THE LOAD EMPTIED ONTO THE ROAD THE TRUCK SLAMMED TO THE GROUND UPRIGHT ON ITS TIRES

EE WAS ON THE LEFT SIDE OF TUNNEL BELT CLEANING UP AROUND THE PLANT WITH THE MUSTANG. HE HAD THE BUCKET IN AIR WITH MATERIAL IN IT AND WHEN HE HIT THE BERM THE MUSTANG TIPPED FORWARD.

EMPLOYEE WAS AT TAIL WHEEL OF 2A BELT CHANGING DIRECTION OF 2A BELT. MATERIAL BUILT UP AT REAR,UNLOCKED ELECTRIC BOX TO CLLEAN OUT BUILT-UP MATERIAL. PLANT OPERATOR STARTED 2A BELT, TAIL WHEE L CAUGHT OVERALLS PULLING LEG INTO TAIL WHEEL.
