An employee was attempting to remove built up material next to load cells in a hopper. EE attempted to use a 5 foot long bar to remove this material. When EE got to more consolidated material, the employee used a hammer to strike the spade end of the bar assist in breaking up the material. A foreign particle became airborne and struck the employee in the chest and was embedded.

Showing all 6EE was removing worn center section on bucket lip of #3 992 Cat loader in mobile shop. EE was attempting to dislodge section from bucket by striking the section with 2 lb hard surface hammer. Small sliver of metal segregated from section edge striking him on left side of abdomen & embedding slightly under skin. EE was seen by physicain (lanced & sutured) & released to full duty.
Employee fell while trying to dislodge a rock from a self-cleaning tailpulley.
EE WAS ATTEMPTING TO DRIVE A BEARING OFF A 4 3/8" DIAMETER SHAFT USING A 3/16 HAMMER AND A PIECE OF KEYSTOCK. HE MISSED THE KEYSTOCK AND STRUCK HIS HAND WITH THE HAMMER. EE REC. 3 STITCHES AND WAS RELEASED TO WORK.
EE WAS CUTTING A MOUNTING HOLE IN RUBBER CHUTE LINER WITH UTILITY KNIFE. KNIFE SLIPPED AND HE CUT HIS LEFT INDEX FINGER WHERE IT JOINS AT HAND.
WHILE TIGHTEN BOLT THE WRENCH SLIPPED OFF CAUSING A LACERATION TO RIGHT FOREARM.
