Changing tire on skid steer, employee was using a ratchet strap to wrap tire to aid in seating the bead for inflation. Once the bead was set and able to receive air the rachet strap was released and the handle sprung back and caught the employee in the right forearm causing a 2" laceration that required 5 stiches.
IE was using a pocket knife with a 3-1/2" blade to cut a rubber mud flap for better fitment on a pickup truck. While making the cut, the IE repositioned the mud flap and the knife cut the IE on the inside of the left thumb resulting in 4 stitches to close the laceration.

Employee was assembling a machine in the workshop under normal conditions and procedures. While tightening bolts with a wrench (standard process, no unusual conditions) felt a "pop" in the right hand shoulder. Minimal pain was felt immediately however the pain grew worse over the next hour. Employee was then driven to medical services for treatment
While unloading ANPAN, driver was using rubber mallet to loosen material from side walls of pneumatic trailer when they bent down about 1/4 bend to strike the pod and felt a dull pain in back.
Employee was stripping a communication cable with a knife and the blade cut through the cut resistant gloves resulting in a minor cut on their left thumb.

Employee & partner were in the process of separating the motor & gearbox of #2 flocculent delivery pump with a come along to pull them apart. Employee was waiting for partner to return, looking toward the truck when the motor & gearbox suddenly separated under the pressure of the come along, the chain of the come along caught employee's hand & smashed it against a nearby pipe.

While helping to stage dozer tracks by straightening them out, employee attempted to straighten the track out by striking it with a track pin. After striking it three times, a small piece of metal sheared off the track pin and embedded in the employee's right knee

Employee was using a Husky utility knife to remove a rubber boot that was stuck on the intake tube of a haul truck. While cutting on the boot, the knife slipped from the rubber and cut the EE's forearm. Coworkers helped EE wrap the wound. EE received 7 stitches.

While installing Mill insert on ball mill 3 it became jammed. While trying to maneuver the component for final placement, the insert popped out of place, the come along tools assisting with the job failed sending the component into a free spin striking 4 employees.

While using hand hydraulic ram to position rear hoist bushing, the ram slipped off the bushing and the pump spun and slammed to the floor with employee's hand in a down force motion causing a wrist pain

Employee was operating a come-along to position a D11 dozer blade stabilizer into position to connect to the machine. Come-along was improperly rigged and the chain was on the safety latch and broke free and stabilizer fell onto employee's right foot.
Employee was installing tracks on CAT Track Dozer D10 - 27 on the right side. The tracks kinked and the employee attempted to un-kink it with a 12lbs hammer. As employee struck the track, fly-metal struck employee's right leg.

Two employees were inside the Ball Mill, attempting to remove a ball chip that was lodged between two liners. A 4lb metal wedge was inserted, between the two liners, to try and hammer it out. One employee swung the hammer while the other stood back. Upon contact with the hammer the wedge bounced back and struck the employee in the mouth.

Employee reported feeling pain in right shoulder and hip after lifting a 16 lb hammer. This was not reported immediately due to an investigation of the incident. MRI was scheduled for 12/03/15 - results have not been received.

While removing a bit from a sub with a hammer, a small piece of metal was lodged into employee's right arm. Doctor recommended leaving it, but employee got second opinion. Became lost time incident on 07/27/12 due to surgery to remove metal.

Employee was using a 12# hammer to strike liners for removal in the ball mill upon striking a liner a piece of metal frayed and stuck employee in right leg. The metal went undetected until 10/18/2011
Break out wrench lost bite of drill rod and kicked back in driller assistant's hand before regaining bite. The employee's wrist was originally examined and first aid care was administered. On 7/21/2011 an MRI showed a minor fracture of wrist bone and torn cartilage.
