During an attempt to remove rigging from a hook on a forklift, the operator extended the boom to assist the rigger. Unfortunately, this action caused the rigger's hand to become trapped between the knuckle of the mat and the item they had just lifted, resulting in three broken bones in EE's right hand.

While removing a catwalk by the plenum, a forklift fork became caught under the platform and lifted it slightly, grazing a welder's shin and causing a small cut above the left ankle.
Changing tire on skid steer, employee was using a ratchet strap to wrap tire to aid in seating the bead for inflation. Once the bead was set and able to receive air the rachet strap was released and the handle sprung back and caught the employee in the right forearm causing a 2"" laceration that required 5 stiches.
HT135 Operator pinched/lacerated finger by stuck/malfunctioning boarding ladder.
The injured employee was attempting to cut the lower mud ring on a gear half that was positioned on a concrete floor, with the split line partially overhanging a small concrete retaining wall. The employee reached with both hands under the gear to cut the mud ring, and the gear shifted, striking them in the hard hat and both hands.
Mine Ops Employee experienced heat dehydration/heath exhaustion.

About an hour after their shift, Employee reports the pain their leg had intensified to the point that they could barely walk, at which time they contacted their supervisor to report it.

EE was organizing lower tire yard. While moving wheels to scrap, EE had stacked Wheel #@1 on top of wheel #2, when wheel#3 that was standing next to the stacked wheels fell into wheel#2 while EE's left hand was in the line of fire, resulting in a broken left ring finger above the first joint.
On 3/24/25 at approximately 11:37 AM a mayday was initiated for an employee who passed out. CPR was initiated and the employee was shocked with an AED multiple times. WVFD arrived on scene and took over CPR but was unable to revive the employee. The scene was secured and legal notifications were promptly made.
Employee lost footing while looking for rocks/minerals while taking a break from the rock wall logging work being performed. The employee used the rock wall to brace from EE's fall and received a laceration to EE's left pinky finger that resulted in three stitches.
MIS Employee felt a popping sound while ascending stairs, felt pain in leg.
Employee's right index fingertip was amputated inside the pin hole of the Tube Press 6 ram when the Overhead Crane raised the yolk out of the candle eye.

Driller helper had right foot on the drill rod bin skid while marking the drill pipe. When EE stepped down off the skid, 8"", EE rolled left ankle. After alerting the mine safety team and following their protocols, they went to a clinic where x-rays were negative. They diagnosed EE with a sprained ankle and put EE on restricted duty.
On 1/16/25 at 11:40am worker was putting pins on a shovel track. As worker stepped down the worker stepped on a rock making rt. ankle twist. ERT on site assessed the injury and recommended a medical evaluation. Diagnosis of a rt. ankle sprain. No fracture. Minor restrictions have been recommended. Employee follow up with clinic in 2 weeks.

Employee walking into work at Mine Ops tripped and fell on the grating in front of the Engineering Building, resulting in a laceration that required stitches. Employee was released to return to work without restrictions.

Dozer started leaking hydraulic fluid. Employee pulled the dozer to a safe location, Witness #1 noticed flames on the right side engine compartment. Witness #1 called the ""Mayday"" and instructed Operator to activate the fire suppression on the dozer. NOTE: Incident occurred 12/7. Injury reported 12/16.
Employee was air-arcing on the revolving frame beneath a P&H 4100A shovel. Hot slag entered through the employees protective coveralls and fell down into EE's steel-toed right boot. The employee sustained 2nd and 3rd degree burns on the top of EE's right foot.
While unloading ANPAN, driver was using rubber mallet to loosen material from side walls of pneumatic trailer when they bent down about 1/4 bend to strike the pod and felt a dull pain in back.

The chuck closed and EE's glove caught in the chuck and spun. EE's right 2 middle fingers were lacerated by the glove material tearing when caught in the rotation.
2/16, Employee injured knee when exiting vehicle. This incident was a later report due to the employee didn't notified supervisor of their injury until days later via email. Employee was seen at the clinic on 03/04 with RD as the diagnosis.

On the above date and time trainee and trainer on 209 was backing under the bucket at shovel 11. Neither the trainee or trainer heard a shovel horn, and the bucket tripped dropping the muck in the bed. The trainee was still backing up, and put the truck in neutral and quickly set the park brake.
Haul truck operator backed up to berm to dump load. The bed would not come up due being loaded heavy on the canopy of the truck bed.
Shovel operator tripped the dipper near the dove tail resulting in rough loading. Haul truck operator felt some pain in their back.
Employee slipped while descending stairs and twisted right ankle.
Employee felt ""pop"" in their knee while walking up to the grind floor.

At approximately 6:35pm the operator of HT152 was getting loaded at SH23, during the backing up process the operator of HT152 came in contact with the dipper of SH23 causing the dipper to trip releasing the material into the back of the truck. Incident is under further investigation.
Operator backed over a boulder in D-10-25 and jarred self when it hit the ground hurting EE's shoulder.
Operator let the clutch out on blade 18 and it lunged forward whip lashing EE hurting EE's back.
EE was changing out the petron barrel under grind line 3. The barrel EE was changing out was half full of petron. To replace the barrel EE put the lid on existing barrel and tried to roll it out of the way. During this process the barrel slipped and caught EE's ring finger on the left hand and pinched it against the back wall.
Employee stated EE felt pain in the neck area starting at 11 AM on 6/8/2020. Employee believes the pain was caused by sitting at the truck desk for long periods of time. Employee called Supervisor on 6/10/2020 and stated was going to the clinic for treatment.
While working at the truck desk at the security gate employee felt pain in EE's neck.
Employee was doing a pre-shift on grader. While checking the engine oil, they grabbed the mounting bracket, put a foot on the step, then grabbed the other mounting bracket, slightly twisted motion pulled a right lower back muscle.
