Showing all 28Belt repairs being installed. Employee was cutting a v-shape cut at edge of belt and utility knife slipped, cutting hand.
Employee was removing the cap screw on the filter housing of the HP 300 cone crusher using a 36" aluminum pipe wrench. The wrench slipped off the nut causing the employees hand to strike a stationary object.
Employee was attempting to free a screen tightening rail with a four foot bar and the rail came loose. The bar then came back and struck the employee on the left side of the face causing safety glasses to cut employee just above the eyebrow.
Individual was cutting flashing and the blade slipped, cutting the bottom part of the thumb. The cut required 3 stitches.
The miner was installing screens on the bottom deck of a rinse screen. As he was swinging the hammer hit the cross member above deflecting the hammer striking his left middle finger causing a fracture to the tip of that finger. The impact of the hammer chipped the bone on the end of his finger.

Installing large bull gear on roll crusher at Wash Plant One, using a 8 lb. hammer to strike a hammer wrench to secure the nut that holds the gear. Hammer glanced off wrench and hit left knee.
While EE was installing screens in the middle deck of the 6x20 he struck his little finger with a 4lb sledge. Causing a laceration and fracture. EE's aim was deflected when he struck the deck at the beginning of his swing.
EE was assisting another employee in removing bucket from skid loader. Bucket pins were stuck. Lifted bucker approx 6" in air & struck handle with hammer. Lower pin released, bucket swung forward, came off upper bucker receptical & swung forward approx 12" - 16" landing on ee's right foot.
Employee was pulling on a bar that became stuck. As he pulled back on the bar, it came loose and he trapped his thumb in between 2 bars.
EMPLOYEE HURT HIS LEFT FOREARM WHILE HE WAS ALIGNING A CRUSHER FRAME, USING A PRY BAR TO SHIFT IT INTO PLACE TO ALIGN BOLT HOLES.
Ee was cleaning between impact rollers on feeder with a bar when his bar get caught between the roller and hopper above. his hand got caught between the bar and the roller cutting his hand betwen his thumb and forefinger.
EMPLOYEE WAS TIGHTENING BOLTS ON SCREEN BEARING HOUSING HOLDING A WRENCH WHILE ANOTHER EMPLOYEE WAS HITTING WRENCH WITH HAMMER. EMPLOYEE WITH HAMMER MISSED, HIT COUNTERWEIGHT & HAMMER BOUNCED ONTO INJURED EMPLOYEE'S RIGHT LITTLE FINGER, BRUISING IT. NO FRACTURE BUT REQUIRED SPLINT & RESTRICTED DUTY.
EE WAS DIRECTING WORK ON A CONVEYER BELT AND TOLD ANOTHER EE TO STRIKE THE TOP OF THE ARM OF A "SIDE KICK" WITH A HAMMER TO FREE IT UP. THE OTHER EE IMMEDIATELY STRUCK WITH THE HAMMER BUT HIT THE OTHER EE'S RIGHT RING FINGER, CAUSING A FRACTURE AT THE END OF THE FINGER.
TWO MEN WERE LIFTING A SCREEN BEARING USING A BAR THROUGH A BOLTHOLE. UPON LIFTING, THE BAR SLIPPED OUT OF THE HAND CAUSING THE BEARING TO FALL ON HIS FOOT. THE IMPACT CAUSED A CONTUSION OF TH E RIGHT FOOT. X-RAYS SHOWED NO FRACTURES.
THE EE WAS CUTTING A 36" WIDE 3 PLY BELT WITH A UTILITY KNIFE. HE WAS HOLDING THE KNIFE IN HIS RIGHT HAND AND CUTTING TOWARDS HIM SELF. THE KNIFE SLIPPED AND HE CUT HIS LEFT HAND.
WHILE PULLING ON A COMEALONG THE CHAIN BROKE CAUSING THE HANDLE TO RELEASE. HIS HAND JERKED BACK AND HE STRUCK HIS ELBOW ON THE FRAMEWORK BEHIND HIM.
WHILE SPLICING CONVEYOR BELT SLIPPED WITH UTILITY KNIFE AND JABBED WRIST.
EMPLOYEE WAS USING A LONG STEEL BAR TO ROCK A 8 BY 20 VIBRATING SCREEN. THE BAR WAS CAUGHT BETWEEN THE SCREEN AND FRAME AND HIT HIM ON THE RIGHT EAR.

DRAIN PLUG WAS TIGHT. EE WENT TO GET A WRENCH, USED A "CHEATER PIPE" INSTEAD. APPLIED PRESSURE AND SLIPPED. EE LACERATED HAND ON THE JAGGED EDGE OF THE PIPE. RECEIVED 6 SUTURES TO LEFT HAND (O N PALM.
EE WAS CUTTING CONVEYOR BELT, SLIPPED AND CUT HAND.
EMPLOYEE WAS CUTTING PASTIC TIE FROM AN EXTEN- SION CORD USING A POCKET KNIFE; HE CUT THROUGH TIE AND LACERATED LEFT PALM.
