AVAILABLE INFORMATION FROM THE SCRAPER OPERATOR AND EYE WITNESS SHOWS THAT THE EMPLOYEE MOVED BETWEEN A BACKING 633 CAT SCRAPER AND HIS PICKUP OUT OF THE OPERATORS FIELD OF VISION. HE WAS RUN OVER BY THE LEFT REAR SCRAPER TIRE RESULTING IN DEATH.

Employee was replacing a screen panel when it slipped out of left hand and landed on right pinky finger, causing a puncture to right pinky finger.

Employee was replacing a guard on a tail pulley and strained shoulder and lower back.

Employee was attempting to free a screen tightening rail with a four foot bar and the rail came loose. The bar then came back and struck the employee on the left side of the face causing safety glasses to cut employee just above the eyebrow.

Employee picked up a 140 pound log washer part and threw it causing a lower back strain.

Employee was in the process of building a guard, and was welding at the time, when another employee with a lit cutting torch pulled on the torch to get more slack in the hose. In the process he ran the lit torch across the arm of the employee that was welding. This caused a second degree burn on the employee's upper left forearm about two inches in diameter.

EE RECEIVED WELDER ARC BURN TO THE EYES WHILE HELPING AN EE WELD CHUTES. EE WAS USING P.P.E. BUT MAY HAVE SUSTAINED THE BURN WHILE WORKING IN THE AREA.

EE WAS STANDING ON A CONVEYOR APPROXIMATELY 5' OFF OF GROUND OBSERVING THE TRANSFER POINT OF THE CONVEYOR HE WAS STANDING ON AND THE DISCHARGE END OF THE CONVEYOR DUMPING ONTO IT. HE LOST HIS FOOTING & GRABBED DRIVE PULLEY OF THE OVERHEAD CONVEYOR CUTTING HIS RIGHT RING FINGER & TWISTING HIS WRIST. THE CUT TOOK 4 STITCHES.

OPERATOR OF SCRAPPER THAT RAN OVER A CO-WORKER AND KILLED HIM ON 2-8-00. OPERATOR HAD BEEN SUFFERING FROM STRESS RELATED PROBLEMS ASSOCIATED WITH THIS EVENT. ON 3-14 HE REPORTED A WORK RELATED ILNESS FOR STRESS.
