He was putting a steel insert behind the shaft of the head pulley at the C.R.S. Two employees working on opposite side of solid guarded pulley had hoisted up the pulley when the hoist shifted and broke. The head pulley shot back (due to the conveyor belt) and the plate hit his hands.

He was readjusting chain on box erector-When he released chain from gear the tension caused it to "jump" and it caught his thumb machine was locked out but could have used pry bar instead of hand to lift chain.
