Fall onto or against objects

Employee was putting a screw back in the baghouse door to close it completely when they fell on top of the ladder. At MCV they found bruised kidney, but no surgeries were performed.

Over-exertion in lifting objects

Employee was lifting and stacking 30 pound boxes onto pallet on to the floor.

Caught in, under or between a moving and a stationary object

During a pause on the line, he leaned forward & lifted his left foot up & set it on a slow moving foot high conveyor belt. His foot moved along an inch or so & slipped down between that belt & the next one in the line, which was not operational. Upon removal he went to the ER-nothing broken just swelling & missed one day on ice at home. (No damage to steel toe boot either.)

Struck against stationary object

He turned and stepped on a roll of packing tape that had vibrated off the top of the Ouachita machine that was under repair. When he stepped on the edge of the roll of tape he turned his ankle.

He slipped on wet ground as he was getting out of Bobcat and landed on back. Bobcat was parked on concrete slab and he dismounted properly but foot slid on slick of mud.

Fall from ladders

He was standing on a step ladder reaching to adjust the sawdust chain when the ladder tipped and he fell. The ladder was standing on concrete. He lost his balance.

He was putting a steel insert behind the shaft of the head pulley at the C.R.S. Two employees working on opposite side of solid guarded pulley had hoisted up the pulley when the hoist shifted and broke. The head pulley shot back (due to the conveyor belt) and the plate hit his hands.

Over-exertion in lifting objects

He had refilled the propane tank for his forklift and was placing it on the back of the lift when the fuel shifted slightly causing him to slightly lose his grip resulting in a sprain or strain. The full tanks weighted 65 lb & there is a slightly different placement on these new lifts.

Struck by flying object

He was replacing grease cartridge when the lock broke on the grease gun. It flew up and hit his face lacerating above rt eye and over bridge of nose. His nose injury was medically glued, and received 6 or 7 stitches at laceration near eye.

He was readjusting chain on box erector-When he released chain from gear the tension caused it to ""jump"" and it caught his thumb machine was locked out but could have used pry bar instead of hand to lift chain.

He was using a blowtorch to cut metal cable. A spark landed on pant leg and ignited. Burned lower leg (calf) of right leg.

Changing nozzle on sew line, machine was started up by lineworker and dye came out.

Medical treatment only. Employee had loosened the bolts on the unit to make changeover when she was tightening them back up, they were too tight. 3 broke and the unit fell to the side and hit left side of her head.

Employee was checking manually the level in the silo on an evening when it had rained. Although he wore gloves his hand slipped as he leaned on the side of the silo. He lost his balance, hit the platform guarding and fell down the ladder he was on.

Employee bent down to pick a piece of paper that had fallen on the floor and hurt his lower back muscles resulting in severe back pain. He could not stand or walk without help.

EMPLOYEE WAS STANDING ON A SMALL STOOL 12""HIGH WHEN SHE SLIPPED OFF AND FELL ON THE FLOOR. SHE SAID SHE WAS FINE & WENT BACK TO WORK. WE CHECKED ON HER 4-5 TIMES DURING THAT DAY, SHE DID NOT HURT UNTIL THE NEXT DAY. WE TOOK HER TO A DR. FOR AN EXAM & X-RAY.

EE WAS WORKING IN SHOP REPAIRING DRAG BUCKET, FOOT SLIPPED AND HE BANGED IT REALLY HARD INTO THE LARGE BUCKET CORNER. FOOT WAS NOT BROKEN - BUT WAS BRUISED AND SWOLLEN FOR SEVERAL DAYS. EE WAS WEARING STEEL TOE SHOES.

EE TRIPPED OVER A HOSE AND FELL ON HIS RIGHT FOOT. HE SAW THE HOSE ON THE WALKWAY BUT FORGOT TO MOVE IT OUT OF THE WAY.
