REPORTABLE ONLY - Employee was using a hammer, driving a pin out of a shield right above EE's head when the hammer slipped out of EE's hand hitting EE in the mouth chipping a tooth.
REPORTABLE ONLY DUE TO SUTURES Employee was bent over to set a track tie under the rail as another employee was using track hammer to clip the latches on another track tie. During the downswing, the track hammer struck the bent over employee on back side of hard hat causing small laceration to scalp, requiring sutures.
Employee was setting a 30 h.p. pump and shoveling material over boots when the bunker washed out. Employee then felt burning on both of his legs. He is being treated for chemical burns.
Locomotive derailed and employee was helping jack locomotive back on to the track. After jacking approximately 4", the load shifted and the jack kicked out striking employee in the face. Employee received stitches and is awaiting surgery to fix his fractured nose.
Employee was using a spinner wrench to tighten the cathead on the power center, as he was pulling up on the wrench it slipped causing the wrench to hit him in the mouth breaking teeth.
Reportable only - Employee was standing close to where they were burning a bad insert out of 120 miner head, when the hot insert was hit with a hammer to clean the slag off a piece of slag came over his glasses landing in the corner of his eye.
Co-worker cut bands on pallet on the outby end of a car while employee was standing on the inby end. When the bands got cut, the weight shifted and caused the banded rollers to fall off the car striking employee in left ankle causing a fracture. Started losing time 4/28/15.
REPORTABLE ONLY DUE TO SUTURES - Employee was attempting to loosen a clevis pin with pliers when the pliers slipped off, his left arm recoiled back striking a yield tube on the adjacent supply car cutting his arm requiring sutures.
REPORTABLE ONLY DUE TO SUTURES - Employee was holding a 3/8 chain that was connected to a water pump, when the other employee moved the equipment to get slack in the chain, the chain tightened up and a broken link caught his finger causing fracture and laceration to end of finger resulting in sutures to his finger.
Employee was helping lift a 50HP motor in the prep plant, using boom truck and chain falls, from off conveyor belt to drive base when the motor hung on something and suddenly came free employee raised his arm to shield himself from motor which forced his arm into an electrical cabinet causing fracture to arm.
Employee was roof bolting when some top broke up and came onto the canopy, he was using a rock bar to remove the rock when the bar slipped hitting him in the mouth chipping teeth and causing laceration to his lip requiring sutures.
REPORTABLE ONLY DUE TO SUTURES - Employee was helping change a shear shaft on a continous miner, when the man in front of him was using a 4lb shop hammer that slipped out of his hands striking the employee on the right side of his face and ear causing a laceration requiring sutures.
Employee was splicing belt using a manual splicer, when he went to clamp down on splicer he felt pain in left shoulder. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT AND WAS TAKEN OFF WORK ON 12/9/14 DUE TO SURGERY BEING PERFORMED ON 12/15/14 TO REPAIR SHOULDER.
REPORTABLE ONLY DUE TO SUTURES - Employee was holding a part while another employee was hitting it with a hammer to straighten it out when his right ring finger was hit by the hammer causing laceration requiring sutures.
REPORTABLE ONLY DUE TO SUTURES - Employee was roof bolting when the drill steel broke off in the pot, he was using a red cable hanger to retract the drill steel when the end of the red cable hanger punctured the back of his hand resulting in two stitches to his hand.
Employee was setting wedges under pipe rack. Employee hit below left knee while using hammer, causing bruising
Employee was jacking on wreck car on 8 section track when the jack mechanical slip causing employee to fall backwards injuring his right shoulder/elbow.
Employee was using channel locks to pull belt up off of mine floor when he noticed pain in shoulder, he started training belt and when he swung a hammer he felt a pop in his shoulder causing a torn bicep.
REPORTABLE ONLY - Employee was adjusting rails on a rail car with a jack bar when the rails shifted the employees left middle finger was caught between the jack bar and rail causing laceration requiring sutures and a fracture to tip of finger.
REPORTABLE ONLY - Employee was trying to correct alignment of belt by striking the roller with a hammer, when he missed the roller and his thumb hit hanger bracket causing fracture and sutures to end of thumb.
Employee stated that he was using a pipe wrench to get a stuck drill steel from roof. He did go for treatment and received stitches.
Employee was putting together a double T outlet for 3" air line, while tightening fittings with a pipe wrench he felt pain in right wrist resulting in a torn muscle.
Employee and coworker were loosening panel bolts on the shearer with a ratchet and socket. They were assisting one another so that the socket did not slip off the bolt head. When the bolt broke loose, the ratchet head pinched employee's right finger between ratchet head and shearer frame. Employee received 5 stitches on 7/22/13.
REPORTABLE ONLY DUE TO SUTURES - Employee was cutting a piece of corrugated pipe when the knife slipped cutting employee hand.
Employee was installing support arm for canopy and while using come-a-longs the chain came out of the clevis allowing the canopy to fall hitting employee's finger causing the tip of finger to be cut off.
Employee was helping install the plow on stage loader. The plow was held on by 2 pair of comealongs and 2 bolts. As the was being lowered the 2 nuts on the bolts popped off allowing the plow to twist and hit the employee on the foot. Employee was standing behind the plow.
Employee was tightening up a screw jack with a sledge hammer and it hit his left hand with the wooden handle against metal screw jack arm. He received treatment at DCH where he received four stitches.
While trimming a corner of belt skirt on feeder, knife slipped out of employee's hand cutting right lower arm. Employee was sent for treatment and received stitches.
Employee stated that he was using a pry bar when the bar slipped and struck employee in the left side of his bottom lip. This resulted in a laceration that required sutures.
Employee was breaking loose lug with a cheater pipe. The lug broke loose and the employee fell into the versa trac.
Employee was using rock bar to pry hollow shaft loose using both hands, when the shaft broke free the pry bar came up and his hand on the bar hit his tooth causing a crack in the tooth. REPORTABLE ONLY DUE TO CRACKED TOOTH.
After employee sharpened axe, he noticed axe head was loose. He secured the axe handle in vice grip and inserted two screws into the end of handle to tighten the hand to the axe head. When he loosed vise to remove the axe it slipped and he went to grab axe in his left hand causing a laceration that required sutures.
Employee was helping jack wrecked loco up. He was holding jack while another co-worker was jacking. When co-worker pushed down on jack handle hard, employee's finger was under it causing it to pinch and cut his right index finger. This required stitches. No lost time.
REPORTABLE ONLY DUE TO STITCHES - Employee was cutting belt to load onto a pallet when he made the cut he brought the knife back up striking left knee.
While working on the lotrac, wrench slipped and struck employee's left wrist against the lotrac. Fracture ulna.
Employee stated that he was replacing a rubber hose. When he cut through the hose, the knife cut his arm. It required 4 sutures to close the laceration.
Employee was shoveling in production shaft when he got debris in his eye and it caused a scratch to his eye.
Employee was using a cheating pipe to break a motor bolt loose. The ratchet slipped and struck employee in the face causing a laceration that required stitches.
Employee stated that he was removing a broken metal band when it came out of the tool and struck the employee in the hand.
Employee was using a pry bar when it slipped causing the employee to fall backwards and hit his head as his hard hat came off striking his head on a cross bar. Employee required stitches.
Employee was using a jack when the handle slipped and struck him in the face.
Employee was splicing a cable when his knife slipped. This resulted in sutures to the left little finger.
Employee torqueing stem bolts into reamer body. The torque multiplier handle slipped out of employee's left hand striking his right hand across the knuckles, lacerating tendons on ring finger and little finger of right hand.
Employee was using a wrench to loosen a bolt on metal guard when the wrench slipped and employee cut his finger.
