The miner was trying to pry a battery cover open with a hammer the lid slipped and struck small finger of left hand causing contusion and abrasion to the finger.
Tightening/loosening a bolt, wrench slipped allowing left hand to strike a beam located near the bolt.
Crew was pulling 670 feet of rail outby with 5/6 nylon rope and clevis. Clevis bent and pin broke striking employee's left arm and rib area.

Employee was helping to take a pallet of cribs off a supply car. Pallet was stuck on edge of car. Employee was using a rock bar to free them. Afterwards he had pain in his lower back and shoulder. Employee was diagnosed with a sprained right chest muscle and started missing time on 7/23/08.
Injured was cutting belt belt slipped (wet) belt knife punctured left wrist.
A lowboy mine car was being lifted by a jack. The jack pivoted; the car came down the rail causing it to go down. The miner was standing out 4 feet inby the car with left foot under rail breaking 3rd & 4th metatarsel bones.
EE was loading a TRS jack onto a jeep and while using a bar to help raise one end felt pain in his lower back. He continued to work until 1/28/08 when he began missing due to his injury.
Employee was removing a rock from on top of the miner with a pry bar and felt a pain in the back of his neck.
Employee was lowering a brookville bus with a jack and bar when his left hand slipped off the bar allowing the bar to come up and striking him in the lower lip.He received 7 sutures to the lower lip.
He was using a slate bar to pry over a pillow block to align a belt roller. As he was applying pressure on the pry bar, it slipped and he fell down, twisted and fractured his left ankle.

Employee, motorman had a car off track & was in the process of using air bags & lifting jack to put trucks back on track. He was using a bar to lift wheel over the rail, when the car shifted & fell & struck his left hip area.
Employee was using a chain come-along to raise the belt to change a roller at #18 wall, #1 north mother belt. While holding the chain with his left hand the come-along slipped and caught his left index finger between the chain and come along resulting in a fracture to the tip of left index finger.
10 H laying track, jack was used to raise rail so it could be lugged to the track tie. He missed hitting lug and struck the rail. The rail came off jack and landed on top of his right foot. *Lost work days began 1/4/07.
Employee was using a 2 lb. sledge hammer to dislodge a stageloader chain link wedged behind the tail sprocket F-14 longwall set-up. As he was strking the link the stageloader chain sprung forward and caught his left hand between the chain and tail sprocket causing a laceration between the left thumb and index finger resulting in 7 sutures to close. He was wearing gloves.
Employee was using a utility knife to cut a piece of 2" plastic pipe, the knife slipped & cut his right thigh.
Employee was using a ratchet and spud to tighten hex bolt for drum extension on the head of miner. The spud slipped off bolt and his right hand ring finger struck the drum extension causing fracture to right ring finger.
8 I track, preparing to set posts because of bad roof. He used a pick hammer to pull post out of jeep the pick hammer slipped out of post and hammer struck him below his lowey lip causing small cut. *suture to close out
Injured was trying to free up frozen lifting jack when he got his finger caught in metal.
5I track switch, middle lowboy car jumped track. While jacking car back on track he felt pain in his groin area. 5/22/06 diagnosis was a hernia.
Injured was rolling rail with slate bar when it rolled and hit the other rail and rolled back onto his right foot and bruised it.
EE was using a hammer and chisel to remove a staple from plug on a new leg jack for shield in F-13 longwall set-up face. The plug came out under pressure causing the chisel to strike him in the right hand, resulting in a laceration to his right hand requiring 7 sutures. EE was wearing gloves.
WHILE CUTTING IN THE #5 ENTRY IN 3 NORTH LEFT, THE UTILITY KNIFE SLIPPED OUT OF THE BELT AND LACERATED EMPLOYEE'S LEFT LEG JUST ABOVE THE KNEE. EMPLOYEE RECEIVED 6 SUTURES TO CLOSE THE WOUND.
EMPLOYEE WAS INSTALLING A COVER FOR THE HEADGATE DRIVE SPROCKET IN THE E-13 LONGWALL SET-UP SECTION. THE EE WAS ASSISTED BY ANOTHER EE THAT WAS USING A PRY BAR TO HELP INSTALL THE COVER IN TO THE CORRECT POSITION. THE PRY BAR SLIPPED AND THE COVER CAUGHT THE EMPLOYEE'S LEFT THUMB TIP BETWEEN THE COVER AND THE DRIVE ASSEMBLY. DIAGNOSED AS A FRACTURE & SUTURES WERE REQUIRED.
EMPLOYEE WAS USING A SLIDE HAMMER TO REMOVE A TRAM SHAFT ON THE F-12 LONGWALL SHEARER AND PINCHED HIS LEFT RING FINGER BETWEEN THE SLIDE AND SLIDE STOP CAUSING A FRACTURE AND LACERATION TO HIS FINGER, REQUIRING 10 SUTURES TO CLOSE.
Cutting belt to roll rubber, knife slipped and grazed upper right leg.
7H section using a sledge hammer to remove belt structure, he accidently hit the belt, causing the hammer to deflect and strike his right shin. causing abrasion which became infected.
Employee was jacking a car on track & felt pain in lower abdomen. Lost work days began 8-26-05.
EMPLOYEE WAS MOVING ROCKS IN A CHUTE THAT CLOGGED AT THE TOP OF #2 RAW COAL SILO. WHILE USING A LONG PIECE OF PIPE TO SHOVE THE ROCK HE FELT A PAIN IN HIS LEFT SHOULDER. WAS LATER DIAGNOSED AS A ROTATOR CUFF TEAR AND EMPLOYEE BEGAN TO MISS WORK ON 11-22-2005.
EMPLOYEE WAS HOLDING A DRIVE PIN WHILE ANOTHER EMPLOYEE WAS STRIKING IT WITH A HAMMER. INJURED GOT RIGHT FOREARM STRUCK BY THE HAMMER. *LTA BEGAN 8/4/05.
While using 2 come-alongs to pull battery off scoop, the eye on the battery broke & the eye & hook came back & struck nose. Lost work days began 8/9/05.
4I longwall, hanging mesh, using ratchet (rope) to pull up mesh. The rope twisted and when he let go of the handle. The handle spun around and struck him in glasses causing cut blow eye. Sutures applied to close.
4 I TRACK HAULAGE, PUTTING ROCK DUST TANK CAR ON TRACK. USING AIR BAGS TO RAISE CAR UP, THEY WERE PASSING A LIFTING JACK TO OTHER SIDE OF CAR. AIR BAG KICKED OUT, STRUCK JACK & JACK STRUCK EMPLOYEE ON THE LEFT CHEEK.
Injured pushed come-along up monorail and hooked it with a chain. Chain slipped off and come-along rolled back and caught his thumb causing a crushing type injury.
Employee was laying on his side, pulling on a 36" pipe wrench to remove a pipe nipple & he strained his rt. shoulder & upper arm.
Pushing against a chute on July 7, 2004. A coworker freed the chute with a pry bar, the full weight of the chute was being held by the employee. After this situation, Employee's ankle continued to bother him, but he continued to work until 9/22/2004 when he reagrevated the previously unreportable injury.
USING A MULTIFIER TO TIGHTEN BOLTS ON THE HEAD OF THE MINER, SOCKET SLIPPED AND HIS FINGER WAS CRUSHED BETWEEN A BAR AND THE HEAD OF MINER CAUSING A FRACTURE TO TIP OF LEFT RING FINGER AND LACERATION.
JACKING JEEP ON TRACK, JACK SLIPPED, BAR HIT HIM ON THE RIGHT KNEE.
Working at headgate removing stageloader flex pan with come-a-long. One section moved and caught the tip of his left little finger between one section and the other. EE was wearing gloves. the wound required 8 sutures to close.
MAKING SPLICE ON 3I BELT, AS HE WAS PULLING ON UTILITY KNIFE TO CUT BELT, IT SLIPPED OUT OF BELT AND CUT RIGHT KNEE. NEEDED SUTURES TO CLOSECUT.
INJURED WAS USING SLATE BAR TO FLIP RAIL TO RIB WHEN THE SLATE BAR CAUGHT BY THE RAIL AND KICKEDBACK CAUSING A LACERATION TO THE RIGHT SIDE OF NECK AND THROAT.
INJURED TOOK OFF HIS SAFETY GLASSES TO CLEAN WHEN ANOTHER EE STRUCK A BLOCK WITH A HAMMER AND A PIECE OF BLOCK STRUCK INJURED IN LEFT EYE CAUSING A BRUISE.
HEADGATE CONVEYER BELT BROKE AND THEY HAD PLACED BELT CLAMPS TO HOLD BELT AND MAKE REPAIRS. AFTER THE SPLICE WAS MADE A HAMMER WAS USED TO KNOCK CLAMPS OFF BELT, A CLAMP FLEW BACK AND STRUCK H IS FORHEAD CAUSING A LACERATION.
EE WAS CUTTING A BLOCK WITH AN AXE THE BLOCK MOVED & HE STRUCK HIS LEFT THUMB ON BLOCK. LOST TIME BEGAN 10-14-03.
