Employee was removing a blow bar on the crusher using a hammer and swung the hammer to strike the wedge to dislodge it, but missed the striking target and got finger between the hammer handle and the door of the crusher. Causing right index finger to bleed a little and swell a little. Was given general first aid at onset, but later went for treatment.

Was cutting rubber hose with utility knife, knife slipped and employee cut his hand.
