Fall to the walkway or working surface

Employee was up on 2nd floor of tower one stone plant. Welding on M606 Hazemag when walking tripped over welding leads falling on EE's face into the steel grating. Striking EE's head and shoulder

Operator closed cab door of loader and right index finger was stuck between the door and the door frame, causing a partial amputation of the index finger.

Struck by falling object

Removed access panel and placed it on the tail pulley guard on the back of conveyor. As they were gaining access to chute the access panel slipped from its position and punctured a finger requiring stitches.

Employee was removing a blow bar on the crusher using a hammer and swung the hammer to strike the wedge to dislodge it, but missed the striking target and got finger between the hammer handle and the door of the crusher. Causing right index finger to bleed a little and swell a little. Was given general first aid at onset, but later went for treatment.

Subject located on mine property deceased from apparent suicide. Authorities notified of abandoned vehicle day prior and came back to search area for missing/wanted man. Found subject at 14:41 and req coroner at 14:44. No mine accident or injury, just trespasser.

Employee climbed up the right side of the ladder and slipped on the ladder falling to the ground on knee. Twisted and sprained right knee.

ICY GROUND-TIRE RAN OVER ROCK AND SQUEEZED IT OUT. THE ROCK ACTED LIKE SQUEEZING SOAP AND FLEW TOWARD EE'S LEG. IT HIT HIM ON THE LOWER LEG.

ROCK TRUCK BACKED INTO SECOND ROCK TRUCK. HURT OPERATOR TRIED TO GET OUT OF THE WAY. BACK OF FIRST TRUCK WAS COMING AT HIM IN CAB. TRUCK HIT AND OPERATOR JAMBED KNEE IN DASH.
