EE was hitting a bearing with a sledge hammer and a shard of metal came off and entered right forearm.
Employee was unloading a shuttle car tire from a supply car, while lifting the tire with a slate bar, he slipped and smashed his left index finger between the slate bar and a rock, lacerating his finger. He was transported to the ER and excused from work.
Employee was installing a pin in the front spring of a tractor when the wrench/pry bar slipped and struck employees right wrist. Initial injury 01/02/13. On 01/07/13, employee picked up a charger and the discomfort in his wrist became worse, he continued to work until 06/18/13 employee underwent surgery and was excused from work.
Injured employee was using a pick hammer to raise up a cable when he accidentally grabbed the pick side of the hammer causing a puncture wound to his left hand between his thumb and index finger. He received 2 sutures and was released to return to work.
Injured employee was using a jack to lift a 9Man man-trip when the jack kicked out and struck the employee on the left foot. He was taken to the ER and diagnosed with a contusion of the left foot and ankle. He was excused from work by the ER physician.
Injured employee was using a ratchet to tighten a bolt when the ratchet slipped causing the employee to hit his hand against a flite. He was examined at the ER and diagnosed with a fracture to the tip of his left ring finger.
Injured employee was using a utility knife to trim rubber from a screen panel when he accidentally cut his left Thumb. He was taken to the ER and received 2 sutures. He was released to return to work.
Employee was pulling on chainfall that came loose pinning his right hand between the chain fall and sump. Employee was wearing metacarpal gloves resulting in only bruise and abrasion, but a prescription medication was given.
Employee was shoveling 4 head trash pump @ 3 tailpiece when he slipped and struck hisself on back of his head and ear with shovel. Received 7 sutures on his ear.
Trying to raise rail to put blocking and gauge rod in and come-along came loose striking employee in mouth and lower right jaw. Chipped front tooth.

Injured member was cutting a piece of belt when the belt knife slipped out of the belt and into the injured's leg. He received 8 sutures.
Injured employee was trying to remove a fitting by using a hammer and chisel when a small piece of metal came off of the chisel, it went through the employee's glove and into his right hand. The metal was removed and he received 2 sutures.
Injured employee was using a utility knife to cut a piece of belt when the knife slipped and cut his left index and middle finger. Injured received 7 sutures.
Injured employee was using a utility knife to cut a plastic lid when he accidentally cut his left thumb. He received 7 sutures.
Injured employee was using a belt knife to cut belt, when he accidentally cut his left forearm, causing a lacertion which required 6 sutures.
Injured employee was taking slack out of the drive belts on Coarse Clean Coal when the wrench slipped causing him to fall approx. 6 ft. His foot became lodged between a bolt and the belly pan. He has been diagnosed with a 2, 3 & 4 Metatarsal Fracture. He has been placed on light duty.
Injured employee was using a tape measure to measure a belt roller when the tape line retracted and lacerated his left little finger. He was transported to the ER and received 5 sutures. He was released to return to work.
Injured employee was setting a prop and while hammering a wedge he lacerated his right hand on a sharp rock. He was transported to the ER and received 3 sutures. He was released to return to work.
Injured employee was using a belt knife to cut the belt. The knife slipped, cutting through his glove and causing a laceration to the tip of his left thumb. He received 4 sutures.
Injured employee was using a utility knife to cut the belt from the #1 Slurry pump when the knife slipped and made contact with his left hand causing lacerations to his 4th & 5th fingers. He received 5 sutures in his 4th finger and 6 sutures in his 5th finger.
Injured was helping change tail roller bearing. He was using a slate bar to pry back on the roller when it slipped and struck him on his right ear causing a laceration. EMS transported him the the ER where he received 7 sutures in his right ear. He returned to work on his next regularly scheduled shift.
Injured was helping move the tool sled with a chain hooked to the scoop and the sled. When the chain tightened up, a bolt broke off of the chain and struck the injured above the right eye. The injury required 4 sutures.
Splicing Conveyor belt slipped on structure and cut leg just above knee.
Helping to make belt splice, he leaned forward to remove nail driver; he was hit on the right side of nose with a 3lb shop hammer, resulting in fracture of the nose.
Using a chain hoist to raise a 18 inch pipe the eye broke loose causing pipe to fall striking ladder he was standing on,causing him to fall to the floor striking his head and elbow requiring 7 stiches in his right elbow
Using hammer to remove a belt pin, while striking with hammer a piece of metal flew up and struck him on the right cheek requiring 5 stitches.
While trying to remove a bent keeper bolt to replace a broken shear pin, he struck his left ring finger with a 3 lb. hammer requiring 13 stitches.
Changing bottom roller on crossover belt. While holding roller up to use a slate bar, the bar slipped, striking him on the left side of his forehead. Caused a laceration, requiring 3 stiches. Returned to work next shift
leveling tailpice up using a mine jack, Jack slipped out stricking him on the nose resulting in laceration requiring stiches
Member was working on #1 slurry pump. Pipe wrench was on shaft of pump barrel, wrench spun around and hit member in right ear, and neck/shoulder area, causing a laceration to right ear.
Cutting hose off of paddle sucker. Utility knife slipped cutting left pinky finger.
He stated that as he was taking up a chain hoist. While removing the refuse screen deck, it moved, bumping his right hip and he felt a pain in lower back. X-rays taken and he was removed from work on 9-6-2007.
Member was using a slate bar to unplug a chute; when the material freed up, it struck the bar, hitting his forehead, causing a laceration requiring stitches.
Individual was setting rebar with hammer, when he missed, hitting rebar and hit his left index finger with blow from hammer he was using.
EE was attempting to change a blow plug on the fluid coupling at the G.H. 2 Belt Head. As he was turning an Allen wrench to loosen the plug, the wrench slipped causing him to strike his right wrist against the metal framing. EE ceased work immediately. X-rays were taken and were negative. Due to the excessive swelling he was removed from work.
He was cutting a zip tie loose using a utility knife. He slipped and cut left hand requiring 4 stitches. He completed his shift and did not report this to his supervisor until he returned to work the next day.
He was attempting to put a wheel back on the mantrip by pushing on the axle with a bar. When doing so the wheel turned causing him to twist his left knee. On 10-7-06 he had a MRI. On 10-20-06 he learn that he would need surgery to repair 2 tears in his left knee.
Ee was using a chain hoist to repair the miner cats. As he was tightning up the hoist to pull the cats back into place, it slipped off causing the hook to come back striking him above the left eye causing a laceration requiring 3 stitches. No days lost from this accident.
EE was using a utility knife to cut off a piece of loose belt off the one belt line. His knife slipped cutting left forearm which required 5 stitches.
While removing a water line coupling, EE slipped causing him to hit his self smashing left middle finger between sledge hammer he was using and a relief valve, resulting in a fractured finger.
Employee was using a sledge hammer to bend a piece of metal on the tail gate of his coal truck. He stated that something popped in his thumb causing pain.
Employee was repairing a gear box in the miner. While using wrench with cheaterbar, bolt freed and he felt something pop in his shoulder. Hix first day away from work was 3/20/06
MEMBER WAS CHANGING THE DRIVE SHAFT IN THE SCOOP. HE WAS TIGHTENING UP THE BOLTS WHEN THE RATCHET SLIPPED OFF OF THE BOLT STRIKING HIM IN THE FACE JUST ABOVE THE LEFT EYE. SUTURES WERE REQUIRED.
INJURED WAS HOLDING A SLEDGE HAMMER FOR THE MECHANIC WHEN THE MECHANIC SWUNG HIS HAMMER & MISSED THE SLEDGE HAMMER. THE MECHANIC'S HAMMER MISSED THE SLEDGE HAMMER STRIKING INJURED'S RT HAND CAUSING A FRACTURE.
Member was using slate bar to pry rock. The bar slipped landing on his right hand causing a laceration. Sutures were required.
Injured was tightening bolt on brake head of #00-5 man Brookeville mantrip when he slipped on mine floor causing rt wrist to become fouled between brake head & frame of mantrip. He was brought outside by EMTs & transported to Boone Memorial. He was diagnosed w/contusion to rt wrist & released. He went to see another dr. later that day & was advised to take physical therapy.
EMPLOYEE WAS INSTALLING CB MOUNTING BRACKET IN ROCK TRUCK WITH A MULTITOOL WHEN THE BLADE INADVERTENTLY CAME OPEN CUTTING RIGHT MIDDLE FINGER.
Ee was making a cable splice on the continuous miner with a utility knife. He cut his left thumb and received stitches.
Employee was cutting a hole in the dust hose when he dropped his knife. He cut his left leg at the knee area. He received 4 stitches.
