EE was tightening at the slack adjustment bolt on the Fletcher RRII roof bolter and the wrench slipped and caused a laceration on his middle finger. He received 3 stitches to close the wound.
Employee was making a belt splice. After end of splice was complete, the two ends would not align, so employee was going to cut off a clip on belt when the knife slipped causing a laceration to his left palm.
EE was using utility knife to open a box. He let knife slip, contacted his left hand, causing injury. He was taken to ER for treatment, stitches were used to close the wound.

Member was changing filter on 4m when using a cheater bar on a wrench applied force breaking wrench causing him to mash his hands w/cheater bar & striking lft middle finger on c/m frame. Cut end of finger about 1/4" from tip of finger (received sutures.)
EMPLOYEE WAS IN THE PROCESS OF CUTTING A PIECE OF CONDUIT WHEN HIS KNIFE SLIPPED CUTTING TWO FINGERS ON HIS LEFT HAND. HE WAS MEDICALLY TREATED WITH 6 STITCHES AND MEDICAL GLUE.
EE FOLDED A #35C CABLE TO CUT IT IN TWO. HE PROCEEDED TO CUT THE CABLE. THE KNIFE SLIPPED AND CUT HIS RIGHT LEG IN THE THIGH AREA.

EE WAS LOOSENING BOLTS ON THE MOTOR WHEN HIS RATCHET SLIPPED CAUSING HIM TO FEEL PAIN IN HIS LOWER BACK. HE DID NOT CEASE WORKING UNITL1/2/01 AT WHICH TIME HE WAS INSTRUCTED BY A DR TO REMAI N OFF WORK. NOTE: THIS REPORT IS LATE BECAUSE OF AN OVERSIGHT ON PAPERWORK
EE A CO WORKER WERE MAKING A BELT SPLICE. WHILE BRADDING THE SPLICE THE CO WORKER STRUCK HIS HAND WITH A HAMMER.
EMPLOYEE WAS MOVING A FLAT CAR BACK ON TO THE TRACK. HE HAD HIS LEFT HAND ON TOP OF THE JACK. THE BOTTOM OF THE JACK KICKED OUT AND CUT HIS LEFT INDEX FINGER.
