Employee was using 3/4 drive ratchet with 18" of extension to loosen the bolts of a shuttle car trunnion. As employee used his weight for leverage the bolt loosened and he fell forward and struck a fender resulting in a broken tooth.

INJ EE WAS HOLDING A BALL SCREW HAMMER WHILE ANOTHER EE STRUCK HAMMER WITH SLEDGE HAMER. THE SLEDGE HAMMER GLANCED OFF BALLSCREW HAMMER STRIKINGEE IN MOUTH. THE INJ EE BROKE 2 TEETH & REQUIRED STITCHES
EE WAS INSTALLING 48" RESIN BOLT. TOP OF WRENCH HOOKE, CAUGHT MIDDLE FINGER ON RIGHT HAND BETWEEN WRENCH AND FRAME OF DRILL MAST, REQUIRING STITCHES TO CLOSE CUT.
EE WAS STRIKING A HUNG CABLE ON WENCH WITH SLATE BAR & CAUGHT FINGER BETWEEN SLATE BAR & FRANE OF SCOOP. EE CUT LITTLE FINGER ON LEFT HAND. THE LACERATION REQUIRED STITCHES.
