Employee was changing offside rear tire on scoop. He had pulled the tire loose on top and stuck the bar at the bottom of the tire and pulled up to loosen the bottom of the tire when he felt a pain in his lower back.
Cutting a lifting waterline (8") and strained his back.
Employee was using 3/4 drive ratchet with 18" of extension to loosen the bolts of a shuttle car trunnion. As employee used his weight for leverage the bolt loosened and he fell forward and struck a fender resulting in a broken tooth.
EE was using a razor knife to cut a plastic bit bucket. When the knife cut through the end of the bucket, the knife slipped and cut his left arm, resulting in three stitches.
The employee was using a knife to cut conveyor belt. While cutting the belt the knife slipped out of the belt striking the employee on the left thumb which resulted in a cut to the left thumb.
EE was at the section power center installing a new cat head or plug on the continuous mining machine cable, when he cut his left hand, requiring 6 stitches.
EE was jacking up 33 tail piece, when jack handle slipped, striking him on his right cheek on his face.
Employee was putting a pin in low-trac bucket. While swinging the hammer, he missed the pin hitting his left index finger.

EE was hammering a pin when a piece of metal sheared off striking him on the little finger.
EE was cutting a piece of belt when the knife slipped cutting him in the right thigh about 4" above the knee. The laceraction required 5 stitches.
Employee was putting a 100 ton pizza headjack on a scoop when the jack slipped and caught middle finger on left hand between scoop frame and jack.
Employee was using a slate bar to free up a cable reel on the shuttle car when the slate bar slipped causing employee to strain his lower back.
EMPLOYEE WAS OPERATING A BUCKET OF BELT NAILS WITH A UTILITY KNIFE WHEN THE KNIFE SLIPPED CUTTING HIS RIGHT THIGH.
THE ROOF BOLTER OPERATOR ATTEMPTED TO STRAIGHTEN A SUPPORT STRAP WITH A BOLT WRENCH. THE WRENCH SLIPPED AND STRUCK THE OTHER ROOF BOLTER OPERATOR BEHIND THE LEFT EYE. EE COMPLAINED OF SOME DIZ ZINESS AND PAIN BUT CONTINUED TO WORK UNTIL THE SHIFT ENDED AT 3:00 PM.
EE WAS RUNNING A ROOF BOLT MACHINE PIECE OF ROCKFELL DOWN DRILL STEEL HE WAS TRYING TO BREAK THEROCK UP & STRUCK HIS RIGHT INDEX FINGER ON THE DRILL HEAD FRACTURING HIS RIGHT FINGER
INJ EE WAS HOLDING A BALL SCREW HAMMER WHILE ANOTHER EE STRUCK HAMMER WITH SLEDGE HAMER. THE SLEDGE HAMMER GLANCED OFF BALLSCREW HAMMER STRIKINGEE IN MOUTH. THE INJ EE BROKE 2 TEETH & REQUIRED STITCHES
EE WAS INSTALLING 48" RESIN BOLT. TOP OF WRENCH HOOKE, CAUGHT MIDDLE FINGER ON RIGHT HAND BETWEEN WRENCH AND FRAME OF DRILL MAST, REQUIRING STITCHES TO CLOSE CUT.
EE WAS STRIKING A HUNG CABLE ON WENCH WITH SLATE BAR & CAUGHT FINGER BETWEEN SLATE BAR & FRANE OF SCOOP. EE CUT LITTLE FINGER ON LEFT HAND. THE LACERATION REQUIRED STITCHES.
EMP WAS CUTTING PIECE OF FLEXIBLE WATER LINE. KNIFE SLIPPED, CUTTING EMPS RIGHT THIGH REQUIRING STITCHES.
EMPLOYEE STRUCK MIDDLE FINGER ON LEFT HAND WITH HAMMER WHILE MAKING A BELT SPLICE.
