A non-injury, non-fatal roof fall occurred at this operation on 8/6/13 in the 1 North Main-Left side intake. The fall was discovered by MSHA inspector and company mine examiner during weekly exams. The fall measures approximately 8' thick 100' long and 18' to 20' wide.
On June 27, 2013, employee file a hearing loss claim with the Kentucky Labor Cabinet Department of Workers' Claims. According to Dr. report, employee has 8% hearing loss. ***THERE ARE NO ACCIDENTS ASSOCIATED WITH THE CLAIM.

The men were using the Mac 8 and Mac 3 at the end of the shift to come out of the mine. The Mac 8 driver's foot slipped off the pedal and caused the machine to slow. The Mac 3 bumped into the back end of the Mac 8. The machines were traveling approximately 5-8 mph. This caused a jolt and resulted in a strain to employee's neck and back.
On June 14,2013 EE filed for Federal Black Lung with the Office of Worker's Compensation. There is no accident associated with this claim.
A roof fall occurred two crosscuts inby spad #7847 on the 6 East Panel. The fall measured 60' long x 20' wide x 5' thick. There were no injuries connected to this fall.
EE reached into scoop to retrieve a knife. He hit his right shoulder on the door frame. He continued to work his shift but his pain got continuously worse.
Employee was trimming a splice and caught his knife on the buckle. He twisted the edge of the belt. The knife glanced off of buckle striking employee on his left thumb. This resulted in a laceration which required two (2) stitches to close the wound.

Employee was replacing the head jack on a continuous miner. While lining up the new jack, the chain hoist slipped, pinning the employee's hand between the jack and the frame of the machine. This resulted in a jagged cut to the employee's right second finger which required two sutures to close.
Employee was bending a 6' roof bolt over the T-bar when he felt his back tighten up. He continued working the rest of the shift and the next days shift. He went to the doctor on 1/27/2012 and they put him off from work until he could see his regular doctor.
Employee was preparing to install his next row of bolts. As he stepped down to the drill station, he stepped in a hole, resulting in multiple fractures to his left foot.
Employee was pulling a Head Drive over with a Chain Ratchet. The chain broke and the chain hit him in the knee. Employee did not go to the doctor until the following day.

EE a truck driver was going to check the oil on a mach truck that he was driving. He said his foot slipped off the bumper step and he fell on his right side. he said he had pain in his right arm & leg. He was taken to Harlan County emergency & x-rayed - no broken bones found.
Employee was loading belt structure onto a Lo-Trac by himself when he felt a sharp pain in his lower back. the employee was 13 breaks from the section when this occurred.
While operating a remote control continuous miner a piece of draw rock 18""x 12""x 1/2"" thick fell from between bolts and struck him on the back of left hand that was resting on the remote box resulting in three stitches.
EE was loading supplies on section scoop when he felt a pain in his lower back on the right side that went all the way down his right leg. EE did not go to the doctor until 9/24/11. He went to see his doctor on 9/28/11. He received a full release on 9/28/11.
Employee was installing a metal roof strap. Turned to get bolt out of tray. When turning back around the strap struck employee in the forehead. Employee received a laceration which required 3 stitches.
EE was plastering a newly erected Brattice when he noticed small pieces of rock dribbling from the top. As he looked up, there was a small piece of draw rock fell striking him in the middle of his forehead resulting in 3 stitches.
The EE was attempting to remove a rock in the sampling unit. The electrical circuit to the belt had been de-energized, but not to the coal sampling machine. The EE got between the belt and the sampling arm. The sampling unit activated by a timer came on pinning the EE with the metal arm. EE was flown by helicopter to Holston Valley Hospital. MSHA hot line notified on 8/29/11.
On 8/4/11, a roof fall found by mine examiner during weekly exams. Fall was inby spad #7932 off 6 east mains. The fall was in an intersection. Fall was approx 24'w by 40'l by 10'h. Fall was in a intake split of air. Fall does not impede ventilation. No injuries no equipment was involved. Area was timbered and cribbed off. This fall will no be cleaned up
Employee was installing a roof bolt and strap to support a ""V"" beam that had been set. As he was holding the strap in place and started drilling the hole, the steel kicked back pinching his middle finger against the ""V"" beam. This caused a cut to the middle finger on his right hand that required 2 stitches.
Employee was in process of removing a damaged roller on the beltline when a section of the rib rolled out striking him on his left side knocking him to the ground.
Employee was walking towards the back of a walk through roof bolter when he lost his footing. As he fell, he tried to catch himself but fell anyway. He scraped his right arm and struck his back. He was transported by ambulance to Harlan A.R.H. Hospital. They did admit him overnight for additional testing.

Employee was building super cribs with 7' timbers on 004 section in 5 rt break. He was building super cribs with 7' timbers and crib blocks on 004 section in 5 rt crosscut. He was helping to put a timber on for the top when he stated that he felt something pop in his upper back just below the neck.
Employee was loading a trickle duster when he tripped over a piece of belt. He tried to catch himself and received a cut which required 4 stitches.
The contractor employee had cut the metal bands from a bundle of crib blocks. The contract employee started to pick up a crib block, when he tripped over an cable bolt causing him to fall. When he fell he hit his left ear on the loose metal band causing a laceration to his left ear.
Employee was making a cable splice, while using a 4lb hammer to hit the knocker. The side of the anvil broke. A piece of metal splintered and cut the skin and the metal embedded just behind the left thumb. The doctor removed the piece of metal and put one stitch in to close the wound.
EE was holding up a highline cable while another employee cut rope hangers down. When the other employee cut one of the rope hangers down, the highline cable came down causing his knife to hit employee's right elbow. (4 stitches)
This accident occurred on the surface. The injured employee was on a mantrip and had his left hand on the back of the seat, when another employee moved a metal mandoor loaded onto the mantrip, which resulted in lacerations to the injured employee's index and middle fingers on his left hand.
The employee was walking in front of the belt feeder when he stepped into a hole and sprained his ankle.
Employee was roof bolting in the #6 right break on MMU. No. 003-0 section. The employee was attempting to bend a six (6') foot roof bolt and his right shoulder popped.
The area was a wet laminated top and was in an intersection approx. 50' L. X 30' W. and 3-5' thick. It fell in the entry between #3 and #4 belts one break outby spad #1715. This fall does not block any travel or escape ways. We will probably clean it up or go over the top of it, it does not effect ventilation.
Employee stepped off of platform on the roof bolter and hit his left arm on a roof cap causing an laceration (6 stitches).
Employee was unhooking duster from mini-trac when he felt a pain in his shoulder. He told several co-workers about this. This was on 7/28/10. Employee continued to work the rest of the week. On Monday, his back and shoulder were hurting so he went to the doctor on 8/2/10. He was given muscle relaxers and 2 days light duty.
EE was setting miner up in right run of the cut, when he stepped on the water line going to the miner with his right foot. His foot twisted off of the water line causing him to fall. He twisted his right knee while trying to get back up.
Employee was putting lid back on top of the ATRS on the roof bolter when it slipped catching his ring finger on his left hand.

Had a roof fall in an old, abounded area of 1 left off 7 east main, area was already timbered and dangered off approx 40' inby spad #7320 in the #5 entry it has a travel way around it and did not effort the air that was circulating this area is to be sealed off as soon as seal plan came back from MSHA. The fall was approx 180' long and 8' to 10' high.
EE went to foreman and told him that his right shoulder was hurting. EE is a roof bolter operator. EE stated that his shoulder started hurting while putting up a roofbolt. The foreman asked the EE if there was an accident, the EE responded no. EE stated again that his shoulder started hurting.
Employee was in the surface warehouse & picked up 2 buckets of roof drill bits. When the employee turned around, he felt a pain in his back but didn't think any thing was wrong. The employee went underground and was helping hang the miner cable. The employee got sick at his stomach and his back started to hurt very bad. Employee brought outside.
EE was welding bolt heads on planetary hub on miner, when EE got flash and burnt his eye's. Doctor at Whitesburg ARH removed a piece of metal from the EE's eye.

Employee was working on a continuous miner in the #2 entry on 003 sect. The employee walked to the opposite side of the machine and ran into a roof bolt plate hanging below the roof. When employee ran into the roof plate it stubbed up his neck.
EE was working on overcast, EE was stacking blocks when he picked up a 8"" block he turned to the left to place the block on top of a 48"" overcast when he said he hurt his back. Accident occured 3 brks outby #16 headdrive. No EQ involved. EE did not slip or fall
Employee was removing safety chain from tongue of trailer. Tongue fell mashing his left ring finger requiring 6 stitches.
EE was plastering on overcast wall outby section of Rt. Mains. EE had to adjust safety glasses and slid them down on the nose to un-fog. EE was putting plaster on underside when the B-bond plaster splattered into the employee's eyes. The employee was brought to the surface, eyes flushed out. Taken to the hospital, eyes examined, Lt. eye scratched, dialated eyes & medication.
EE was raising drill head, when fast feed lever stuck causing drill steel to bend, striking his left hand.
Walking from #2 to #1 entry, when his foot slipped causing him to twist his ankle.
The EE was lifting a piece of belt that covered the suctin hose on the roof bolter. The belt covering the suction hose had some rope bolts laying on belt. When the EE started to lift the belt, he felt a pain in his lower back.
Employee was cutting on a piece of cable when his belt knife slipped cutting his upper right leg. Required gluing cut back together. Employee was on 003 section when injured.
EE was on 003 section roof bolting in the #4 entry. EE moved drill head next to the rib to put up a roof bolt. When drilling, a piece of the coal rib fell off, injuring the EE's right elbow and shoulder. Deep cut on elbow.

Fall was found during exam. It is on the old abandoned 1-LT panel on 003 section. The fall is in the # 6 entry approx 15 - 20' inby survey station 7297 & extending approx 175 ft to survey station 7306. Fall is approximately 3 ft on both ends & 10'-12' thick and approx 20' wide. Area had ben supported with 4 ft bolts & resupported w/10' & 12' cable bolts. Coal Seam @ approx. 8'.
EE was exiting scoop deck. As he was turning, his foot slipped, cauing him to twist his left knee.

This fall is from 2' to 7' high & about 18' wide, 35' long. The fall may have been caused by an undetected faulty area along with a block of coal left in the next coal seam. It will not be cleaned up.

When we drove through the area it was laminated sandstone with pressure from block of coal left in the room above the pressure causing the laminated sandstone to break up causing the rook fall in an intersection. Fall was approx 4' to 8' high, 18' wide and about 40' long in #6 intake entry, inby spad #6516 1 break.
Employee was picking up roof straps and a sharp end of the strap cut his right wrist.

While hanging curtain, EE put a ladder up against the rib. A piece of coal fell from the rib, striking EE on the left hand, index finger requiring 5 stitches.
Employee was putting up last row of bolts in #3 entry on 003 section. When he swung the head in to put up the next bolt a piece of rock with a sharp side fell and struck him on the forearm.

Employee was using a hammer & chisel to remove right rear drive axle on roof bolter when dirt & rock fell into his right eye.

EE got 3 of his teeth knocked out while roof bolting. He had a long piece of drill steel laying in tray and was swinging drill head to inside. The drill steel caught on roof bolter and flew out, striking EE in the mouth.

EE ACCIDENTALLY RAN HIS HEAD INTO A ROOF BOLT WHILE MOVING THE MINER CABLE INJURING HIS NECK.
The injured was involved in a rescue and recovery operation. Attempts were being made to locate and recover a person who was missing after a roof fall. Another roof fall occured at the recovery site injuring the individual.

The fall injured 2 persons who were involved in a rescue and recovery operation. In addition the fall caught a scoop which was being used in process.
A roof fall occurred in the No.1 entry of the 1-left off 1 right mains section (MMU-003-0) and measured approx. 30 ft long x 20 ft wide x 11 ft thick. The fall fatally injured 2 persons and caught 2 mobile roof supports.
DRAW ROCK FELL FROM TOP CRUSHING EMPLOYEE'S RIGHT RING FINGER BETWEEN ROCK AND DRILL POT OF ROOF BOLTER.
The mantrip was stuck on a rock and ee was trying to free it up with a slate bar. The bar slipped and ee crushed his finger between the bar and mantrip.
EMPLOYEE WAS HELPING HANG A MINER CABLE AND STRAINED HIS BACK WHILE TRYING TO HOLD IT UP. HE DID NOT GO TO DOCTOR UNTIL 5/31/05

EMPLOYEE WAS GETTING OUT OF A SHUTTLE CAR WHEN HE STEPPED IN A HOLE TWISTING HIS RIGHT KNEE.
EMPLOYEE WAS JERKING ON MINER CABLE AND STRAINED HIS RIGHT SHOULDER.
EE WAS DRIVING SHUTTLE CAR WHEN HE DROVE THROUGH MUD WHICH SPLATTERED IN HIS LEFT EYE.
EE WAS DRIVING A SHUTTLE CAR DOWN HILL WHEN THE TRAM PEDAL STUCK THROWING EE INTO THE PANIC BAR.THE SHUTTLE CAR THEN HIT THE RIB THROWING EE INTO CANOPY.
EE SAT DOWN IN THE SEAT OF THE LOW TRAC AND SAT IN SOME CHEMICALS THAT A REPAIRMAN HAD BEEN USING TO CLEAN THE CONTROL PANEL OF LOW TRAC. THIS CAUSED AN ALLERGIC REACTION TO EE'S REAR END.
EMPLOYEE WAS TRYING TO LIFT A PIECE OF STRUCTURE WITH ANOTHER EMPLOYEE AND STRAINED HIS LOWER BACK.
EE WAS PUTTING UP A CABLE BOLT WHEN THE CABLE BOLT BENT CAUSING THE WRENCH TO LEAN OVER AND BREAK. WHEN THIS HAPPENED THE EE'S ARM WAS BENT BACK OVER HIS SHOULDER AND THEN OVER THE CANOPY CAUS ING A STRAIN TO HIS RIGHT SHOULDER.
EE WAS PUTTING UP A 48"" STRAP AND AS HE LIFTED IT OVER HIS HEAD, A PIECE OF METAL FELL OFF OF THE STRAP AND INTO HIS LEFT EYE. THE PIECE OF METAL WAS EXTREMELY SMALL. EE FLUSHED HIS EYE HIMSEL F FOR SEVERAL DAYS BUT IRRITATION KEPT GETTING WORSE. EE WENT TO E.R. ON 4/17/03 & HAD OBJECT REMOVED.
EMPLOYEE TWISTED HIS RIGHT KNEE WHILE EXITING HIS SHUTTLE CAR.
EE WAS OPERATING A MINER WHEN HE WAS STRUCK ON RIGHT SHOULDER BY A RIB ROLL.
EE WAS TAKING A 4""WATERLINE APART. THE WATERLINE HAD PRESSURE AGAINST IT. AFTER TAKING THE BOLTS OUT, THE LINE STILL WOULD NOT COME APART SO EE APPROACHED THE LINE TO SEE WHAT WAS WRONG. AS HE TAPPED ON THE LINE IT SPRANG LOOSE & STRUCK HIM JUST ABOVE THE RT. EYE IN THE FOREHEAD AREA.
EE WAS WATCHING ANOTHER EE HAMMER ON A SPLICE MACHINE WHEN A PIECE OF THE HAMMER CHIPPED OFF AND STRUCK EE IN THE FACE. THE METAL SHARP WAS ABOUT 1/8"" IN SIZE. METAL SHARP WAS REMOVED A HARN E MERGENCY RM. NO STITCHES WERE REQUIRED.
EE WAS WALKING FROM THE MINER WHEN HIS LEG GAVE OUT CAUSING HIM TO SPRAIN HIS RIGHT ANKLE.
EE WAS ROOF BOLTING WHEN THE RIB ROLLED OUT AND STRUCK HIM IN THE HIP.
EE WAS CARRYING A PUMP WHEN HIS BACK GAVE OUT & HE FELL TO HIS KNEES. HE WAS IN ABOUT 1 1/2- 2"" OF WATER WITH A MUD BOTTOM. APPROX. WT. OF PUMP ABOUT 80 LBS.
EMPLOYEE SLIPPED WHILE STEPPING OFF OF ROOT BOLT MACHINE CAUSING HIM TO TURN HIS LEFT ANKLE.
EE WAS LOADING STRAPS INTO THE PINNER WHEN HE TRIPPED HITTING HIS INDEX FINGER ON THE STRAPS.
EMPLOYEE WAS DRILLING A HOLE WHEN HIS DRILL STEEL BEND AND SLAPPED HIM ON TOP OF HIS RIGHT HAND FRACTURING HIS RIGHT INDEX FINGER.
EE WAS BOLTING TOP AND STATED THE STAB JACK LEAKED OFF CAUSING THE CANOPY TO STRIKE HIM IN THE HEAD.
THE MINER EXAMINER DESCOVERED A BUILD UP OF WATER ON THE SECTION DURING THE PRE SHIFT EXAMINDED AND CAUSED WATER TO RUN OUT OF THE GOB AND ONTO THE SECTION. WE BELIEVED THIS TO BE BEING SUBMIT
EMPLOYEE WAS TRYING TO RESTOCK THE CABLE GOING TO THE SPLITTER BOX WHEN HE STRAINED HIS LOWER BACK. HE STATED THAT HE WAS IN AN AWKWARD POSITION WHILE TRYING TO PULL THE LEADS WHERE THEY NEEDE D TO GO.
EE WAS HELPING PULL THE MINER CABLE WHILE THE MINER OPERATOR BACKED THE MINER OUT OF A CUT THAT WAS READY TO FALL IN. THE EE WAS JERKING ON THE MINER CABLE TO HELP GET THE MINER OUT BEFORE THE ROOF FELL IN ON THE MINER. THIS JERKING ACTION CAUSED HIM TO STRAIN A MUSCLE IN HIS LOWER BACK.
EMPLOYEE STATED HE WAS PICKING UP A BOX OF GLUE AND TURNED WITH IT. WHEN HE DID HIS FOOT GOT HUNG BETWEEN THE CROSSOVER MATS AND THE FRAME OF THE FLETCHER DOUBLE HEAD PINNER CAUSING HIM TO TWI ST LEFT KNEE.
RIB ROLLED OUT AND STRUCK EMPLOYEE ON RIGHT ANKLE.
EE WAS CUTTING A 18"" ROPE HANGER WITH A BELT KNIFE WHEN HE CUT THROUGH THE ROPE AND INTO HIS LEFT HAND. HE RECEIVED 4 STITCHES TO HIS HAND.
EMPLOYEE WAS STACKING DOWN A BRATTICE WHEN HE REMOVED A BLOCK, ANOTHER ONE FELL ON HIS LEFT FOOT CAUSING HIM TO RECEIVE A BROKEN TOE. THE BLOCK FELL JUST BEHIND HIS STEEL TOED PLATE IN HIS BOO T. THE BLOCK WAS 6"" HIGH 6"" WIDE 16"" LONG. THE BLOCK WEIGHED APPROX 25 LBS AND FELL FROM APPROX 4' HIGH.
WHILE ON LUNCH, EE STEPPED ON A ROCK & TWISTED ANKLE.
EMPLOYEE WAS INSTALLING ROOF BOLT WHEN HE LOOKED AWAY FOR A SECOND, ROCK FELL STRIKING HIS RIGHT FOREARM.
EMPLOYEE WAS HANGING A CURTAIN WHEN THE RIB ROLLED OUT AND FELL ON HIS LEFT LEG. APPROX SIZE OF ROCK 3'LONG, 18""THICK.
EMPLOYEE WAS CRAWLING ACROSS A PILE OF MUCK AND WHEN HE CAME DOWN, THE OTHER SIDE OF HIS FOOT GOT HUNG AND CAUSED HIM TO TWIST OR SPRAIN HIS KNEE.
EE WAS WALKING UP THE #5 ENTRY WHEN HE STEPPED IN A HOLE AND TWISTED HIS RIGHT ANKLE.
EE WAS OPERATING A JOY 14CM15 MINER WHEN A LARGE ROCK )APPROX. SIZE 4 1/2' X 3' X 2-4"" THICK) FELL OUT FROM BETWEEN THE BOLTS NAD STRUCK THE EE IN THE HEAD. THE ROCK THEN LANDED ON BOTH WRISTS AND PINNED THE EE TO THE GROUND, CAUSING HIM TO BRUISE HIS BACK ON THE ROCK THAT HE LANDED ON.
EE WAS PREPARING TO LOAD A SHUTTLE CAR WHEN A PIECE OF RIB ROLLED OUT CAUSING THE ROCK ABOVE THERIB TO FALL. THE ROCK STRUCK EE ON THE BACK & SHOULDER AREA THEN FELL ON HIS RIGHT ANKLE THE SIZ E OF THE ROCK WAS APPROX 3-4'L, 18"" WIDE & 4-6"" THICK
EE WAS STANDING BESIDE THE RIB WHEN IT ROLLED OUT AND A PIECE APPROX. 1' IN DIAMETER LANDED ON HIS RIGHT FOOT.
WHILE BOLTING TOP A PIECE OF DRAW ROCK FELL FROM THE TOP STRIKING THE EE ON THE LEFT ELBOW. THE EE WENT TO THE DR. AND RECEIVED FOUR STITCHES.
EE WAS OPERATING A FLETCHER ROOF BOLTER WHEN A PIECE OF DRAW ROCK FELL OUT AND LANDED ON IS LEFT FOOT. EE'S LEFT FOOT NEEDED 4 STITCHES, APPROX. SIZE OF ROCK WAS 12"" IN DIAMETER, 3-4"" THICK, 4 -5 POUNDS.
THE EMPLOYEE WAS LIFTING A PIECE OF BELT STRUCTURE (APPROXIMATELY WEIGHT 60 LBS.) WHEN HE STRAINED SOMETHING IN HIS GROIN AREA.
THE EMPLOYEE WAS HELPING TO MAKE A BELT SET-UP AND WAS CARRYING A TOP ROLLER WHEN HE TRIPPED OVER SOEMTHING CAUSING HIM TO FALL, BRUISED HIS LOWER BACK AND LEGS.
DURING THE PRE-SHIFT OF THE 1 EAST SUB-MAINS. A ROOF FALL WAS DISCOVERED INT HE NO. 6 HEADING. THE FALL OCCURRED AT SURVEY STATION NO. 203 AND MEASURED APPROX 90'LX20'LWX4 TO 10'H. THERE WER E NO INJURIES OR EQUIPMENT INVOLVED AND VENTILATION WAS NOT AFFECTED.
A FALL OCCURRED IN THE NO. 5 HEADING OF THE 002 SECTION LOCATED IN THE EAST SUBMAINS. FALL WAS APPROX 120 FT LONG BY 20 FT WIDE BY 3-8 FT HIGH.CONTINUOUS MINER AND ROOF BOLTING MACHINE WERE C AUGHT. THERE WERE NO INJURIES AND VENTILATION WAS NOT AFFECTED.
