Pillar were being mined on the MMU 003-0. Mining had been completed in a block and the continous miner had been moved. The crew was in the process of moving the mobile roof supports from the area when a roof fall occurred in the intersection fatally injuring the vicitm.

A non-injury, non-fatal roof fall occurred at this operation on 8/6/13 in the 1 North Main-Left side intake. The fall was discovered by MSHA inspector and company mine examiner during weekly exams. The fall measures approximately 8' thick 100' long and 18' to 20' wide.

Struck against a moving object

The men were using the Mac 8 and Mac 3 at the end of the shift to come out of the mine. The Mac 8 driver's foot slipped off the pedal and caused the machine to slow. The Mac 3 bumped into the back end of the Mac 8. The machines were traveling approximately 5-8 mph. This caused a jolt and resulted in a strain to employee's neck and back.

On June 14,2013 EE filed for Federal Black Lung with the Office of Worker's Compensation. There is no accident associated with this claim.

A roof fall occurred two crosscuts inby spad #7847 on the 6 East Panel. The fall measured 60' long x 20' wide x 5' thick. There were no injuries connected to this fall.

EE reached into scoop to retrieve a knife. He hit his right shoulder on the door frame. He continued to work his shift but his pain got continuously worse.

Employee was trimming a splice and caught his knife on the buckle. He twisted the edge of the belt. The knife glanced off of buckle striking employee on his left thumb. This resulted in a laceration which required two (2) stitches to close the wound.

Employee was replacing the head jack on a continuous miner. While lining up the new jack, the chain hoist slipped, pinning the employee's hand between the jack and the frame of the machine. This resulted in a jagged cut to the employee's right second finger which required two sutures to close.

Employee was bending a 6' roof bolt over the T-bar when he felt his back tighten up. He continued working the rest of the shift and the next days shift. He went to the doctor on 1/27/2012 and they put him off from work until he could see his regular doctor.

Fall from machine

EE a truck driver was going to check the oil on a mach truck that he was driving. He said his foot slipped off the bumper step and he fell on his right side. he said he had pain in his right arm & leg. He was taken to Harlan County emergency & x-rayed - no broken bones found.

Employee was loading belt structure onto a Lo-Trac by himself when he felt a sharp pain in his lower back. the employee was 13 breaks from the section when this occurred.

While operating a remote control continuous miner a piece of draw rock 18""x 12""x 1/2"" thick fell from between bolts and struck him on the back of left hand that was resting on the remote box resulting in three stitches.

EE was loading supplies on section scoop when he felt a pain in his lower back on the right side that went all the way down his right leg. EE did not go to the doctor until 9/24/11. He went to see his doctor on 9/28/11. He received a full release on 9/28/11.

EE was plastering a newly erected Brattice when he noticed small pieces of rock dribbling from the top. As he looked up, there was a small piece of draw rock fell striking him in the middle of his forehead resulting in 3 stitches.

The EE was attempting to remove a rock in the sampling unit. The electrical circuit to the belt had been de-energized, but not to the coal sampling machine. The EE got between the belt and the sampling arm. The sampling unit activated by a timer came on pinning the EE with the metal arm. EE was flown by helicopter to Holston Valley Hospital. MSHA hot line notified on 8/29/11.

On 8/4/11, a roof fall found by mine examiner during weekly exams. Fall was inby spad #7932 off 6 east mains. The fall was in an intersection. Fall was approx 24'w by 40'l by 10'h. Fall was in a intake split of air. Fall does not impede ventilation. No injuries no equipment was involved. Area was timbered and cribbed off. This fall will no be cleaned up

Employee was installing a roof bolt and strap to support a ""V"" beam that had been set. As he was holding the strap in place and started drilling the hole, the steel kicked back pinching his middle finger against the ""V"" beam. This caused a cut to the middle finger on his right hand that required 2 stitches.

Employee was in process of removing a damaged roller on the beltline when a section of the rib rolled out striking him on his left side knocking him to the ground.

Employee was walking towards the back of a walk through roof bolter when he lost his footing. As he fell, he tried to catch himself but fell anyway. He scraped his right arm and struck his back. He was transported by ambulance to Harlan A.R.H. Hospital. They did admit him overnight for additional testing.

EE was loading belt structure when he felt pain in his back. EE worked his regular shifts until 3/1/11 when he went to the doctor . The conditions were wet and muddy.

Employee arrived at the section and got off the mantrip. As the employee started to walk toward the feeder his feet got tangled in an old metal band and a belt rail that was embedded in the mud. The employee fell onto his knees, jerking his neck and back.

During weekly examinations, an unintentional roof fall was found 3 breaks outby the face area of the old 003 section. Not an active section. The fall was 1 break inby spad #8927. The fall was approx 50' long x 18' wide. Fall was in #4 entry. Height was undeterminable. Area was timbered/dangered off. No injuries or equipment involved. Will not be cleaned up.

Employee was building super cribs with 7' timbers on 004 section in 5 rt break. He was building super cribs with 7' timbers and crib blocks on 004 section in 5 rt crosscut. He was helping to put a timber on for the top when he stated that he felt something pop in his upper back just below the neck.

Employee was loading a trickle duster when he tripped over a piece of belt. He tried to catch himself and received a cut which required 4 stitches.

The contractor employee had cut the metal bands from a bundle of crib blocks. The contract employee started to pick up a crib block, when he tripped over an cable bolt causing him to fall. When he fell he hit his left ear on the loose metal band causing a laceration to his left ear.

Employee was making a cable splice, while using a 4lb hammer to hit the knocker. The side of the anvil broke. A piece of metal splintered and cut the skin and the metal embedded just behind the left thumb. The doctor removed the piece of metal and put one stitch in to close the wound.

The area was a wet laminated top and was in an intersection approx. 50' L. X 30' W. and 3-5' thick. It fell in the entry between #3 and #4 belts one break outby spad #1715. This fall does not block any travel or escape ways. We will probably clean it up or go over the top of it, it does not effect ventilation.

Employee stepped off of platform on the roof bolter and hit his left arm on a roof cap causing an laceration (6 stitches).

Employee was unhooking duster from mini-trac when he felt a pain in his shoulder. He told several co-workers about this. This was on 7/28/10. Employee continued to work the rest of the week. On Monday, his back and shoulder were hurting so he went to the doctor on 8/2/10. He was given muscle relaxers and 2 days light duty.

EE was setting miner up in right run of the cut, when he stepped on the water line going to the miner with his right foot. His foot twisted off of the water line causing him to fall. He twisted his right knee while trying to get back up.

Employee was putting lid back on top of the ATRS on the roof bolter when it slipped catching his ring finger on his left hand.

Employee was roofbolting in the #3 RT crosscut. Employee was pushing up a roof bolt when the bolt wrench broke. The bolt wrench hit the employee on the left side of his face cutting a place about 1"" inch long in his upper lip. (required 4 stitches).

Had a roof fall in an old, abounded area of 1 left off 7 east main, area was already timbered and dangered off approx 40' inby spad #7320 in the #5 entry it has a travel way around it and did not effort the air that was circulating this area is to be sealed off as soon as seal plan came back from MSHA. The fall was approx 180' long and 8' to 10' high.

EE went to foreman and told him that his right shoulder was hurting. EE is a roof bolter operator. EE stated that his shoulder started hurting while putting up a roofbolt. The foreman asked the EE if there was an accident, the EE responded no. EE stated again that his shoulder started hurting.

Employee was in the surface warehouse & picked up 2 buckets of roof drill bits. When the employee turned around, he felt a pain in his back but didn't think any thing was wrong. The employee went underground and was helping hang the miner cable. The employee got sick at his stomach and his back started to hurt very bad. Employee brought outside.

EE was putting plaster on underside when the B-bond plaster splattered into the employee's eyes. The employee was brought to the surface, eyes flushed out. Fall was found during exam. It is on the old abandoned 1-LT panel on 003 section. The fall is in the # 6 entry approx 15 - 20' inby survey station 7297 & extending approx 175 ft to survey station 7306. Fall is approximately 3 ft on both ends & 10'-12' thick and approx 20' wide. Area had ben supported with 4 ft bolts & resupported w/10' & 12' cable bolts. Coal Seam @ approx. 8'.

Employee was roof bolting on 003 section. A piece of drill steel was hung together with the drill steel inside the drill hole. While the employee was trying to get the steel apart, the two pieces that were hung together came loose or apart and hit the employee on his right side.

EE hit top with canopy of shuttle car, dislodging a piece of rock, which fell into deck and stuck left arm and left leg. EE did not go to doctor until 12/29/08 to have knot on left elbow drained. EE did not miss any work.

Employee was shoveling belt conveyor. He came upon a large piece of coal that was to heavy to lift upon belt conveyor. He hit the large piece of coal to break into small pieces. When breaking large piece of coal and small piece broke off and hit him in the right eye.

We had a rock fall in the old abandoned 002 section in a crosscut between the no. 2 & 3 entries between spad no. 6072 & 6073. It does not block any airways or travelways. This fall is from 2' to 7' high & about 18' wide, 35' long. The fall may have been caused by an undetected faulty area along with a block of coal left in the next coal seam. It will not be cleaned up.

EE was weighing down a curtain with a rock. The rock slipped out of his hand and mashed his finger.

When we drove through the area it was laminated sandstone with pressure from block of coal left in the room above the pressure causing the laminated sandstone to break up causing the rook fall in an intersection. A roof fall occurred in the No.1 entry of the 1-left off 1 right mains section at survey station 4629 (40 feet outby). The fall was approx. 25 feet long by 12 feet wide by 4 feet thick. The fall injured 2 persons who were involved in a rescue and recovery operation. In addition the fall caught a scoop which was being used in process.
