Injured was assisting coworkers to remove hydraulic hose on portable screener. Zip ties holding the hose had to be removed to take hose off. EE was holding the hose with his left hand and used his pocket knife to cut zip tie off. While cutting, his hand slipped, lacerating his left hand between the thumb and pointer finger. Resulted in four sutures to the laceration.

EE heard a noise in the truck shop so he unlocked it & went inside to check it out & a man came full force at him. Employee pushed him away & then the man punched him in the nose. EE hit him 2 or 3 times in the face with his fist & the man landed on the floor. EE put his knee on his back & heard footsteps behind him then was hit in the head. He was unconscious for about 40 minutes

Principal injured was removing the service jack off the side of the #2 Shuttle Car. He took bolts out of the jack and was removing for maintenance. The jack fell pinching his hand against shuttle car and lacerating middle finger. Employee cleaned and bandaged the wound and completed his shift. He later visited a physician and received thirteen stitches to his middle finger.

Employee was working to remove the teeth from an excavator bucket. After cutting the teeth from the bucket EE used a sledge hammer to dislodge the teeth. While pounding the teeth off the bucket a piece of metal broke from the hammer striking his leg. EE was taken by ambulance to Western Maryland Health Systems where he received 5 stitches to repair the laceration.

Employee was performing maintenance on the belt press. He attempted to lift the end of a roll with a come-a-long when he felt a sharp pain in the right shoulder. The pain persisted the rest of the shift with some tingling in his right arm and hand. He went to the chiropractic doctor the next day.

Mechanic was hammering on a drill bit on a Drill Tech DK45 highwall drill with a sledge hammer when a metal fragment struck the mechanic and lodged in the left side of his jaw. Employee went to the hospital and the object was removed. Employee reported to work the next day.
