Showing all 24Am employee was releasing a chain binder on a flinger duster with a roof bolter wrench. When the binder came loose the wrench hit the employee in the forehead causing a laceration. The employee was taken to the Emergency Department and received 9 stitches, then was released to come back to work with no restrictions.
Employee pinched right middle finger between a slate bar and belt structure when EE was knocking ice away from the belt line. The employee received 3 stitches.

EE was pulling a roof bolt machine with a scoop on the section using a chain. The chain broke and one end came threw and opening on the scoop canopy and struck EE in the face.

EE WAS CUTTING THE OUTSIDE OF A PIECE OF MINER CABLE WITH A UTILITY KNIFE, THE KNIFE SLIPPED, AND CUT HIS RIGHT THUMB. THE INJURY DID NOT INVOLVE ANY ASPECT OF COMPLIANCE WITH RULES & REGULATION, JOB SKILLS OR ATTITUDE.
