A qualified surface electrician was working inside of an electrical cabinet to close a hole temporarily until the cabinet could be replaced. EE took out a utility knife to cut a piece of plastic. While EE was making the cut, the blade slipped, slicing through the tip of EE's left thumb. The employee was taken to the local ER, where sutures were added to close the laceration.

Showing 50 of 111IE was using pieces of concrete block to fill in the holes on the sides of the wall IE was building. As IE was using a rock hammer to fit a piece of block into place, IE struck left index finger between the block and the hammer, causing a laceration to the index finger. IE was taken to the medical facility for treatment where IE received sutures to close the wound.
IE stated was working on a main control valve for a 793-C truck. IE was prying up on the valve with a pry bar using left hand while trying to get a plug out with right hand. The pry bar slipped and the valve slid catching IE's finger between the valve and the work bench, causing an avulsion and a tuft fracture to the right index finger.
Ice had built up on a bottom roller of the clean coal collecting belt. IE (temporary employee)struck the bottom roller with a hammer to dislodge the ice buildup. IE's left hand was caught between the bottom roller and the belt. IE received a fractured left humerus.
A mechanic was working on a ram car tram motor. IE was having trouble positioning the motor so they used a drill steel to pry with. While prying, the drill steel slipped and they struck themselves in the mouth with it causing a laceration that required 6 sutures.
IE unsuccessfully tried pulling a pin from a belt splice. EE used a claw hammer to grab the pin then struck the claw hammer with another hammer. As hammer struck hammer a metal shard dislodged, became a projectile and struck the IE's left forearm. The shard was removed at the local ER, two sutures were required to close the opening caused by removing the shard.
IE was taking the .75" bolts out of the conveyor motor cover. When EE pulled upward with the ratchet, EE felt a pop and what felt like a shock in the shoulder and it ran down right arm. EE finished the job, and was still having pain at end of shift. On 1/11/17 the IE started missing work.
IE was using a chain fall to assist in raising the lid on the #2 dryer that was in the down position. The IE was pulling downward on the chain when the dryer lid become dislodged and moved suddenly. IE came down suddenly onto IE's right leg. The initial diagnosis was a right quadricep muscle strain. Eventual MRI showed a torn quadricep muscle which required surgery on 4/21/17.
Employee was making a splice on a feeder cable cutting the Insulation off of one of the phase leads, and the knife blade struck left index finger, causing a laceration that required 7 stitches.
A roof bolter walked out-by to an intersection to get supplies. A CM operator was changing bits nearby. The CM operator stuck the bit with a hammer causing a metal shard to project striking the front chest/shoulder area of the bolter - approx. 5ft. away. Dr. at local ER determined the shard would not be removed. On 9/9/2016 the shard was removed.
A contract employee was assisting to help repair the track on a continuous miner. When he was swinging a 12 lb. sledge hammer the sledge hammer glanced off the pin hammer, pinching his left index finger between the sledge handle and a steel rub rail on the side of the continuous miner. The injuries incurred were a laceration and fracture of the left index finger.
Injured employee was in the parking lot after his shift working on his personal truck. IE's personal knife slipped while cutting plastic off his truck bumper and struck him on the right wrist. He received a laceration which required medical attention.
Employee was cleaning gob out of a conveyor deck using a screwdriver. He felt pain and a pop in his right elbow. He received medical care and was placed on restricted duty.
The IE employee stated he was driving a connector link on a conveyor chain on the #18 continuous miner. He stated he was using a 15lb. sledge hammer to drive the connecting link to the strap. While striking the link a piece of metal struck the employee in the right arm approximately 3" above the wrist. IE had surgery to extract the metal from his arm.
The IE was changing bits on the continuous miner, and as he was attempting to remove the bit with a hammer a small piece of metal broke off the shoulder of the bit, and struck the employee in the right leg and embedded into the quadricep muscle.
Employee was making a splice on miner cable and said he heard a pop in his right wrist. He worked until thru 6/28/13 and was taken off work by a orthopedic surgeon waiting on surgery. Due to a compensation investigation this has just became a lost time injury.
Injured employee walked up behind a man using a pry bar. The employee with the pry bar turned the bar and when he did he spun it around and hit the injured employee in the mouth. Required 5 sutures to close the wound.
Employee was in the process of putting a cover on the side of a Continuous Miner. The cover slipped and caught his right thumb between the cover and a pry bar. He received a laceration to his right thumb requiring 9 sutures to close.
Employee was hitting with a hammer and a piece of the hammer came off and went into his arm. Surgeon had to removed the piece of the hammer. He received 6 sutures.
Employee was in the process of jacking up a belt tail using a track jack. He felt pain in his right shoulder. He continued to work until 01/20/2012 when he was seen by a physician. An MRI revealed a torn right rotator cuff and a partial tear to his right bicep tendon. Surgery has not yet been scheduled.

Employee was using a utility razor knife to cut off the edge of a conveyor belt. The knife slipped and he struck himself of top of the right leg with the blade of the knife. He received a laceration approximately 1.5 inches long that took 5 sutures to close.
The employee stated that he was splicing a miner cable. He had the cable lying on his leg and as he was cutting the cable open with a self-retracting knife, the knife slipped and he cut his leg just above his right knee. The IE was transported to the hospital, and received 6 sutures to close the wound.
Employee was changing a ripper tooth on a D-11 dozer. The ripper tooth pin was in a bind so he struck the ripper tooth with a sledge hammer. He said that when he struck the ripper tooth a small piece of metal struck the back of his right hand resulting in a laceration. The employee received 5 sutures.
EMPLOYEE HAD BEEN BEATING ON AN AREA THAT WAS WELDED ON A RAMCAR AND A PIECE OF SLAG CAME OFF AND WENT INTO HIS ARM. THIS HAPPENED ON 11/22/10. EMPLOYEE WORKED UNTIL IT WAS TAKEN OUT ON 1/21/11. SLAG WAS REMOVED AND HE RECEIVED 6 STITCHES, AND WAS TAKEN OFF WORK UNITL 1/31/11.
While installing a supplemental pressure pump for pumping of the NW Seals, the employee stated that he was using #430 and #460 channel locks to break apart a section of miner waterline. He stated that he felt something pop in his right shoulder. He continued to work until he had surgery on 12/17/2010 to repair a torn rotator cuff.
The employee was changing a tram chain on a roof bolter. The IE was using a chisel and a sledge hammer to take a link out of the chain. When he struck the chisel with the sledge hammer the chisel flipped up striking the employee in the upper lip, causing a laceration.
IE was struck on the left little finger by a hammer while making a belt splice.
The employee stated that he and another employee were making a splice on # 4 continuous miner cable. As IE was making the splice, a strand of the copper ground wire punctured the inside of his right thumb, at the first joint. Two days later the wound has become infected and he has had two surgeries to help remove the infection in his thumb.
The injured was unattaching a belt scraper on the 2 Main East, while using a chain hoist to lift the scraper it popped loose pinching his right hand between the scraper and the top roller causing an avulsion that required 3 stitches to close.
The employee stated he was cutting the rubber jacket loose on a continuous miner cable when he cut into the heel of his left hand with a utility knife, through the leather glove he was wearing.
Employee got cut by razor knife. It was glued at the emergency room. On 1/21/09 after working for 2 days, employee went back to Dr. for a check up. The laceration was opened back up and Dr. used 4 sutures to close wound back up.
Injured was removing packing from a conveyor motor lead with a pry-bar and struck his hand with a small sledge hammer in the process. His hand swelled and he continued to work. He sought medical attention on 1/13/09 and was removed from work on 1/20/09 because the swelling was not going down.
Changing hydraulic shovel lip and was driving new pins into tooth tips. He was holding pin with channel locks. When he swung the hammer to strike the pin, the hammer head struck the tooth to his right, causing the hammer to strike his left thumb and forced it against another tooth. He was taken to Quest Orthopaedic and diagnosed with a left closed mid-shaft thumb fracture.

A plant electrician was in the process of cutting a hole in a piece of belting being used as a splash guard on magnetic separators. His utility knife slipped out of the belting and struck him on the forearm causing a laceration in it requiring stitches to repair.
The injured was trimming a piece of belt to be used for a skirt rubber on the tail piece for Unit #2. He laid the belt across his right leg, and as he was cutting, the utility knife he was using slipped out of the belt causing a laceration / puncture to his right thigh. The injured required three sutures to close the wound.
EE was assisting in the installation of a new foot shaft on a CM.He was using a pry bar to hold the shaft in place while another EE was triyng to drive the shaft in using a hammer.A piece of metal broke off from the hammer or the shaft and struck the injured in the left forearm. Employee was struck in the left eye by a piece of metal either from a brass hammer or from a roof bolt plate. He was wearing safety glasses but the metal entered in behind his glasses. He continued to work up until 6/18/08 when he sought medical attention to have the metal removed.
Employee struck a pin with a sledge hammer and caught his index finger between the sledge hammer handle and the frame of the truck. The injury required sutures.
The injured was brushing float dust from the outside rail of the belt structure, the wire brush he was using came in contact with the outside barrel of a top belt roller, causing the wooden handle to strike his left index finger. The injured received three sutures to close the wound.
While the employee was taking the bucket off of the diesel scoop, the quick attach pin on the left side stuck. After removing the bucket they tried to move the pin back the opposite direction. They used a brass sledge and a B&O hammer to free up the pin. After hitting the B&O hammer, a splinter of steel broke off of the hammer and struck the employee in the back of his hand.
Injured was loading powder trucks under the storage bins. He used a 10 lb. dead blow sledge hammer to beat on the chute, to loosen the stuck material. He swung the hammer app. 75 to 100 times, when he felt a pop and burning sensation in the shoulder. He was examined and an M.R.I. revealed an internal roator cuff tear. He was ordered off with no use of left arm.
Injured employee was using a standard utility knife to cut a piece of mine belt. The injured employee was using both hands pulling the utility knife towards himself, when it slipped out of the cut and lacerated his right leg just above his knee. The laceration was approximately 3/4" long. He received 4 staples to close the wound.
EE was hammering a nail into a board next to the mine roof to install a line curtain in #10 entry. While installing the nail, a piece of a petrified stump fell from the mine roof striking him on the head, left shoulder and left leg. The incident caused abrasions.
EE, off side roof bolter, was prying down loose top in entry 6/7 crosscut 38. He felt his back pop and states that his left leg and lower back hurt.
Employee was digging a hitch in rib, soreness started developing & swelling.
Employee was trying to break a nut loose on the west side of the raw coal screen. He was using a boxed end wrench. The wrench slipped of and he fell. When he fell he dislocated his right knee in the process.
Employee cut plastic tie holding hose and punctured left wrist with pocket knife.
Injured was using a utility knife to cut off a piece of electrical tape from its roll. As the knife cut through the tape it slipped and stuck him on top of the upper right leg. 4 staples were installed to close the laceration.
