Was using a railroad jack to jack up a header. When EE was letting the header down the jack caught and the bar came up and hit EE in the chin. This resulted in a small laceration and a chipped bottom tooth.

Employee was attempting to get his steels unclogged, by striking his steels with a 1 foot wrench. When he struck the steels, the wrench bounced back and hit him in the mouth. This resulted in a chipped tooth and 4 stitches in his bottom lip.
Contract Employee was in the act of splicing a belt at 3 E Header. The employee was cutting toward himself when the knife slipped cutting him across his left upper leg. Contract Employee received five stitches at the Elk Creek nurses station and returned to work.
Knocking a piece of metal off of a sub in the yard with a pry bar. The pry bar slipped mashing his left pinky finger between the pry bar and the sub. This resulted in a laceration to the pinky that required 7 stitches to repair.
EE was helping mechanic make a splice in a car cable. When he was cutting the outer jacket off with his belt knife, the knife slipped cutting his left hand resulting in a laceration that required stitches.
EE was pulling pry bar off of his scoop and caught his hand between the end of pry bar and coal rib resulting in accident. He received 3 sutures in little finger on his right hand. He was back to work on 12/10/2012.
Employee was changing a buffer on a belt wiper. He was using a pry bar to get it out & when it came loose it hit him in the mouth, chipping three teeth.
Employee was cutting belt. He was pulling belt knife towards himself, when knife slipped and came across his right leg above his knee. The laceration took three sutures to close up. Employee came in early to get started on belt.

INJURED WAS USING A SET OF CABLE CUTTERS AND A BALL-PEINE HAMMER TO CUT THE CABLE TO THE RIGHT LENGTH. AS EMPLOYEE STRUCK THE CABLE CUTTER WITH THE HAMMER, THE BALL PART OF THE HAMMER CAME LOOSE AND STRUCK HIM IN THE RIGHT FOREHEAD CAUSING A SMALL LACERATION. THIS REQUIRED TWO SU
