Accident type, without injuries

Examiner discovered a unplanned roof fall in the 4AU Panel (6+30) XC#9 between #1 and #2 entries. Effecting the ventilation per CMI traveling with examiner to MPL #13. Approx 50' in length, 18'in width and > 8' in height.

A continuation of a fall that was discovered on 4-12-16 was discovered to have continued to the brattice line. 4 AU1 Panel (9+80) XC#14 Entry# 3. Length approx. 30' x Width 18.5' x > 7' in height. This was found with a MSHA CMI during a EO1 inspection.

Our Matrix tracking system went off line at 11pm on 4-5-16. We had our mines under manual zone tracking as our technicians searched for the issues. The issue had not been resolved at 10:48 am on 4-6-16. With speaking to our technicians and approaching our 12th Hour we notified MSHA. We remained under manual tracking until issue was resolved.

Unplanned roof fall occurred in the 1st North West Mains. Spad 123+73, XC # 151, between #6 and #7 entries. Length approx. 35' X width approx. 19' X height approx. 10'. This fall occurred on power-charger cables.

Unplanned roof fall occurred in the 4AR Panel. Between spads 7+40 and 8+10, XC's 10-11, entry #6. Length approx. 35' X width approx. 18' X height approx. 10'. This fall occurred on high voltage and pump power cables.

Unplanned roof fall occurred in the 2E Panel 43+95, #7 Entry XC-61. Length 30' X width 18' X height approx. 10'. Had pump power cables running through the intersection.

After drilling hole the steels were stuck together and in the chuck. The employee was trying to get them out. There was a roof bolt laying across EE's boom, as EE went to move the roof bolt and sump EE's pot forward at the same time, mashed EE's left pinky, ring and middle finger. This resulted in three fractured fingers.

Was using a railroad jack to jack up a header. When EE was letting the header down the jack caught and the bar came up and hit EE in the chin. This resulted in a small laceration and a chipped bottom tooth.

Struck against stationary object

Employee was loading his pin tray. As he pulled some roof bolts off of the bolter he hit his right elbow on a roof bolt that was installed to the top. Causing a laceration to his right elbow.

An unplanned roof fall was discovered at 9:30 am located at the 1st NW main xc 152 spad 124+43 #9 entry. The roof fall was on top of some power cables. Approx length 40' by 20' width by above 6' high.

Struck against stationary object

Employee was traveling #7 entry when the off side of his car dropped off in a hole throwing him into his canopy. Then the off side of the car hit the rib causing him to hit his left side on the control panel.

Was in the process of coupling trailers back together. He turned to walk back to his hauler and foot rolled over in a rut and he felt a pull in his left knee.

Was loading pin tray with pins, twisted to put pins in tray felt sharp pain in lower back.

Struck by falling object

Was bolting in the face of #10 entry, when a piece of rock fell out and struck EE's left forearm and hand. EE went to the doctor and was released, worked for a couple of days and still complained of pain. EE went to a specialist that placed EE off of work with a contusion on 9/16/2015.

Over-exertion in lifting objects

Picked up 4 pin boards and twisted to carry them to the front of the bolter and strained his back.

Struck by falling object

Roof bolter operator was installing bolt in roof, when he went to straighten bolt out, the bolt came out of the roof and struck him on his forehead. This resulted in a laceration to forehead that needed sutures.

A bunch of rock was discovered at XC-13 on the 3rd54 Road at 10:45. Spad # 9+30, Entry #5. Approx Length 16' Width 18' Height 3' to 5'. The rock impeded our secondary escapeway and landed on some pumps and pump cables. It was an Intake overcast bottom that the rock dislodged when the pins broke.

The employees had dropped in a little after 7am. At 7:57 the managers was preparing to drop in, the cage was at the surface of the shaft but would not drop to the 9 seam. Hydraulic Brake Valve manifold had failed. The RP evacuated the miners via the slope.

While MSHA inspector was enroute to examine the AC seals encountered a roof fall in the 7th SW submains between xc 22 & 23, which he believed impaired the ventilation to the AC seals, per citation #9045464 issued on 6-4-15 at 15:10. The operator made curtain adjustments, supported around fall. Approx 40' x 18' x8'. Spad # 18+00 to 18+90.

The operator is reporting under contest that NO ACCIDENT occurred. We are reporting to terminate issued citation # 9044123. There was no damage to the hoisting equipment, no individuals where endangered, no damage interfered with the use. The Ethernet card/PLC stopped communicating.

Roof fall was discovered in the 1G panel xc 34 (24+00) xc # 3 Entry # 2. Length approx. 50', width 18', height greater than 8'.

Over-exertion in pulling or pushing objects

Employee was dragging drive chain out of header hole and felt burning pain in his left shoulder. This is not the first incident with this shoulder, he was seen by a doctor in January 2014 for pain in this shoulder as well. X-ray reports show a mild degenerative change since the last x-ray of his left shoulder.

Roof fall was discovered 2nd S.E. Submains between xc 9-10, entry #7 (7+30 to 8+20) Length 50 to 70 feet, Width 18 feet, height above 6 feet. This fall was on HV cable and pump cable.

Employee was unloading overcast rails by hand when one came loose and rolled towards him. He stopped the rail with his left arm and twisted left knee. This resulted in a torn left meniscus and left labrum tear to shoulder. Employee did not start missing work until 5/28/2015.

Employee was getting out of scoop deck. He had his right foot on the ground and was removing his left leg to plant on the ground. When he twisted his right knee popped. He continued working until 6/11/2015 when he had surgery on his right knee.

Was riding on man trip with jacket sleeve hanging outside of ride. A tight corner was turned and the jacket became caught between the ride and rib pulling his right arm between the ride and the rib causing a fracture to his right forearm.

Was in the process of shoveling rock dust out of a ram car bucket. He got a shovel load of rock dust and twisted his back. He felt a sharp pain in his back.

A rock miner was hung up in some mud, he was in the process of putting wood under the miner. He had the remote box laying beside him, while pushing the wood under the miner he rolled over and hit the remote box causing the stab jack to mash his left hand.

Walking through door on bottom, fell through door and sprained left foot/ ankle.

Struck by falling object

While putting up outside pin in #8 left a piece of draw rock fell out and deflected off of the canopy and fell striking him on his right foot.

Injured was standing at the 3rd to last row of bolts moving the miner from the 4th lift back to the other side to clean up the cut. The tail was swung over towards the operator, he thought the tail was straight. He turned to flag a car when the tail pinned him against the rib, and the miner drug him down the rib to inby the 2nd to last row of bolts.

EE was reaching over getting pins off of the bolter. The operator moved the bolter without ee knowing resulting in the bolter running over his right foot and causing an abrasion to his right leg, that required sutures to repair.

Employee was attempting to get his steels unclogged, by striking his steels with a 1 foot wrench. When he struck the steels, the wrench bounced back and hit him in the mouth. This resulted in a chipped tooth and 4 stitches in his bottom lip.

Employee was adjusting a pin to install and felt pain in arm and wrist. Employee did not notify about the illness until 11/20/2014. He then went to the doctor on 11/21/2014, where he was diagnosed with tendonitis on his right forearm. After a follow up he has been scheduled for physical therapy.

A employee had came out of the mine and cut some dock chain. As he was walking on the cage to re-enter the mine, one of the chains fell down the shaft striking the hoist control wires. At 1:39 notified MSHA, was advised it was going to take approx. 3 hours to repair wiring.

Employee was in the process of building a brattice when some of the block became dislodged and fell, striking his left middle finger. This resulted in 4 double stitches.

Struck against stationary object

Employee was trying to break loose a fitting on a rotation hose with two pair of channel locks. When the fitting broke loose, he hit his right little finger on the boom resulting in a fracture.

Struck by falling object

Employee was kneeled down talking to his supervisor, when a head became dislodged from the roof and struck him on the back lower part of his leg. This resulted in a fracture to his tibia and fibula.

Unplanned roof fall occurred in 2nd South East Submains, XC 90, Entry # 3, (26+20). The fall was on a pump discharge line and pump power cable. It was approx.: 20' long x 17' wide x greater than 6'.

EE was washing the road grader when his feet became entangled in the wash down hose causing him to trip and fall on his right knee. This resulted in ee not being able to return to work on his next scheduled shift.

A unscheduled roof fall was discovered at 3:54 AM on the Slope Belt 1st North Mains. spad (1+60) XC # 3 Entry # 4. Approx Length 40' x width 18' x Greater than 6'. This fall was on the belt line entry, that was utilized as our secondary escapeway.

Contract Employee was in the act of splicing a belt at 3 E Header. The employee was cutting toward himself when the knife slipped cutting him across his left upper leg. Contract Employee received five stitches at the Elk Creek nurses station and returned to work.

Fall was discovered in the 3rd North East spad (45+00) XC# 57 Entry # 7. Fall approx 35'in length, 19'in width, and 6' in height. High Voltage cable was under the fall but never knocked the power.

A rock fall was discovered in the 1st N.W. Mains between spad #s (68+49&69+39), XC'S 86&87 in entry # 3. This fall was blocking our secondary escapeway. Approx Length 40', Width 18', Height 7'.

Employee was unpinning sub and rectifier from a tractor, the pin was in a bind so he asked the operator of the tractor to move backward. When the tractor rolled backward the pin caught the employee's right hand causing a laceration to his hand. This resulted in six stitches to repair the hand.

Employee was unloading steel jacks off of a supply trailer, when he picked one up it slipped in his hand and mashed his left middle finger that required stitches to repair.

Roof fall was discovered by examiner at approx 2:15. He then contacted the RP who traveled to the location 1st NW Mains between XC#126-127,spad #104+37 to 105+27. Once the RP arrived investigated the fall, collected the information needed for the questions ask per MSHA. He contacted outside to make the call at 2:40.Approx Length 35',Width 19',Height unknown.

Employee was getting some supplies and tools out of a planetary box when he laid his left hand on the side of the box while the lid was open. All of a sudden the lid on the box fell striking his left hand causing a fracture to his hand. The doctor released him with no restrictions and he is wearing a flexible splint.

Employee was reclaiming some belt framing. He stepped off of a supply trailer onto the ground and twisted his left ankle. This resulted in a sprained left ankle, that did not allow him to return to work.

Was backing miner out of face of #6 entry. Employee was walking when his left knee popped twice. He stated that he did not step in a hole or on a rock. After getting the results of the MRI the report states that it is degenerative in nature.

At 6:42 the yard man noticed some liners on the yard. He reported to the hoist man witch immediately shut the hoist down. Inspected the hoist and found the liners had came out of the lower sheave wheel. We elected to keep the hoist down. Mechanical it could run and safely. We did not want to take a chance on damaging our ropes or slinging any other liners out.

EE had just installed a pin that was bent in the roof. He went ahead and finished his row of bolts. When he was finished he went to load more pins into his tray, when he turned around he ran into the bent pin causing a laceration to his nose that required 3 stitches.

Walking down a belt line and stepped in a hole and fell and twisted his hip and lower back. This was on 2/24/2014 it was a reported only accident. On 3/5/2014 he went to see a Dr. an x-ray was completed and the results were negative. He was still having some pain so a MRI was completed on 3/25/2014 that revealed a fracture to his hip. Placing him off of work.

EE was getting ready to set over to finish cleaning up 9 right, when a rock fell out between the rib pin and the rib striking him on his left shoulder and left lower leg resulting in fractures to the tibula and fibula in his left leg.

Filing this 7001 under protest. Operator after an investigation does not feel an accident nor injury occurred which was claimed by employee. Completing this 7001 to terminate a B-Order issued on 7-21-14 by MSHA, original Citation# 4278698 on 6-26-14.

Employee was struck and rolled down the coal rib of the # 2 entry by the ATRS of the DBT bolter on # 1 unit right side. Employee had been hanging curtain prior to the bolter tramming and he was trying to get to the opposite operators side of the bolter when struck. He was not seen by the operator.

Knocking a piece of metal off of a sub in the yard with a pry bar. The pry bar slipped mashing his left pinky finger between the pry bar and the sub. This resulted in a laceration to the pinky that required 7 stitches to repair.

A rock fall was discovered by a pump man in the 3 V panel. The fall was on the cable leading to the pump. 3 V panel (3+70) Entry # 2. 19'in length x 19' in height x greater than 6'.

Examiner found a rock fall located 4th South West Submains Outby Unit #1 in the primary escapeway. (22+60) XC # 30, Entry # 4. It was Approx 35' in Length, 19' in Width, Greater than 6' in height.

EE was helping mechanic make a splice in a car cable. When he was cutting the outer jacket off with his belt knife, the knife slipped cutting his left hand resulting in a laceration that required stitches.

The section foremen on unit #1 called out at 7:28 a.m stating he had a fall inby his tailpiece located in the 4th south west submain (spad 21+20) xc # 28 entry # 4. No one was injured, he did have a change in ventilation. Called and reported to MSHA 7:30 a.m. Contacted state agency at 7:36 a.m. Approx size 19' in length x 19' width x 6' in height or greater.

Employee was rolling up a 1 1/2 in. water hose from filling up the water cart when it flipped up striking him on the right side of face and eye. This injury resulted in lacerated sclera.

A unplanned roof fall located in the intersection of the 1st North West Main. XC-154, Entry #7. It was approx 22' in length x 22' in width X greater than 8' in height. There was pump line and cable in this intersection.

Employee was hanging some curtains on #2 unit. He made a examination of the roof, and began to nail the curtain to the pin board when a rock fell and hit the outside of his right foot. He went to ER and had x-rays completed that showed negative findings of a fracture, he is off right now due to swelling and pain in right foot.

Employee was walking into 3D header and stepped hole twisting his right knee, on 10/15/2013. He went and had x-rays completed on 10/15/2013 and had negative findings. He then saw a orthopedic surgeon on 10/31/2013, which he placed him off of work until 11/04/2013.

Roof fall was discovered in the 7th North West Submains, XC-17, Entry#3, Spad 14+50. This was the intersection of a neutral entry, non-escape way. Approx size 30'in length x 30' in width x greater than 7' in height. There was a pump, barrel, discharge line, and power cable in that intersection.

Moving bolter, lowered ATRS and a rock fell out and struck EE on the rt shoulder. On 3/21/2014 EE was installing a pin in the roof when he felt a pop in his rt shoulder. EE stated that his shoulder has not been the same since 10/04/2013 accident that was a reported only. He went to the Dr. on 3/24/2014 and started missing work.

Employee was spotting timbers to be set. One of the timbers fell off of the trailer, the employee picked the timber up and twisted to toss the timber over to the rib when he felt a pain in his back. The injury resulted in a strained back. The employee is still off of work waiting on a follow up Dr. appointment.

The miner was moving into the face while the employee was rehanging a flapper on a backup curtain, when his left ankle got tangled up in the miner cable causing him to twist his left ankle. The twisted ankle resulted in a soft tissue injury which placed him off of work.

Employee was putting up a pin when the pin board smashed his right little finger between the pin board and the bottom of the canopy.

Employee was setting up to put up first pin, when he raised his canopy the collar for the second stage of the jack went up & it caught his pinner steel raising it up and putting it in a bind. When the employee grabbed it it sprung loose catching his finger between steel and pin tray. Causing a laceration to his left middle finger, requiring 5 stitches.

At approx 8:05 a roof fall was discovered in the 3 W Panel, XC # 13, between entries #3 and #4. 20' length x 18' width x approx 6' height.

EE was on a new set up pulling cables, a hauler pulled a shuttle car into a xc to be spotted. He went to unpin the tongue from the car, but the pin was bent so he went to the hitch on the hauler to try and unpin it there when he tried to pull the pin out the car rolled causing the pin to move mashing his Lt ring finger between the pin and the counter weight of the hauler.

Employee was sitting on a mantrip with his right leg outside of the mantrip. The water car was being backed into entry and Employee was not seen, resulting with right foot being run over by water car.

Employee was removing the hitch from the tractor when his hand slipped and the hitch mashed his right ring finger which resulted in 8 stitches.

Employee was spotting a bundle of pins with a low trac operator he was placing a pin board under the bundle of pins. The board shifted and the pins mashed his right thumb resulting in 4 stitches.

Roof fall was discovered in the 6th North West Submain Headings, xc-28, Entry # 7, (20+80). Approximate size of fall was length 25', width 18', and height 6' plus.

Employee was moving power, his hand was inside a cat head when it got hung jerking out of his hand, breaking his finger nail. Employee was taking to urgent care, it was cleaned, x-rayed, and bandaged. He was referred to an orthopedic surgeon to repair finger nail. Employee seen the ortho on 01-09-2013 and returned to work that night.

EE was reclaiming crib block from a pile underground onto a trailer. He was stacking them on trailer & when he reached to get another, his helper threw another smashing EE's right ring finger. His finger was broken & had deep lacerations that required surgery.

EE was pushing a roof bolt into the hole when he felt a pain in his lower back. He was taken to the Hospital and has a strained muscle. He began missing work on 12/13/2012.

EE was pulling pry bar off of his scoop and caught his hand between the end of pry bar and coal rib resulting in accident. He received 3 sutures in little finger on his right hand. He was back to work on 12/10/2012.

EE was grinding key stock on a wheel unit when the grinder kicked back striking him on his right index finger causing a laceration.

EE came in early to get belt framing for a belt lay on 3rd. shift. There was a piece of belt in front of trailer he was trying to get out. He had a rope dog hooked to the belt & when he started to pull it his hand got caught between chain & ride resulting in injury to his left hand.

Employee lifted a rock that was in the way of the crib he was going to build when he felt a pull in his stomach, resulting in hernia. He RTW on 11-19-12 and was hurting at the end of shift. He did not work on 11-20-12. He doesn't know when he will return to work.

Men were fixing to get on cage when they noticed a distorted Hoist Rope. The Cage was shut down until inspection and repairs can be made.

Employee was walking belts when he attempted to remove some belt fibers that had wrapped around edge of roller. He pulled on fiber & when it got tight the roller grabbed fiber & pulled his thumb into roller resulting in injury.

EE was pulling a load of concrete block with a jeep. He had his hand on top of jeep and when he turned, the block pinched his hand between the jeep and the block.

Employee was in the 11 seam, Unit #2, number 9 entry. Employee had placed rib pin and had swung in to place center pin, he noticed some loose rock so he began to scale the loose rock down. A piece of rock he scaled struck the canopy of bolter then bounced and struck him in the face causing a laceration.

EE was heading outby from unit sub when he tripped over cables or rock. He strained his achilles tendon in his right ankle.

Employee was in the process of removing a rock that had hung in 2 C Header. He heard a ""pop"" in his elbow on 8-13-12. He had a MRI on 8-14-12. 8-15-12 he had his follow up on MRI. 8-16-12 MRI revealed torn bicep tendon he was scheduled for surgery on 8-27-12. He started missing work on 8-28-12.

Examiner was traveling with MSHA inspector when they found a rock fall in the 2NE Parallel at spad # 23+80 (xc34) in the No.3-Intake entry. They were able to set over in the No.4-entry.

EE was putting tools on Tool Slide, when a Golf Cart pulled in xc & struck EE on right leg, just below knee. The drivers dinner bucket had slid behind the brake pedal, & driver couldn't stop Golf Cart resulting in accident.

Employee had drilled his first steel up getting ready to drop to add his second steel, when a rock fell down the steel, striking his boom, breaking in half, and then striking him on the right leg above his knee but below his hip. Employee finished his shift. The next day it was stiff, resulted in a deep muscle bruise.

Post accident drug screen was positive for THC, we sent him for a secondary confirmation test, the results did not come in to 4/30/12 which was negative. He returned to work on 5/1/12 The employee was ordered physical thearpy on 4/28/12.

In the #9-seam, employee had completed the drilling process for his rib pin. He had put glue in hole and was in the process of putting his pin in the hole when the pin lodged in some draw rock on mine floor. He jerked on the pin & when he did, he lacerated his bottom lip.

In the #11 seam. Employee had just completed returning tools to the tool slide. Employee then sat down on his golf cart getting ready to leave when a rock fell from top striking his hard hat knocking it off resulting in a laceration to the top portion of his forehead.

Employee was changing and tightening hoses on the feeder when he felt a sharp pain in his shoulder, he reported it on 2/22/12. On 3/2/12 his shoulder was still causing him pain and discomfort so he sought medical attention. Nothing was discovered. On 3/9/12 at this visit they restricted his duty causing him to start missing work on 3/12/12. Injury still unknown.

Employees feet was resting on the outer bottom frame of the ride. When the mantrip made a left turn, his right foot was caught between the rib and the frame. Resulted in a fracture of the big toe and portions of his other toes avulsion. He had surgery which resulted in his big toe, second toe, and third toe being partially amputated and grafted.

Employee had been working on a piece of equipment and had completed repairs. He started running to another piece of equipment that was down the same time the first piece of equipment was down. He caught his back on a pin and plate that was damaged causing a laceration.

The injured employee was installing a belt roller when he felt a pain in his back. The injury resulted in a sprain to his lower back. He went to a physician on 9/24/2011 and was given a full release. He continued to work but was still experiencing some pain and on 9/29/2011 went to another physician who recommended physical therapy. On 10/11/2011 he began physical therapy.

#11 Seam, DBT Bolter, injured operator was placing starter steel into the top, when a piece of rock or coal fell striking him on his left index finger. The injury resulted in 9 stitches to his index finger.

He sought orthopedic consult on 8-5-11. He started physical therapy and was unable to return to work do to restrictions. Notified by workmans comp that his claim had been approved on 8-18-11.

Employee was dropping H/V down to reclaim and while in the process of letting go the H/V hanger caught him between his pointer and middle finger on his right hand causing a laceration that received 12 sutures.

The injured employee was reaching for a roof bolt when a rock fell striking the top of his hand pushing it downward. His hand was smashed between the rock and bolt causing the laceration. He received 22 sutures in the fatty tissue between his index finger and thumb.

The injured employee was adjusting a jack pipe for the tailpiece. In the process of doing this, the jack pipe fell and struck him on his back. This resulted in a fracture to a rib.

While in the process of unhooking a shuttle car from a wagman tractor, the injured employees left index finger was caught between the wagman and a hitch pin. This resulted in a laceration to his left index finger in which he received sutures. The employee went on a scheduled vacation the next week, so this is the reason for the return to work date.

In the process of reclaiming high voltage cable,the high voltage cable was being pulled with a cloth rope and shive wheel. The injured employee put his hand on the rope while it was being pulled and his right little finger was pulled into the shive wheel. This resulted in his right little finger being cut off at the last joint.
