Employee was working out of a bucket truck swinging a sledge hammer. When he swung the hammer to strike a pin he accidently pinched his left index finger between the handle of the sledge hammer and the lip of the bucket truck at the same time he struck the pin with his bottom hand holding the handle to the sledge hammer. He fractured his left ring finger.

Scrapping floor with scrapper (hand-held putty knife), had hand in front of scrapper - no gloves on - scrapper slipped, cut skin off tip of finger (index) and cut middle finger.

Work: Remove drag hitch extension pin using welded pulling eyes on bucket and pin with a 2 ton come-a-long. Employee was cleaning dirt from pin while it was under tension. Weld broke and he was struck in the face by the hook, clevis and eye. Laceration of upper lip required 8 stitches, with cleaning of abrasions to nose and cheek.

EE was loosening a hydraulic filter housing on an 830E haul truck company # 2039 when his chain wrench broke and the chain grazed (lacerated) his left cheek. He was taken to the campbell County Hospital & received 9 sutures to close the wound. Returned to work next reg. scheduled shift.

EE WAS ATTEMPTING TO PULL A METAL CHOKER OFF A RIPPER SHANK ON A 854 DOZER WITH A PRY BAR. THE PRY BAR SLIPPED OUT OF THE LOOP WHEN HE WAS APPLYING PRESSURE AND HE ROLLED HIS RIGHT ANKLE OVER AS HE FELL. EE FRACTURED HIS RIGHT ANKLE. HE WAS TRANSPORTED TO THE HOSPITAL AND RECEIVED SURGERY TO REPAIR THE ANKLE.

PRESSING A PIN INTO A DOZER FRAME WHEN THE BOLTS STRIPPED OUT OF FRAME, CAUSING THE ASSEMBLY TO FLY BACK, PINCHING FINGER BETWEEN THE BRACE AND JACK HANDLE. THIS RESULTED IN A FRACTURE OF THE FINGER TIP.
