Person was removing an inner race off of a Pickup truck #120 Isuzu. He was removing the bearing race with a heel bar and a hammer. As he struck the heel-bar a piece of the race broke off and it penetrated his outer left index finger between the hand and the first knuckle. After the doctor removed the small piece of metal, it resulting in 4 stitches.

Employee was using a 3/4 ton come-a-long to remove a 40' roll of 60" mine belt from a 975. The belt was lifted high enough for clearance, and then was being lower to the ground when the come-a-long released & free wheeled until the belt hit the ground. EE was holding onto the slack end of the come-a-long and his arm was jerked upward thus staining his left shoulder.

The employee was attempting to remove a plastic plug from a valve bank he was replacing. He grabbed his razor knife and tried to pry the cap out. The knife blade cut through the cap and also cut his left thumb.

The employee was letting down a track jack that was holding up a take-up truck. As he applied pressure to the jack handle it slipped hitting him in the left side of the head resulting in a small laceration.

The employee was removing cables from the #1 shield in prep. for extended push. He was cutting down zip ties and as he was performing this duty he cut through his left arm just above his wrist. Received 5 stitches

Employee was holding a shuttle car cable in his left hand while preparing the cable for a splice when his razor knife slipped and cut the palm of his left hand. 7 stitches were required to close the wound.

The employee was in the process of retrieving 6" plastic aqua pipe from the tailgate entry of the long wall (16th East). He hit the plastic couple with an ax in an effort to break the coupler when a small piece of the plastic coupler broke off and struck him in the forehead just above the left eye. 3 stitches were required to close the wound.

Employee was using a sledge hammer to remove a solid block stopping in 14th east return, when he felt a sharp pain in his lower back. He reported the incident and 5 days later (8-5-12) he went to the doctor who kept him off from wok for 4 days.

Employee was assisting the electrician to make a splice in a pump cable. Employee was holding one end of the cable in his left hand while prepping the end of the cable. As the razor knife cut thru the outer jacket if the insulation, it also cut thru his glove and cut the index finger on his left hand. 5 stitches were required to close the wound.

Employee was attempting to recover a 3' X 3' ventilation regulator from an unused overcast. He was using a razor knife to trim away the foam mine sealant when the knife slipped and contacted his right forearm, resulting in a laceration which required 5 stitches.

Employee was securing chain link fence to square sets with tie wire. While attempting to tighten the tie wire, his dykes slipped off of the tie wire and hit him in the mouth, requiring 6 stitches.

The injured employee was watching as another employee was using an ax to break a 6" plastic aqua pipe coupler when a small piece of plastic broke off of the coupler and hit the injured employee in the upper lip. 3 stitches were required to close the wound.

Employee was working on shield #71 and using a 4 pound hammer to clean off the areas to be worked on when a small metal fragment became embedded in his right eye. Employee was wearing prescription safety glasses. The foreign object was surgically removed.

EE was using 2 prybars to reposition the engine cover so that the mounting brackets could be rewelded. As EE hit the cover with a hammer, 1 of the pry bars flipped out and stuck him in the face. Resulting in 3 stitches to close the wound.

EE was attempting to remove a broken fitting from a spray block on the miner. As he attempted to hit a pick hammer into the fitting to extract the broken fitting, he missed the pick hammer and struck his left hand with the ax. Resulting in a hairline fracture of the left thumb.

After EE had drained the oil from the tailgate haulage and was attempting to tighten the drain plug, the socket slipped off of the drain plug and the ratchet stuck him in the forhead, resulting in a laceration which required 6 stiches.

While the EE was attempting to remove a section of mono rail, he had removed the roll pin from the out-by end of the mon-rail. As he cut the links of the belt chain with a pair of bolt cutters, the section of mono-rail dropped and smashed his ring finger on his right hand, which required stitches.
